Extractions: A twenty seven years old woman has come to the hospital with complaints of swelling, pain and restriction of movements in the right little and ring fingers and in the right elbow for the past one year. The radiographs of the patient's right hand and forearm were obtained (fig.1,2). Fig 1 : Frontal radiograph of the hand Fig 2 : Lateral radiograph of the elbow Diagnosis: MELORHEOSTOSIS (LERI'S DISEASE) The radiographs showed irregular dense cortical hyperostosis involving the phalanges and metacarpals of fourth and fifth fingers and carpal bones on ulnar aspect (fig.1), and also the medial aspect of ulna and lower end of humerus (fig.2). 1. A whole extremity is affected, and the dense cortical proliferations appear to flow as a regular continuous flow from shoulder to finger or hip to foot. 2. Only half of the extremity, usually the proximal half is affected.

Design and patients: We retrospectively reviewed the CT ( n =7) and/or MR imaging findings ( n =12) of 17 patients with characteristic radiographic findings of melorheostosis (undulating cortical hyperostosis with marked uptake on radionuclide bone scintigraphy). Results: CT and MR imaging revealed cortical hyperostosis as high attenuation and low signal intensity on all MR pulse sequences, respectively. Encroachment on the marrow space was seen in all cases resulting from endosteal involvement. Thirteen patients demonstrated 14 soft tissue masses with infiltrative margins in 80% of cases and seven showed extensive mineralization on CT or MR imaging (low intensity on all pulse sequences). Seven soft tissue masses were predominantly nonmineralized with intermediate signal intensity on T1-weighted and intermediate to high signal on T2-weighted MR images corresponding to vascularized fibrous tissue with variable collagen content pathologically. A 37-year old woman presented with intolerable pain associated with a slowly growing mass on the dorsum of the right hand. The radiographs were characteristic of melorheostosis involving the third and fourth metacarpals, with a cortical bony projection extending from the base of the third metacarpal eroding slightly into the fourth metacarpal, associated with bursal formation. Magnetic resonance imaging (MRI) demonstrated an inflamed bursal collection adjacent to the spur. This region corresponded to the patient's severe clinical, focal pain. To our knowledge, this is the first case report in which melorheostosis of the hand has been associated with bony spur formation and complicated by an inflamed bursa. Keywords.

Staging studies were highly suggestive of the parosteal osteosarcoma arising in the area of melorheostosis. The patient was treated by wide excision of the lesion without removing the entire melorheostosis. The defect was reconstructed by autogenous iliac bone graft. [X-Ray] Lateral radiograph at presentation. The smooth, symmetrical thickening of the posterior cortex was thought to represent melorheostosis.
