Extractions: This disease was described by Leri and Joanny in 1922. It is a rare, non-hereditary lesion that affects both sexes. It is usually apparent in early childhood and even in the first few days of life. About 50% of persons affected will develop the symptoms by 20 years of age. The classic radiographic appearance is that of sclerotic lesions of bones that look like wax dripping down the side of a candle. the name is derived from the greek melos "limb" and rhein "to flow". With an estimated incidence of 0.9 cases per million persons, it can be calculated that there are approximately 1,000,000 to 1,500,000 persons with meloreostosis currently alive. However, only about 300 cases have been reported in the literature. Patients with meloreostosis may have associated cutaneous and soft tissue lesions such as vascular malformations, neurofibromatosis, hemangioma, arterial aneurysms, linear scleroderma, tuberous sclerosis, hemangiomas, and focal subcutaneous fibrosis. Adults generally complain of pain, joint stiffness, and progressive deformity. In children the condition affects mainly the bones of the extremities and pelvis, and may result in limb length inequality, deformity, or joint contractures. Joint contractures may be accompanied by extraosseous bone formation.

Findings: There is prominent sclerosis involving the terminal tuft of the great toe, extending the entire length of the phalanx medially. Additionally, there is a linear area of sclerosis in the distal medial proximal phalanx. A small round area of sclerosis is seen in the head of the hallux metatarsal. Diagnosis: 1. Melorheostosis of the distal and proximal phalanges of the hallux (biopsy proven). 2. Enostosis (bone island) of the head of the hallux metatarsal. Discussion: Melorheostosis is an uncommon nonhereditary sclerosing bone disorder which often presents with pain and decreased range of motion, though it may often be detected incidentally. Our patient, seen at the proverbial outside hospital, underwent biopsy which was consistent with the radiographic diagnosis of melorheostosis. Association with the other sclerosing bone dysplasias (osteopoikilosis, osteopathia striata my be seen, and is known as mixed sclerosing bone dystrophy. This may in turn be associated with vascular and/or lymphatic anomalies, or Trevor's disease. Isolated melorheostosis may be associated with linear scleroderma, neurofibromatosis, or tuberous sclerosis, as well. The typical distribution is sclerotomal in one or more bones of an extremity. The axial skeleton can also be involved, however, either separately or in conjunction with extremity findings.

Methods and Materials: We investigated 23 cases of melorheostosis in our files. Evaluated were: radiographic appearance; involved skeletal region (i.e. upper/lower limbs); number of involved bones. Results: Five basic forms of radiographic patterns were found: 1. The osteoma like pattern (7 cases). 2. The classic pattern with "flowing candle wax" appearance (5 cases). 3. The extraosseous myositis ossificans like pattern (1 case). 4. The osteopathia striata like pattern (6 cases). 5. Mixed pattern (3 cases with type four and three pattern, 1 case with type one and three pattern). In 5 cases the upper limbs including the shoulder girdle were involved, in 16 cases the lower limbs including the pelvis, in 1 case the upper and lower limbs, and in 1 case the sacrum. In 10 cases only 1 bone was affected, in 4 cases 2 bones and in 9 cases 3 or more bony elements. Conclusion: The radiographic spectrum of melorheostosis is broader than reported in the literature up to now. We classified 5 basic forms of radiographic appearance, that open a corresponding spectrum of differential diagnosis (i.e. parosseal osteosarcomas). As the histology of melorheostosis is unspecific, a correct clinical-radiological diagnosis is important. Possible pathogenesis and associated clinical abnormalities of melorheostosis will be discussed.

Extractions: Melorheostosis HOME Melorheostosis is a rare, non-familial condition of hyperostosis of the cortical bone that usually presents unilaterally in long bones but has also presented in vertebra, ribs, skull and jaw. It usually involves the periostium and endostium of diaphyseal cortical bone and can be associated with various soft tissue changes. The incidence of this condition is quite rare, approximately only 400 known cases reported worldwide. However, the prevalence in the population that remains undetected is significantly higher and estimates on the order of 11.5 million worldwide have been suggested. The age of presentation ranges from the neonatal period to the elderly. The peak age of presentation is between the ages of 5 and 20 and approximately half of the cases are detected by age 20. There are no gender differences in the reported cases as well as no difference with respect to ethnicity suggesting increase risk in certain populations. The etiology of melorheostosis is thus far unknown. There have been certain theories proposed but to this point have not been proven. One theory is that an embryonic infection of a sensory nerve is responsible for the pathogenesis. This was proposed on the basis of cases where multiple lesions found in the same limb, tend to be associated with a single sclerotome. Another theory that has been proposed is that of a mosaicism. This would better explain the sporadic and asymmetric pattern found in some cases.
