Arch Intern Med 2000;160:1160-1168 Abstract BACKGROUND: The role of hyperinsulinemia as a cardiovascular risk factor is controversial. We studied whether hyperinsulinemia is independently associated with increased cardiovascular morbidity and mortality. METHODS: Fasting serum insulin level and other cardiovascular risk factors were determined in 1521 men in eastern Finland aged 42 to 60 years with neither cardiovascular disease nor diabetes at baseline. Forty-five cardiovascular deaths, 110 acute coronary events, 48 strokes, and 163 any cardiovascular events occurred during an average follow-up of 9.5 years. A total of 163 cardiovascular events (45 cardiovascular deaths, 110 acute coronary events, and 48 strokes) occurred during an average follow-up of 9.5 years. RESULTS: In Cox regression analysis adjusting for age and examination years, fasting serum insulin level as a continuous variable was directly associated with the risk of cardiovascular death (P = .006), acute coronary events (P = .04), and stroke (P = .02). Men with insulin levels of 52 to 66 pmol/L, 67 to 89 pmol/L, and 90 pmol/L or more (3 highest quartiles) had 1.4-fold (95% confidence interval, 0.5-3.7), 1.4-fold (95% confidence interval, 0.5-3.7), and 2.5-fold (95% confidence interval, 1.0-5.9; P = .05) cardiovascular mortality, respectively, compared with men with insulin levels of less than 52 pmol/L (lowest quartile) (P = .04 for linear trend). Adjustment for serum lipid levels, blood pressure, and obesity reduced the excess cardiovascular mortality in the highest insulin quartile by 7%, 33%, and 67%, respectively. There were no statistically significant differences in the incidence of acute coronary events and stroke between the insulin quartiles.

So why have most people never heard of this condition? Good question, one I wish I had a good answer for, but I don't. Scientists and doctors have known for years that elevated insulin levels was deadly over the long-haul, yet few recommend actual dietary changes to their patients. Instead they let the insulin remain high, run it's course, and then put their patients on hypertensive medications, statin drugs, beta blockers, and eventually insulin itself as the pancreas literally stops producing it, worn out from the efforts.

So every day a new treatment is been examined and now oral antidiabetic drugs are in experience. Several drugs for treatment of hyperinsulinemia are investigated such as metformin, diazoxide and troglitazone. Metformin inhibits hepatic glucose production and enhances sensitivity to insulin in peripheral tissues. Nestler et al (1996) recently reported a reduction of ovarian cytochrome P450c17a activity on the amelioration of hyperandrogenism in obese PCOS patients treated with metformin. So we in my private clinic and university infertility center we tried to exam the effects of metformin in PCOS patients. The patiens who are going to be ovulated by induction are randomly divided into two groups, one group take metformin 500mg 3 times a day for 3 months ago befor induction ovulation and other group have not take this drug. Extractions: Introduction: The syndrome of angina or angina-like chest pain with a normal coronary arteriogram, often referred to as syndrome x by cardiovascular dysfunction and abnormal pain sensitivity are presented in patients. This syndrome demonstrated a spectrum ranging from normal coronary arteries to atheromatous plaque. On the other hand, the insulin resistance syndrome (The metabolic syndrome) is also responsible for a large number of CVD cases. The association between syndrome X and insulin resistance syndrome needs to be investigated. The aim of this study was to examine hyperinsulinemia as a manifestation of insulin resistance in postmenopausal women. A total of 149 post-menopausal women matched for age (48-58 years) and duration of menopause period were examined for sex hormones, fasting insulin, blood pressure, and body mass index (BMI) in this study. Subjects with clinical angina pectoris with normal (Angio-, n=50) and abnormal (Angio+ group n=50) arteriogram compared to healthy (control, n=49) subjects. Results: FSH, LH, Estradiol and progesterone concentrations and FSH/LH ratio were similar among three groups. Both Angio (-) and Angio (+) groups had greater BMI compared to controls. Results Next Data on parameters of fasting insulin and insulin response in the OGTT and carotid artery ultrasonography were available for 518 control and 513 hypertensive subjcts. Various parameters and calculated variables of fasting insulin and insulin response in the OGTT are shown in Table 2. The maximal IMT values and the mean and far-wall IMT and CPIMT values were presented above in 5.3. et al. 1985) and the area under curve (trapezoidal method), were statistically tested with IMT. The most powerful and most consistent predictor of IMT among the different insulin parameters in stepwise regression analyses was the 2-hour post-load insulin, which was used in the following analyses alone without any other insulin parameters in order to prevent multicollinearity. In the control groups 2-hour insulin had a weak non-significant positive correlation with IMT, but in the hypertensive groups it had a weak non-significant negative correlation with IMT. To determine whether an independent association between IMT and the different insulinemic measures existed, multiple forward and backward stepwise regression analyses (Table 9) were performed with different IMT parameters as the dependent variables and age, pack-years of smoking, LDL cholesterol, systolic BP and 2-hour insulin (data on serum 1-hour and 2-hour insulin concentrations, ISI, IRI and insulinemic area under curve not shown) as the independent variables.
