Systemic Histoplasmosis is caused when, after inhaling the histoplasmosis spores, the spores under certain temperature conditions, turn into yeasts. The spore, which is called Histoplasmosa Capsulatum, isfrom a fungus which can grow in soil and its grouwth is aided by bird (water fouwl, chickens, swallows, etc) and bat droppings. These scars, which in the case of Ocular histo are what cause the sight degeneration, usually later in life. Ocular Histoplasmosis occurs when abnormal blood vessels grow through the healed histo scars and into the interior of the eye. This condition, which is secondary to Ocular Histoplasmosis is called Sub Choroidal Neo-Vascularization. This may cause bleeding or increased scarring. If the scars are located close to the macula or center of vision, which is usually the case, reduction or loss of central vision may occur.

Information Histoplasmosis Basic Information After the spores have been inhaled into the body, they develop into their pathogenic yeast form. They can reproduce and spread through the blood, usually accumulating in the lungs. A person with a strong immune system will develop a nearly complete lifelong immunity within two weeks of exposure. The main factors that determine the course of the infection are the immune system of the infected person and the extent of exposure. For this reason, people who are HIV positive are particularly at risk of a severe case of histoplasmosis if infected. Symptoms In episodes of histoplasmosis where the infected person has a healthy immune system, there are often no symptoms. This is referred to as "acute self-limited histoplasmosis". If the person does develop symptoms, they are most often flu-like in nature, such as fever, cough, fatigue and chest pain. Symptoms usually appear ten days after exposure, and subside without treatment after a few weeks. When symptoms disappear, it is unusual for there to be long-term ill effects. It is important to realize that the severity of the symptoms depends on the duration of exposure and the immune system of the person. A person climbing around for a long time in a Histoplasma capsulatum-contaminated cave will develop a much worse case than a person who walked quickly through a similarly contaminated field. Also, someone with AIDS would develop a much more severe illness than a healthy person, even if both persons were exposed to the fungal spores for the same amount of time.

In the United States, it is most common in the southeastern, mid-Atlantic, and central states. The lungs are the portal of entry for this infection. Extractions: Peer Review Status: Internally Peer Reviewed The appropriate evaluation of a lung nodule is a significant clinical challenge. There are many potential causes for lung nodules, however as clinicians we are most concerned with the separation of malignant and inflammatory disease. This is an important distinction because the diagnostic and therapeutic approach differs significantly for the two disease categories. Infectious causes of lung nodules will differ among different geographic areas. In the great river valleys of the central United States, the most common cause of infectious lung nodule is Histoplasma capsulatum infection. The organism Histoplasma capsulatum was first described in 1906 by Dr. Samuel Darling in the Panama Canal Zone. The name referred to his belief that the organism, found in reticuloendothelial cells, was an encapsulated plasmodium. The name persists despite the fact that it is now known to be a non-encapsulated yeast. The organism is found world wide, but in the United States it is endemic to the Mississippi and Ohio River valleys. High concentrations of the organism can be found in bird roosts, caves inhabited by bats, school yards, areas with rotten or decaying wood, and chicken coops. Human activities in and around theses regions may lead to infection. In all cases, the lung is the point of entry. In the 1940's a skin test was developed by Christie and Peterson for histoplasmin, and it was soon found that vast numbers of people in endemic areas had positive tests but were asymptomatic. Shortly thereafter large numbers of people with calcified lung lesions on radiographs and a negative tuberculin skin test were found to have positive histoplasmin skin tests.

Infection results from inhalation of spores that are disseminated into the atmosphere when soil and or organic materials containing the infectious material is disturbed. Infection is common while overt clinical disease is not. Individuals with preexisting, immune-compromising conditions are at higher risk of developing clinical disease after exposure. The number of spores inhaled and hosts susceptibility play roles in the development of clinical disease. The clinical disease may vary from a mild respiratory illness to a chronic disseminated disease or a chronic pulmonary disease resembling tuberculosis. Generally, the disease is mild or unrecognized as histoplasmosis; rarely does it develop to a severe progressive disease. Indiana is part of the eastern and central United States where exposure and infection is prevalent. In 1999, 75 confirmed cases were reported for a crude incidence rate of 1.27/100,000. Figure 1 shows the county of residence for reported cases for 1999 and Figure 2 shows the county of residence for cases for the past five years. In 1999, Marion County reported 28% of the cases, but only has 14% of the population. Crude incidence rate for Marion County in 1999 was 2.6/100,000, which is approximately double the state rate. When bird or bat guano dries (becomes friable), the organisms therein can travel as airborne contaminants. Inhaling the dust from infected droppings or feathers can result in pulmonary infection. Histoplasmosis is one such disease. The short story is that some rodents are infected with a type of hantavirus that causes (HPS). Deer mice (plus cotton rats and rice rats in the Southeastern states and the white-footed mouse in the Northeast) are the rodents carrying hantaviruses that cause hantavirus pulmonary syndrome. These rodents shed the virus in their urine, droppings and saliva. The virus is mainly transmitted to people when they breathe in air contaminated with the virus. This happens when fresh rodent urine, droppings or nesting materials are stirred up. When tiny droplets containing the virus get into the air, this process is known as "aerosolization." There are several other ways rodents may spread hantavirus to people: If a rodent with the virus bites someone, the virus may be spread to that personbut this is very rare. Researchers believe that you may be able to get the virus if you touch something that has been contaminated with rodent urine, droppings or saliva, and then touch your nose or mouth. Researchers also suspect that if virus-infected rodent urine, droppings or saliva contaminates food that you eat, you could also become sick.
