"Hematuria" means blood in the urine. So if you have microscopic hematuria, you have red blood cells in your urine, but you can't see the blood when you urinate. What are some common causes of microscopic hematuria? Here are some common causes of blood in the urine: How will my doctor check for microscopic hematuria? Your doctor will usually start by asking you for a urine sample. He or she will test your urine (urinalysis) for the presence of red blood cells. Your doctor will also check for other things that might explain what is wrong. For example, white blood cells in your urine usually mean that you have an infection. If you have blood in your urine, your doctor will ask you some questions to find out what caused it. If the cause isn't clear, you may have to have more tests. You might have an ultrasound or an intravenous pyelogram (this is like an X-ray). A cytoscope or endoscope may be used to look inside of your bladder. These tests are usually done by a urologist.

The source of the bleeding can be located anywhere along this tract: the kidneys, the ureters (the tubes running from the kidneys to the bladder), the prostate gland (in men), the bladder or the urethra (the tube that carries urine from the bladder out of the body). The bleeding may happen only once or it may be recurrent. It may or may not be accompanied by pain or discomfort during urination, and it can indicate different problems in men than in women. The cause may be as routine as an infection or as serious as cancer. Whatever the circumstances, hematuria should be regarded as a danger signal demanding immediate attention. The development of hypertension, diminished renal function or proteinuria > 1 g/ 24 h indicate a poor prognosis. Idiopathic hematuria: the diagnosis is established by normal findings on biopsy. Long term follow up is necessary to exclude Alport syndrome. The development of diminished renal function, proteinuria or hypertension demands a second biopsy.

Hematuria was confirmed by the referring vet who found a large quantity of red blood cells on sediment examination of a voided urine sample. Treatment with cephelexin and then enrofloxacin has not been associated with any improvement. Five days ago, the urine appeared to be pure blood and some clots were noted; the dog's gums were very pale and capillary refill time was prolonged. The owner has not noticed any straining, pollakiuria, or polyuria/polydipsia and the dog's appetite is normal. The dog lives indoors but is allowed outside. On presentation, physical examination reveals a mildly enlarged prostate gland that is symmetrical and non-painful. The first step in evaluation of hematuria is to determine the source of hemorrhage (ie, upper urinary tract, lower urinary tract, or genital tract).
