Abdominal pain, cramps, gas, bloating, diarrhea and constipation are among the symptoms. For years, many thought it was a psychological condition, rather than a physical one. But the discomfort and inconvenience that result from the gut-wrenching pain of irritable bowel syndrome (IBS) are very real. "It feels like you're giving birth over and over again," says Hope Marcus of Miami, Fla. Indeed, people with IBS can't stay out of the bathroom or their stomachs feel like they're tied in knots. Their bowel movements are either frequent and runny or sporadic and unusually hard. Mike Hoffman of West Palm Beach, Fla., says his intestines feel like they're constantly twisting and untwisting, "as though they're being wrung out like a dishrag." Jeffrey Roberts of Hallandale, Fla., says the sudden recurring knife-like pain "doubles you over," while others claim it brings you to your knees. Some liken the pain to a charley horse, but in the gutpain that is horrendous and then intensifies. "And don't picture a diarrhea attack happening in the privacy of your own clean bathroom," says Cynthia Huschle of Tolland, Conn. "Picture it in the bathroom at work, the store, the movie theater, the park, the baseball stadium, or the port-o-potty at the fair."

Extractions: IRRITABLE BOWEL SYNDROME A symptom of irritable bowel syndrome is a cramping, tight feeling in the abdomen, particularly after a meal. This is accompanied by irregular bowel movements with alternating hard movements (constipation) and diarrhoea. It is not known exactly why irritable bowel syndrome occurs but it is known that all kinds of factors have their influence on intestinal movement. The movement may be either large or small. This is why diarrhoea alternates with constipation. The complaint can also be caused by the wrong choice of food and lack of physical exercise. SELF-CARE Eat low fat and high-fibre foods. You may also add extra fibre to your food (ask for this at the pharmacy). Eating enough wholemeal bread, fruit, raw vegetables, lean meat or fish is often quite adequate. Ensure that you drink enough. This means 3 pints (1.5 litres) per day (minimum) or about 10 cups or 6 glasses. Take regular physical exercise. For instance, where there is the choice always walk, or go running or take at least three quarters of an hour's walk each day. Eat regularly and go to the toilet as soon as you feel the need. Further data indicates that up to 50 percent of patients who present to a gastroenterologist's office for further evaluation of symptoms has irritable bowel syndrome. In the past, diagnosis of irritable bowel syndrome was a diagnosis of exclusion. That is, other medical problems were eliminated first. Although this is still part of an evaluation, investigators have developed a set of criteria which assist physicians in the diagnosis of irritable bowel syndrome. These are called the Rome Criteria. Presentation: Patients with irritable bowel syndrome often have fluctuating symptoms over long periods of time. Patients may experience gas, bloating, dyspepsia, constipation, diarrhea, flatus, diarrhea alternating with constipation, and the passage of mucous in the stools. Extractions: Irritable bowel syndrome (IBS) is defined as alternating diarrhea and constipation coupled with abdominal pain. IBS has also been called irritable colon, mucus colitis and spastic colitis ( colitis is inflammation of the colon or large intestine), even though IBS does not cause inflammation of the colon. Irritable bowel syndrome affects some 40 million Americans, more than twice the number affected by diabetes (17 million) or coronary heart disease (12.6 million). Yet, because it is not a life-threatening condition, IBS takes a back seat to other conditions such as cancer when it comes to research funding. Thus, little research has been done on treating the condition or determining its cause. In addition to lack of funding for research, another hurdle is the condition itself: it is defined by a subjective description of symptoms (i.e., pain, cramping), which are not only much less reliable than objective symptoms (i.e., the presence of an ulcer), they are more difficult to measure. The lack of research means that little is understood about the actual mechanisms that cause irritable bowel syndrome. And because symptoms vary among individuals, no single irritable bowel syndrome treatment method will work for all IBS sufferers. All these factors make irritable bowel syndrome clinical trials and the participants who volunteer for them all the more important.

Thank you in advance for your kind assistance/guidance. A: You are not alone and IBS is not terminal, but it is extremely frustrating for the 15% of the US population that suffers from it. No one knows why IBS is much more common in women (75% of IBS patients) and is usually diagnosed at a young age (under 50). With menopause, women often find that illnesses or symptoms get worse for awhile. Hormonal changes can also cause bloating and changes in bowel habits.

I developed IBS last summer; it was clearly correlated to job stress. The miserable symptoms were all there - cramping, gas, diarrhea, constipation. As you noted, conventional medicine hasn't come up with a treatment. However, I did some research on my own and through the Internet found the Journal of the American Medical Association (November 11,1998 Vol. 280 pp.1585) study you mentioned that used a treatment involving Chinese herbs. At a natural foods store (Outpost Natural Foods in Milwaukee), I found a remedy called Calm Colon that is a combination of 20 Chinese herbs, made by a Los Angeles company called Samra. I checked their Web site (http://www.samra.com) talked to people at Outpost Natural Foods, and decided to try Calm Colon. The results were stunning. Less than 12 hours after I took the first dose, my symptoms were dramatically improved. I took Calm Colon regularly for about three months, and while I still had occasional cramping, the symptoms were largely gone.

The disorder has been said to account for 30 to 50 percent of all digestive conditions seen in clinics and office practices. Though often recognized in infants and children, it usually begins in late adolescence or early adult life, and rarely appears for the first time after the age of 50. It causes almost as much industrial absenteeism as the common cold and has been written about in English medical literature since the early 19th century. When the bowel is functioning normally, it produces orderly, gentle muscular contractions, alternating with relaxation, that propel its contents toward the rectum and anus for elimination. In irritable bowel syndrome, the muscular contractions are disorganized, harsh, or spasmodic, resulting in abdominal discomfort and erratic propulsion. Sometimes, this causes frequent, occasionally "explosive" elimination of liquid stools; and at other times, the bowel spasm delays normal evacuation, causing constipation. Therefore, although it seems contradictory, both diarrhea and constipation can be produced by the same basic mechanism in irritable bowel syndrome.

Irritable bowel syndrome (also known as IBS) classically involves recurrent abdominal pain in association with constipation and/or diarrhea, passage of mucus and frequently a sensation of bloating discomfort. Esophageal spasm - this is manifested by symptoms of chest pain and difficulty swallowing solids or liquids termed dysphagia. This symptom complex is frequently mistaken for cardiac pain. Nonulcerative dyspepsia - this is a broad and vague term encompassing a variety of symptoms such as belching, bloating, abdominal distension, upper abdominal pain most often after meals.
