Extractions: Hypospadias Hypospadias is a birth defect found in boys in which the penile meatus is not at the tip of the penis. The meatus is the term for the opening of the penis through which urine normally exits the bladder. The incidence is reported to be 1 in 300 live male births. There is some family risk of hypospadias, as familial tendencies have been noted. Up to 14% of male siblings are affected. Hypospadias is usually classified according to the location of the opening. As the defect increases in severity, the opening to the penis will be found further back on the penis. The most severe types can have openings at the region of the scrotum and even in the perineum (the region between the anus and scrotum). Hypospadias can be associated with another defect called chordee. Chordee is a downward curvature of the penis. Both hypospadias and chordee must be repaired so that a child can have normal urinary and reproductive health. back to top Embryology: As a boy is developing in utero, the penis begins to form in the sixth week of fetal life. Two folds of tissue join each other in the middle and a hollow tube is formed in the middle of the future penis. This tube is the urethra and its opening is called the penile meatus. As the skin folds develop to form the penis, any interruption in this process leads to the meatus being located in a location further from the end of the penis. The exact etiology for this premature cessation of urethral formation is poorly understood. In addition, the etiology of the often-associated abnormal downward curvature (chordee) is also poorly understood.

The urethral opening may still be on the glans or head of the penis or it may be further down the shaft. In some men who are affected with hypospadias, there is another abnormality called chordee. In this condition, the penis curves downwards and the foreskin only covers the front of the penis. In the most severe forms of hypospadias the urethral opening is so far back it is almost in the scrotum. The scrotum itself may be small and the testes undescended (not in the scrotum, but still deep in the abdomen). When babies are born like this it can be difficult to work out what sex they are without further tests.

In the most serious cases, boys can't urinate normally. Men, who have this condition and have not been treated, may not be able to function sexually. The rate in which this defect occurs has doubled according to several studies conducted in the United States and Europe over the past 30 years. In a study in metropolitan Atlanta from 1968 to 1993, the rate increased from 18 per 10,000 births to 40 per 10,000 births. Researchers speculate that the increase may have something to do with exposure to environmental toxins. Some have suggested the increase also may be due to increased interest in detecting and reporting the condition. However, a federal Centers for Disease Control and Prevention report suggests since hypospadias not only are becoming more common, but severe cases are increasing, other explanations should be sought.

The urethral opening is not in the normal anatomic location but rather somewhere on the undersurface of the penis. Boys with hypospadias often are missing the undersurface half of their foreskin so that the foreskin forms a hood. There is often a bend (called chordee) in the erect penis. What causes it? Hypospadias results from incomplete development of the urethra. It is sometimes inherited. Other than inherited cases, the cause is usually not known. Why is it important? Hypospadias may cause deviation of the urinary stream so that the boy is forced to sit to urinate. Additionally, sexual function may be hampered by the location of the urethral opening or by the bend in the penis. Can it be corrected?

There are a number of separate alterations from normal, that when present in various combinations, are the spectrum of the disorder called Hypospadias. Basically, hypospadias means that the opening of the urethra,(meatus) is not in its normal position at the tip of the glans or head of the penis. This urethral defect is most often combined with an abnormality of the prepuce (foreskin) which is often opened and looks like a hood over the end of the penis. The urethral meatus can open, in this disorder, anywhere from a slight dislocation on the glans, down to the perineum behind the scrotum, which in that situation the scrotum would be separated. The glans (head) is often opened and flattened, which will later require repair as part of the corrective procedure. The urethra can also be patulous or gaping which is another variable. The corpus spongiosum, the spongelike tissue around the urethra is often not developed properly, is scarred or is absent in that part of the urethra involved. There is a "urethral plate" or posterior mucosa present in most cases. Chordee, or curvature of the penis, is often combined with the urethral defect thus giving the penis a shortened appearance. The farther the meatus (opening) is from the end of the penis, usually the more chordee that is present. The curve is most often ventral or curving downward. It can also be lateral, and in addition, various degrees of turning in the long axis can also be present.

During pregnancy, the urine channel starts developing as a groove between two ridges. The ridges grow and fuse together on the under side of the penis to make a tube. This tube closes from behind the scrotum (the sac holding the testicles) out to the tip of the penis. As it forms, the tube leaves a little line on the skin which you can see on the scrotum and the penis. If the closing over of the tube stops before it reaches the end of the penis, hypospadias results. No one knows exactly what causes hypospadias, but we do know that it is not caused by anything either parent did during or before the pregnancy. Hypospadias can occur in some family lines. Associated findings in boys with hypospadias are a downward curvature of the penis with erections (chordee) and a "hooded" foreskin, where the foreskin is present along the top of the penis (dorsal penis), but deficient along the bottom of the penis (ventral penis). The surgical treatment of hypospadias is is designed to position the meatus at the tip of the penis (the normal location), and correct any downward curvature. The choice of surgical technique is made on an individual basis. TREATMENT OPTIONS Treatment options include the use of the redundant dorsal skin as a flap to augment the deficient uretha, incision of the urethra dorsally and ventral closure (Snodgrass repair), and other techniques. At our center, we often manage patients with a history of prior repairs that have failed and who have urethral openings proximal to the normal location without available local skin for use in repair and/or strictures of the reconstructed urethra. These patients generally require complex reconstructions that may include the use of buccal mucosa grafts or staged repairs with a split thickness skin graft. However, it is our experience that even complex repairs are associated with a high success rate.
