Pulmonary hypertension is rare in patients with systemic lupus erythematosus (SLE) although pulmonary involvement occurs in about 50%. Pulmonary hypertension without obvious pulmonary infiltration is even rarer, the mechanism of which is unclear. We report here a case of SLE who presented with severe pulmonary hypertension and cor pulmonale without any evidence of parenchymal lung disease. The patient was a 19-year-old Saudi female who was admitted to King Khalid University Hospital, Riyadh with a two-year history of dyspnea, easy fatigability, palpitations, anorexia, loss of weight, dry cough and episodic attacks of fever every three to four months with night sweats for the last two years. Five days prior to admission she developed pain in various large and small joints with swelling in interphalangeal and right wrist joints and described features of Raynaud's phenomenon. Chest roentgenogram showed cardiomegaly with prominent pulmonary conus. No lung infiltrate was noticed. High resolution computed tomography (CT) scan was not done. Ultrasound of the abdomen revealed an enlarged liver with mild ascites. Pulmonary function test revealed a restrictive pattern with decreased diffusion lung capacity. Ventilation/perfusion scan of the lungs did not reveal any evidence of pulmonary embolism. Cross-sectional echocardiography and Doppler studies revealed significant enlargement of the right atrium and right ventricle with dilated pulmonary artery and moderate degree of pericardial effusion. Moderate tricuspid incompetence was also detected.

Extractions: Lupus is a chronic, autoimmune disease which causes inflammation of various parts of the body, especially the skin, joints, blood and kidneys. The body's immune system normally makes proteins called antibodies to protect the body against viruses, bacteria and other foreign materials. These foreign materials are called ANTIGENS. In an autoimmune disorder such as Lupus, the immune system loses its ability to tell the difference between foreign substances (antigens) and its own cells and tissues. The immune system then makes antibodies directed against "self." These antibodies, called "auto-antibodies" react with the "self" antigens to form immune complexes. The immune complexes build up in the tissues and cause inflammation, injury to tissues, and pain. More people have Lupus than AIDS, cerebral palsy, multiple sclerosis, sickle-cell anemia, and cycsic fibrosis combined. Lupus Foundation of America market research data show that between 1.4 and 2 MILLION people reported to have been diagnosed with Lupus. For most people, Lupus in a mild disease affecting only a few organs, but for others, it can be serious, or even life threatening. For more about what Lupus is, click here: "Lupus" is Latin for the word "wolf," and it is common medical lore that the "butterfly rash" seen on the cheeks of many Lupus patients is so similar to the markings of a wolf. It is estimated that up to 2,000,000 Americans have been diagnosed with lupus. Lupus can be difficult to diagnose as the symptoms come and go and mimic other illnesses. The symptoms include transient joint and muscle pain, a rash caused by or worsened by sunlight, sores in the nose and mouth, low grade fevers, hair loss, pleurisy, fatigue, appetite loss and/or painful sensitivity of the fingers to cold. While medical science has not yet found a cure for lupus, new research brings unexpected findings and increased hope each year.
