Extractions: TREATMENT OF BLEPHAROSPASM, MEIGE SYNDROME, AND HEMIFACIAL SPASM ABOUT DR. MAURIELLO'S WORK TREATING PATIENTS WITH FACIAL DYSIKINESIAS SINCE 1983 Home Botulinum toxin type A (Botox ) temporarily weakens the eyelid muscle spasms that result in involuntary eyelid closure. He has used the Botox injections to treat involuntary facial and neck spasms as well. These eyelid spasms, in particular, depending on their severity, render patients functionally disabled and troubled. Dr. Mauriello has treated over 600 patients with blepharospasm, Meige syndrome, and hemifacial spasm in over 10,000 treatment sessions since 1983. He has published results of limited myectomy and full myectomy surgery in patients who partially respond to botulinum toxin injections or have excess upper lid skin that weighs down the eyelids. Dr. Mauriello's long interest in blepharospasm began in the early 1980's. He was part of the original National Institute of Health's study entitled, Botulinum Toxin in the Treatment of Benign Essential Blepharospasm. This multicenter center was under the direction of Alan Scott, MD, Principal Investigator from Pacific Medical Center in San Francisco (1983-1989). Dr. Mauriello was honored to be selected a consultant to the Committee of Ophthalmic Procedures Assessment, American Academy of Ophthalmology 1985-89. This committee evaluated the effectiveness of botulinum toxin, type A, during its experimental research development. He currently serves as a Reviewer to the American Academy of Neurology Therapeutics Assessment Committee and its Subcommittee Assessment of BotulinumToxin Therapy.

Lash Page 4 The absence of confirmed cases of sympathetic ophthalmia after evisceration in a large group of patients suggests that the risk for sympathetic ophthalmia is extremely low. Abstract Purpose: To investigate a possible relationship between evisceration and sympathetic ophthalmia. Methods: Data from Mt. Sinai Medical Center and University Hospitals of Cleveland were collected and histopathologic specimens were reviewed for 51 of 90 patients who underwent evisceration between 1980 and 1996 and who returned for follow-up examinations. Additionally, a survey was sent to members of the American Society of Ophthalmic Plastic and Reconstructive Surgery, the Uveitis Society, and the Eastern Ophthalmic Pathology Society to determine the number of enucleations and eviscerations performed and the documented incidence of sympathetic ophthalmia after evisceration. Results: No clinical or histopathologic evidence of sympathetic ophthalmia after evisceration was found among patients treated at the two medical centers. The collective surveys showed a strong preference for enucleation over evisceration, but did not document evidence of sympathetic ophthalmia after evisceration.
