    Extractions: Overview The University of Maryland Greenebaum Cancer Center is setting the standard for a new approach to stem cell transplantation to fight multiple myeloma and similar cancers. Instead of giving low doses of chemotherapy, the Greenebaum Cancer Center advocates giving high-dose chemotherapy with stem cell transplantation early in the course of some cancers. This new approach aims to fight cancer cell resistance to chemotherapy. High-dose chemotherapy lowers the chances of myeloma cells escaping and becoming resistant to treatment. Stem cell transplantation and high-dose therapy should be done early in the disease when myeloma cells are still sensitive to chemotherapy and when the bone marrow has not been damaged by conventional chemotherapy. To maximize the chances of controlling multiple myeloma as long as possible, the University of Maryland Greenebaum Cancer Center gives patients further treatments after their transplantation every three months for one year. There is preliminary evidence that these further treatments increase survival.

Multiple myeloma is a cancer of a certain type of white blood cell called the plasma cell. The plasma cell is born and normally resides in the bone marrow. The normal function of the plasma cell is to produce antibodies that fight infections and possibly even other cancers. Although all normal plasma cells appear similar, each plasma cell is uniquely programmed to produce a single type of antibody called a monoclonal antibody. Myeloma arises in the bone marrow of patients when a single cell or clone of plasma cells begins to grow uncontrollably and produce an overabundance of one type of antibody or monoclonal antibody. In this situation, instead of many different plasma cells producing a normal array of different antibodies, the patient's immune system overproduces a single type of antibody and not enough of the other type of antibodies. The major clinical manifestations of myeloma involve frequent infections, bone pain, fatigue due to anemia and kidney damage.

Plasma cells are normally present in the bone marrow (making up less than 5% of the bone marrow) and are responsible for the production of antibodies when the body is dealing with infection. Cancerous plasma cells (myeloma cells) build up in the bone marrow, interfering with normal immune response and bone marrow production of normal blood cells. These myeloma cells invade and damage the bone, causing tumors (myelomas) to form. Myeloma cells then travel to other sites in the body, causing further tumors (hence the name "multiple myeloma"). Common features or symptoms of multiple myeloma are bone pain (especially with movement), anemia, fatigue, hypercalcemia, and increased incidence of infections (such as bladder or kidney infections, skin infections, pneumonia, or sinusitis). Testing for and diagnosis of multiple myeloma normally begins with blood and/or urine tests. These are followed by a direct biopsy to find myeloma cells, or a bone marrow sample showing elevated levels (at least 10%, but usually more than 30%) of plasma cells. Adjunctive diagnostic testing can include whole-body x-ray, to assess for lytic lesions or severe osteoperosis.

    Extractions: Plasma cells are "B" lymphocytes in their final stage of development, when they are actually producing antibodies to fight "germs" like bacteria, viruses, fungi or protozoan parasites. They are an integral part of the immune system . There are basi-cally two types of immunity, called "cellular" and "humoral" immunity. Lymphocytes are white blood cell (about 30% of adult's while blood cells) that are actively involved in both types of immunity. "Cellular" immunity means that the white blood cells directly kill foreign invading germs, such as by enveloping and digesting them, while the "humoral" type means that the invaders are coated by antibodies and then gradually enveloped and digested by other white blood cells.
