Extractions: Infants with intermediate hemolytic disease are generally born at or near term (40 weeks) and appear to be in good condition. If, however, these infants are not carefully treated at birth, they can develop anemia and increased levels of bilirubin (hyperbilirubinemia) as a result of the increase red blood cell destruction. When red blood cells are destroyed, one of the byproducts is bilirubin. High levels of bilirubin can cause a condition called kernicterus which results in nerve damage in the brain. Prevention of kernicterus includes careful monitoring of neonatal bilirubin levels and putting the baby under special lights (phototherapy) to reduce the level of bilirubin in the body. In many cases, an exchange transfusion is needed. This involves removing much of the baby's blood and replacing it with Rh-negative blood. The new blood, which is carefully screened to be compatible with the baby and to be free from diseases (including HIV and Hepatitis), helps to eliminate the anemia by bringing in healthy red blood cells and also removes much of the excess bilirubin. Kernicterus is quite rare with advanced techniques in neonatal management. Experts in the field report knowing of few, if any, cases in recent history. Those babies who do develop kernicterus and survive may suffer from deafness, cerebral palsy and/or other conditions caused by the nerve damage.

Estos anticuerpos anti-Rh positivos se desplazan a través de la placenta hacia el feto y destruyen los glóbulos rojos circulantes. Por lo general, el primer hijo no se ve afectado (a menos que la madre haya tenido embarazos interrumpidos anteriormente, los cuales podrían haber sensibilizado su sistema) ya que toma tiempo para que la madre desarrolle anticuerpos contra la sangre fetal.

Unconjugated bilirubin is lipid soluble and travels in the circulation largely bound to albumin. However, if the levels of unconjugated bilirubin are high a small fraction of free bilirubin escapes from the vascular compartment and enters the cellular lipid fraction of brain cells, causing a brain damage that may or may not be permanent, i.e. kernicterus.

There is a coronal section of medulla on the left and cerebral hemisphere on the right demonstrating kernicterus in deep grey matter of hemisphere and brain stem. Increased unconjugated bilirubin, which accounts for the kernicterus, is toxic to the brain tissue. Kernicterus is more likely to occur with prematurity, low birth weight, and increased bilirubin levels.
