Extractions: Departments Patient Information Multiple chemical sensitivity (MCS) is a syndrome in which multiple symptoms reportedly occur with low-level chemical exposure. Several theories have been advanced to explain the cause of MCS, including allergy, toxic effects and neurobiologic sensitization. There is insufficient scientific evidence to confirm a relationship between any of these possible causes and symptoms. Patients with MCS have high rates of depression, anxiety and somatoform disorders, but it is unclear if a causal relationship or merely an association exists between MCS and psychiatric problems. Physicians should compassionately evaluate and care for patients who have this distressing condition, while avoiding the use of unproven, expensive or potentially harmful tests and treatments. The first goal of management is to establish an effective physician-patient relationship. The patient's efforts to return to work and to a normal social life should be encouraged and supported. M ultiple chemical sensitivity (MCS) has been described under various names since the 1940s.

Some people believe that exposure to a chemical or chemicals triggers a symptom complex that has been called MCS. Those symptoms occur in many organ systems. No physical signs can be found consistently in MCS patients. There are a number of synonyms for MCS, including: 20th Century Disease, Environmental Illness, Total Allergy Syndrome, Idiopathic Environmental Illness and Chemical AIDS. MCS appears to affect young women at a proportionally greater rate than men or older women. Among the proposed theories are dysfunction of the immune system, and neurological abnormalities specifically chemical sensitization of the limbic system and various psychological theories. Extractions: Multiple Chemical Sensitivity Syndrome is marked by multiple symptoms in multiple organ systems (usually the neurological, immune, respiratory, skin, "GI," and/or musculoskeletal) that recur chronic-ally in response to multiple chemical exposures. MCS usually starts with either an acute or chronic toxic exposure, after which this initial sensitivity broadens to include many other chemicals and common irritants (pesticides, perfumes and other scented products, fuels, food additives, carpets, building materials, etc.). MCS Symptoms commonly include difficulty breathing, sleeping and/or concentrating, memory loss, migraines, nausea, abdominal pain, chronic fatigue, aching joints and muscles, and irritated eyes, nose, ears, throat and/or skin. In addition, some with MCS show impaired balance and increased sensitivity not just to odors but also to loud noises, bright lights, touch, extremes of heat and cold, and electromagnetic fields. MCS Studies show about 2/3 of those with Chronic Fatigue Syndrome and Fibromyalgia also have MCS (and vice versa), as do 1/2 of those with allergies. Like CFS, MCS is more common in women and can start at any age, but usually begins in late puberty to mid-life. There is no unique test for MCS, but immune, porphyrin, EEG, balance, and SPECT scan abnormalities are common.

Extractions: Consensus Criteria for MCS The following consensus criteria for the diagnosis of MCS were gleaned from the study by Nethercott et al.(14) (funded in part by grants from US NIOSH and US NIEHS): [Added in 1999]: Symptoms involve multiple organ systems. To assist physicians who are unfamiliar with the evaluation of MCS, we recommend that clinical protocols include validated questionnaires for screening and characterizing chemical sensitivity,(15,16) a list of overlapping disorders to consider in the differential diagnosis of MCS, and a list of signs and test abnormalities associated with MCS in the peer-reviewed literature (summarized by Ashford and Miller(17) and Donnay(18)). Although no single test is yet considered diagnostic of MCS, those suggested by signs, symptoms, or history may be helpful in treating and tracking the disorder. The presentation of MCS may vary greatly among cases and over time. Some individuals are totally disabled by severe symptoms suffered on a daily basis, for example, whereas others are disabled only minimally by mild symptoms suffered occasionally. We, therefore, recommend that any clinical diagnosis of MCS be characterized and followed over time using quantitative and/or qualitative indices of life impact or disability (e.g., minimal, partial, total);

Extractions: Joe E. Heimlich A trend noted by many people has been an increase in what seems to be allergic reactions in the general population. Ranging from congestion to sneezing to more severe reactions such as rashes, breathing problems and worse, these reactions are often non-specific to natural allergens. Some activists and researchers have identified chemicals as potential factors in these reactions. Multiple Chemical Sensitivity (MCS) is the name given to the broad issue of reactions to specific or cumulative chemicals in the environment. This fact sheet will present a definition of MCS and the related Environmental Illness; explore factors of and criticisms to MCS; and examine the individual's role in reducing potential sensitivity. In theory, MCS is an adverse physical reaction to low levels of many common chemicals. Chemical sensitivity is generally accepted as a reaction by certain individuals to chemicals but debate rages on whether MCS is classifiable as an illness. One of the difficulties in classifying MCS as an illness has been the complex nature of chemicals in the environment and the interaction effects with and within the human body. The length of exposure, the concentration of the chemical(s), and the individual's threshold of resistance are also factors complicating a simple definition. In the relatively few but growing documented cases of severe reactions to chemicals, there seems to be no single stimuli or predictor of reactions. The most severe cases, often called either Environmental Illness or 20th Century Disease, sometimes result in individuals isolating themselves from society, synthetic products, and any type of chemical product.
