Plus récemment, on a démontré qu'un hôte de l'estomac appelé HELICOBACTER PYLORI est en corrélation avec la gastrite, l'ulcère et la néoplasie de l'estomac. D'après les études du St George Hospital de Londres, et de Anna Morgando dans le Lancet du 27 mai 1995, l'hélicobacter serait également toujours présent dans l'estomac d'un grand nombre de patients atteints d'un infarctus du myocarde. Procédons par étapes. Certaines conditions doivent être réunies pour que cela se produise: la présence de souches de l'Hélicobacter Pylori, productrices de la cytotoxine vacuolisante, des années d'infection et d'absence d'antioxydants naturels dans les cellules de l'estomac. En revanche, il est plus difficile d'expliquer l'association entre Hélicobacter Pylori et infarctus (myocarde, cerveau, muscles ...). Selon certaines statistiques, la fréquence des maladies coronariennes chez l'homme atteint d'un ulcère peptique est de 4 à 6 fois plus grande que celle des personnes sans ulcère. On peut constater la même statistique en ce qui concerne l'association entre l'attaque cérébrale et l'Hélicobacter.

Extractions: Stephen Goldfinger, M.D. Helicobacter pylori is a spiral-shaped gram-negative bacterium that thrives within the acid milieu of the stomach, presumably because of its urease activity, which generates ammonia from endogenous urea to maintain a neutral microenvironment. Within the past 10 years it has become clear that H. pylori plays a critical pathogenic role in type B gastritis and peptic ulcer disease, both duodenal and gastric. Epidemiological studies also suggest a causal relationship between H. pylori infection and gastric adenocarcinoma and gastric lymphoma. The prevalence of H. pylori increases with age in the US, where it is uncommon below 40 ( < 20%) and then climbs to a plateau of 50% by the age of 60. In developing countries with poor sanitation a very high prevalence occurs even in childhood. Spread appears to be person to person by fecal-oral route. Clusters occur in families and among children in orphanages and mental institutions with crowding and poor sanitation. H. pylori penetrate the mucus layer of the stomach to attach to specific binding sites on the surface of gastric endothelial cells; they do not invade the gastric mucosa. When found in the duodenum, H. pylori are confined to the surface of metaplastic gastric epithelia produced by inflammation.

Extractions: Zinc-dependent matrix metalloproteinase (MMP) enzymes digest the connective tissues surrounding cells. This process helps keep the spaces between cells neat and tidy and allows cells to multiply. When things go wrong, however, the MMPs eat away at tissues surrounding joints causing inflammation and destroying the joints leading to the often-crippling pain of osteo- and rheumatoid-arthritis. Michelle Browner of Roche Bioscience in Palo Alto, California and her co-workers have recorded the X-ray crystal structures of two MMPs in which they had loaded inhibitor molecules - diphenylether sulfones - that stop them working. The X-ray structures, which reveal the positions of every atom in the molecule, can be used to produce an accurate computer model of the enzymes. According to Browner the models demonstrate that how well a molecule blocks the activity of the MMPs - human collagenase-1 and collagenase-3 - depends on the size and shape of the 'pocket' in which the molecules fit. This pocket is long and open in collagenase-3 so diphenylether sulfones, can fit. But, collagenase-1 has a smaller pocket and the bulky diphenylether group of the compound simply cannot squeeze inside properly. The researchers have developed inhibitor molecules that block different MMPs to different degrees, based on their results.

These are known as stomach and duodenal ulcers, collectively known as peptic ulcers. You may notice no symptoms although you have an ulcer, but frequently they are associated with indigestion or pain in the upper abdomen or even lower chest. This pain may be worse before or after eating and frequently wakes you in the early hours of the morning. The pain may go to the back or sometimes be perceived elsewhere. If an ulcer bleeds you may vomit blood or partially digested blood (which looks like coffee grounds) or pass black stools (melaena), which contain changed blood, when you go to the toilet. If any of these happen call the doctor immediately (even out of hours). Many factors make you more likely to have an ulcer. Your doctor may treat you on the assumption of the diagnosis, but may well arrange either a barium meal X-Ray or a visual inspection using a fibre-optic tube passed down the throat (upper gastrointestinal endoscopy). The endoscopy is preferred by the experts as they can see the lining of the stomach and duodenum and take biopsies which allow them to rule out more serious conditions as well as looking for the presence of Helicobacter pylori.

Extractions: Prof Chris Hawkey Dr Yash Mahida Keith Bishop Lay persons guide to Helicobacter pylori: Helicobacter pylori is a bacterium present in the stomach of about 30% of people in the U.K. and most people in developing countries. Usually it lives harmlessly in the stomach, like many other gut bacteria, but in some people it causes stomach or duodenal ulcers. If the bacterium is eradicated, these get better and do not recur. Infection with certain strains of the bacterium slightly increases the risk of developing stomach cancer, but it is thought that infection may also protect against cancer of the oesophagus (foodpipe). Research in the Institute of Infections and Immunity focuses on why some infections result in disease whilst the majority do not. In particular, we are investigating a toxin produced by some strains, how proteins (including the toxin) are produced by the bacterium, and what makes the bacterium produce factors which cause damage in the stomach and duodenum. We are also interested in genetic differences between individual strains of Helicobacter pylori and how H. pylori
