    Extractions: EPM is a debilitating neurologic disease of horses. It can affect the brain, brainstem, spinal cord or any combination of these three areas of the central nervous system. Clinical signs may suggest focal or multifocal disease, which means the disease may affect a very small (focal) part of the central nervous system (CNS) or may parts of the CNS (multifocal). Therefore the disease may present itself with a variety of different clinical signs, dependant on the location of the damage caused by the organism within the CNS. There is no vaccine currently available. EPM occurs in much of North America. Surveys conducted in central Kentucky, one county in Pennsylvania and the entire states of Ohio and Oregon have revealed that approximately 50% of the horses have been exposed to this parasite. We know that a positive serum test indicates exposure to the parasite, not necessarily the presence of disease, the incidence of which is much lower. In the studies that looked at the distribution of seropositive cases geographically, it was found that climatic factors may affect exposure rates, i.e., an increase of freezing days was associated with a decrease in numbers of horses exposed to the parasite. A similar effect of very hot environments may also decrease the survival of the infective stages of the protozoan parasites and lessen exposure to horses. EPM appears to have a sporadic distribution, although outbreaks have been reported on farms in Kentucky, Ohio, Indiana, Michigan, and Florida.

Both indicate spinal cord involvement by some pathological event: myelopathy is a more inclusive term that does not imply an aetiological factor while myelitis should be restricted to inflammatory diseases of the spinal cord. Myelopathy includes myelitis; myelitis produces a myelopathy. Any process that impairs the function of the spinal cord produces a myelopathy; in the common use, however, the term is usually employed with an adjective such as "compressive", "degenerative" or "congenital". Despite the fact that conditions such as spinal cord tumours or hydrosyringomyelia produce symptoms of myelopathy they are usually considered as specific, separate entities. On the other hand, "myelitis" is frequently a sort of wastebasket in which all myelopathies of unknown or undetectable aetiology are included. With these caveats in mind an attempt at classification can be made.

It may occur alone or in combination with demyelination in other parts of the nervous system. Onset of the disorder is sudden. Symptoms may include low back pain, spinal cord dysfunction, muscle spasms, a general feeling of discomfort, headache, loss of appetite, and numbness or tingling in the legs. Transverse myelitis may be caused by viral infections, spinal cord injuries, immune reactions, or insufficient blood flow through the blood vessels in the spinal cord. It may also occur as a complication of such disorders as optic neuromyelitis, multiple sclerosis, smallpox, measles, or chickenpox.

    Extractions: I have had right side shoulder and hip pain for over 5 years. In the last year I have developed right sided numbness (mostly in my arm), spasticity in my right lower leg, various funny sensations such as a feeling of wetness on an arm or let that is not wet and feeling things crawling on me that aren't really there. I have also had L'hermittes for the last year. My neurologist ran brain and cervical MRI's, SSEP testing and a Lumbar Puncture. The cervical MRI showed a vertical lesion and the SSEP testing showed that my right side responses were considerable lower than my left. The LP came back normal as did the brain MRI. My neurologist has been great to me and incredibly patient. He says that with or without a definite diagnosis, he can still treat my symptoms.

The term myelitis refers to inflammation of the spinal cord; transverse simply describes the position of the inflammation, that is, across the width of the spinal cord. Attacks of inflammation can damage or destroy myelin, the fatty insulating substance that covers nerve cell fibers. This damage causes nervous system scars that interrupt communications between the nerves in the spinal cord and the rest of the body. Symptoms of transverse myelitis include a loss of spinal cord function over several hours to several weeks. What usually begins as a sudden onset of lower back pain, muscle weakness, or abnormal sensations in the toes and feet can rapidly progress to more severe symptoms, including paralysis, urinary retention, and loss of bowel control. Although some patients recover from transverse myelitis with minor or no residual problems, others suffer permanent impairments that affect their ability to perform ordinary tasks of daily living. Most patients will have only one episode of transverse myelitis; a small percentage may have a recurrence.

Causes and symptoms Transverse myelitis has many different causes, often triggered by a variety of viral and bacterial infections (especially those associated with a rash such as measles or chickenpox ). Sometimes, there appears to be a direct invasion of, and injury to, the spinal cord by an infectious agent (such as herpes zoster or the AIDS virus).
