Extractions: Introduction, Summary and Lecture Notes Course notes 2MED module information An increase in plasma lipid is often a feature of atherosclerosis, a condition which affects the walls of arteries and which may lead to heart disease, stroke and gangrene of the extremities. Since the high lipid content of the plasma is believed to underlie these conditions attempts have been made to develop pharmacological agents which reduce the concentration of plasma lipid thereby reducing the probability of formation of atherosclerotic plaques However, the pioneering work of Brown and Goldstein on Familial Hypercholesterolemia demonstrates that there are genetic risk factors involved in this disease. The study of the hyperlipidemias and control of lipid metabolism can be used as a paradigm to demonstrate the power of combined molecular genetic, biochemical and cell biological approaches to molecular medicine. There are different types of hyperlipidemias, all of which are risk factors for developing heart disease. Other factors such as genetics, environment, habits, and presence of other diseases such as diabetes and hypertension may also contribute to the development of heart disease. Some of these factors are within our control; others are not. Studies have shown an association between high blood cholesterol and premature heart attacks. Excess cholesterol in the blood can settle in the arteries and form a plaque (a raised lesion on the inside of an artery). Over time, this plaque can build up and narrow the arteries, which in turn may clog the flow of blood. This process can begin in early childhood and may eventually result in coronary artery disease, heart attacks or stroke.

Extractions: Advanced Search Almost 13 million American adults require drug therapy to meet the low-density lipoprotein goals set by the National Cholesterol Education Program. Attempts to achieve these goals through diet and exercise are often unsuccessful. Major studies in recent years have demonstrated that statins decrease low-density lipoprotein levels, coronary events and overall mortality. Statins are the most commonly prescribed lipid-lowering agents because they are effective, well tolerated and easy to administer. Niacin has beneficial effects on all of the main lipid components, and new extended-release tablets have fewer adverse effects. Fibrates remain the most effective agents in lowering triglyceride levels and should be limited to this use. Bile acid sequestrants are seldom prescribed because of their adverse gastrointestinal effects and cumbersome administration. (Am Fam Physician 2000;61:3371-82.) C oronary heart disease (CHD) is the most common cause of death in the United States. Each year, 500,000 Americans die of this disease, and associated morbidity costs more than $200 billion annually.

Extractions: Medical research has proven that elevated total cholesterol levels in the blood result in an increased risk for heart disease. The National Cholesterol Education Program recommends that adults age 20 years or older have a cholesterol test done at least once every five years. How is high cholesterol detected? When your doctor orders your blood cholesterol test, he or she may order a preliminary assessment of total cholesterol and high density lipoproteins or HDL (good cholesterol), which can be done without fasting. Your doctor may order a complete cholesterol profile which requires fasting and includes total blood cholesterol, triglycerides, low density lipoproteins or LDL, and HDL. A high LDL level is considered dangerous because it seems to promote the deposit of cholesterol in the artery walls. HDL is considered beneficial because it helps carry cholesterol away from the tissues. How is high cholesterol treated?
