Extractions: Columbus, Ohio Web page created on December 13, 1997 Henri Meige described in 1904 what is now commonly called oral facial dystonia. There is some variation in what has been described since in what has been called Meige's syndrome, but in all descriptions there is blinking and chin thrusting. Some patients have lip pursing or tongue movements and, for a few, the movements spread into the shoulders. The cause remains obscure, treatment less than ideal, and frustration is a major factor as it is with blepharospasm. Many physicians, even regarding the more common essential blepharospasm, may have had no experience with Meige's syndrome and the patient may be told the process is psychologic. It is not. There are some similar conditions related to medication effects, for example, the mouth movement seen when excessive levodopa is present in patients with Parkinson's disease. Tardive dyskinesia, the movements that can follow prolonged use of major tranquilizers, can be associated with mouth and tongue movements. Some normal, elderly individuals develop spontaneous mouthing or chewing movements. Meige is more than and different from any of these other mouth movements. Blinking forcefully is often a part of it as is grimacing and the chin thrusting forward. At times there is a joint interactive movement between the oral movements and the eye movements. The patients are more likely to be women than men and usually at middle age or beyond. There is, as in all neurologic conditions, some variation with stress, but the movement is present at rest and with activity and when with others or alone. As with almost all movement disorders, it disappears in sleep.

The term blepharospasm ['blef-a-ro-spaz-m] can be applied to any abnormal blinking or eyelid tic or twitch resulting from any cause, ranging from dry eyes to Tourette's syndrome to tardive dyskinesia. The blepharospasm referred to here is officially called benign essential blepharospasm (BEB) to distinguish it from the less serious secondary blinking disorders. "Benign" indicates the condition is not life threatening, and "essential" is a medical term meaning "of unknown cause". It is both a cranial and a focal dystonia. Cranial refers to the head and focal indicates confinement to one part. The word dystonia describes abnormal involuntary sustained muscle contractions and spasms. Patients with blepharospasm have normal eyes. The visual disturbance is due solely to the forced closure of the eyelids. Riopelle Abstract: Background: Meige syndrome is a movement disorder that includes blepharospasm and oromandibular dystonias. Its etiology may be idiopathic (primary) or it may arise secondary to focal brain injury. Acute respiratory distress as a feature of such dystonias occurs infrequently. A review of the literature on Meige syndrome and the relationship between dystonias and respiratory compromise is presented. Methods: A 60-year-old woman suffered a cerebral anoxic event secondary to manual strangulation. She developed progressive blepharospasm combined with oromandibular and cervical dystonias. Neuroimaging demonstrated bilateral damage localized to the globus pallidus. Years later, she presented to the emergency department in intermittent respiratory distress associated with facial and cervical muscle spasms. Results: Increasing frequency and severity of the disorder was noted over years. The acute onset of respiratory involvement required intubation and eventual tracheotomy. A partial therapeutic benefit of tetrabenazine was demonstrated.

Extractions: FORWARD BY A. J. LEES In the early sixteenth century, Brueghel painted faces with open mouths and contracted facial muscles, similar to those we now associate with cranial dystonia [1]. In 1899, Gowers [2] described conditions producing tonic and clonic jaw contractions. His differential diagnosis of tonic spasms included tetanus, trauma, hysteria, brainstem lesions and hypothermia. Convulsions, rigors, paralysis agitans, facial pain and chorea were recognized as causes of clonic spasms. In 1910, Meige [3] reported a syndrome of spasms of the eyelids in addition to contractions of the pharyngeal, jaw and tongue muscles. Characteristic of dystonia, these spasms were often provoked by voluntary action (e.g. talking, eating), or lessened by humming, singing, yawning or voluntarily opening the mouth. Some of the patients with Meige syndrome developed other signs of dystonia including torticollis or writer's cramp. In 1976, Marsden [4] concluded that blepharospasm and oromandibular dystonia were adult-onset segmental torsion dystonias. other reviews have supported this assertion [5-15]. We now use the term 'oromandibular dystonia' (OMD) when referring to dystonia of the masticatory, lower facial and tongue muscles with resulting spasms and jaw deviation.

I've had Botox injections for the blepharospasms but now I am noticing more and more strange things occurring: holding my breath, breathing through my mouth, accidentally biting the inside of my lip and mouth while eating or even talking, "screwing up" (spasms) of my nose and lower facial muscles, clenching my teeth and jaws, having to talk myself through writing out a check or even this article. Can't read a newspaper or fine print very much. It goes on and on. I had a face lift and upper bleph in July 1997. What a fantastic change in my appearance - I work for the plastic surgeon - love him like a brother. But, I'm now wondering if the surgery didn't trigger some underlying condition that I may have had and no one knew. Has anyone else experienced anything like this after facial cosmetic surgery - is it a possible side effect?

Dear Suzye, My husband was had the bleph operation 2/27. One week later he began having eye spasms, which developed into full-blown Meige Syndrome. He is now permanently disabled. Bo-tox injections, artane and valium..along with pain meds let him get through the day. Some days are better than others. We both feel that the operation was the catalyst for his illness.
