Researchers with the largest Phase III trial to date for mesothelioma, an aggressive cancer affecting the lining of the lung, reported results showing that patients on a new chemotherapy drug regimen live longer and have less pain than those on an older drug. The findings were announced at the annual meeting of the American Society of Clinical Oncology meeting in Orlando, Fla., on May 20, 2002. According to lead author, Nicholas J. Vogelzang, M.D., University of Chicago Cancer Research Center, "This is the largest clinical trial ever conducted in this disease and the 25 to 30 percent improvement in survival for patients on the combination therapy is the first time anyone has documented a significant improvement in patients treated for mesothelioma." Malignant pleural mesothelioma is associated with a history of asbestos exposure in about 70 to 80 percent of all cases and there is no approved or very effective chemotherapy for the disease. Researchers hypothesized that pemetrexed might prove effective in treating this disease because it targets key enzymes thought to play a role in allowing the rapid growth of this tumor.

Mesothelioma is most often found in the surface lining of the chest (the pleura), the abdomen (the peritoneum), and less commonly in the heart and testicles. About 2,000 to 3,000 new cases are expected to be diagnosed in the United States in 2002, but the numbers appear to be increasing. Men are three to five times more likely to get the disease. Because the disease is often advanced before symptoms appear, the disease has historically been regarded as incurable and fatal, but more and more people are pushing for a cure. Most people with malignant mesothelioma have worked around asbestos. Many people have treated mesothelioma as a passing phenomenon that will disappear along with the use of asbestos. However, while asbestos use was curtailed in the United States in the 1970s, the latency period for contracting mesothelioma is between 15 and 50 years. Also, asbestos is continuing to be used in other countries: Asia, Europe, Great Britain, Australia, and South America. The disease is also a risk among other occupations, such as mining and manufacturing. Types of Mesothelioma There are three main types of mesotheliomas. The most common, found in 50 to 70 percent of the cancers, is the epithelial type. This type has the best outlook. The other two less common types are called mixed/biphasic and sarcomatoid . About three-fourths of mesotheliomas begin in the chest cavity and another 10 percent to 20 percent begin in the abdomen.

Extractions: In 1997, the last year for which data are complete, there were 1329 new cases of lung cancer diagnosed in men, and 1145 lung cancer deaths recorded. Among women there were 1014 new cases diagnosed, and 817 deaths recorded. About 85% of the new cases were non-small cell cancers; the remainder was small cell carcinomas. Approximately 60% of the diagnosis were among males and 40% among females, but this ratio is rapidly changing as the incidence among women is growing. Age Three quarters of diagnosed lung cancer patients were 62 years old and older, and half were 69 years and over at diagnosis. The mean age at diagnosis was 68 years. The youngest person diagnosed in BC during the last ten years was 17 years old, the oldest 104. Age distribution is presented on the histogram below: Survival A large proportion of incident lung cancer cases are fatal, therefore, lung cancer is the most common cause of cancer mortality for both men and women. The 5-year survival remains poor at about 11% for men and 14% for women, with little evidence of improvement over time (National Cancer Institute of Canada, 1995). The plot indicates that the vast majority of patients diagnosed with Lung Cancer died from this cancer; few died from other cancers or non-cancer causes. Median Lung Cancer specific survival about 8.5 months. Median overall survival for these patients is 7 months.
