Infants are usually floppy and may be observe to have limited ocular excursions and ptosis. Diagnosis is afforded by skeletal muscle biopsy with electron microscopy, histochemical staining, biochemical testing for genetic analysis. C HRONIC P ROGRESSIVE E XTERNAL O PHTHALMOPLEGIA describes a variety of conditions presenting at any age characterized by gradual limitation of eye movement function. While CPEO may be limited to just the extraocular muscles and lids these conditions are usually accompanied by systemic involvement of other muscles and tissues, especially those with high energy requirements such as brain and kidneys when the basic defect is a mitochondrial DNA abnormality (mtDNA disorders - see prior Grand Rounds presentation for more on these conditions). DYSTROPHIES are primary muscle diseases genetically determined, both recessive and dominant. Common problems are aches and pain in the neck and shoulder region and disorders in the hands or the wrists. The European project PROCID aimed at providing a scientific base for prevention measures against such problems. A key element in medical and physiological studies of such issues is the analysis of electromyograms (EMG), i.e., the recordings of the electrical signals that are associated with all muscle activity. Ideally, the medical researcher would like to see the activation pattern of each individual motor unit (muscle fibre group). However, the measured electrical signal is the superposition of the signals from a large number of such of motor units, the 'signature' of which is not known a priori and may even change over time. Extractions: As you will be aware, teams of scientists all over the world are working to convert the large amount of knowledge we have gained about the Duchenne muscular dystrophy gene into practical treatments that will help cure or lessen the effects of the disease. Many different approaches are being taken, and several groups reported important progress in 1999. More details of these publications can be found by clicking on the American Muscular Dystrophy Association web site. The discovery of the "Utrophin" gene, closely related to the DMD gene, by Professor Kay Davies in Oxford has given rise to this avenue of research. Good progress has been made on ways to increase the production of Utrophin in muscle, but the study of patients with naturally high Utrophin levels indicates that they may have problems with blood clots, and another study showed that natural over-expression of Utrophin was not sufficient to prevent DMD-associated heart disease. Nevertheless, mice with a form of DMD did show improvement when Utrophin gene therapy was used.

Extractions: March 09, 1999 NEW YORK (Reuters Health)Creatine, an over-the-counter amino acid supplement popular among athletes, may help to build muscle and increase muscle strength in patients with muscular dystrophy and other neuromuscular disorders, a study suggests. People with these muscle disorders had a small increase in handgrip, ankle, and knee strength after taking creatine monohydrate daily for 10 to 12 days, according to the report in the March 10th issue of the journal Neurology. The study included 21 people with various types of neuromuscular disorders including muscular dystrophy, which cause muscle atrophy (wasting) and loss of strength. "The treatment appears to be well tolerated in the short term," report Dr. Mark Tarnopolsky and Joan Martin, of McMaster University Medical Center in Ontario, Canada. But the researchers caution that more studies need to be conducted to determine if creatine continues to have a benefit over a longer period of time. The subjects consumed either a placebo ("dummy" pill) or 10 grams of creatine monohydrate daily for five days followed by five grams of creatine monohydrate daily for five to seven days.
