Myelitis betyder betændelse i rygmarven, transversel refererer til positionen for betændelsen som tværs henover rygmarven, svarende til et bestemt segment. Betændelsen kan ødelægge myelin, som er en fedtsubstans der omgiver nerveceller/nervefibre. Betændelsen kan ødelægge forbindelsen mellem nerver i rygmarven og resten af kroppen. Den præcise årsag til sygdommen er ikke kendt i detaljer, men den ses ofte efter virusinfektioner. Sygdommen er ikke arvelig. Sygdommen kan også ses som komplikation til andre sygdomme, herunder infektionssygdomme, autoimmune sygdomme samt ved blodkarsygdomme i rygmarven. Sygdommen kan også ses som led i dissemineret sclerose. Symptomerne er pludselig indsættende rygsmerter, muskelsvaghed og snurrende fornemmelser i benene. Kan udvikle sig til lammelser samt manglende kontrol over blære- og tarmfunktion. Efter 2 til 12 uger sker der en vis genopretning af normale funktioner, men mange får blivende symptomer. Diagnosen stilles klinisk, dvs ved den lægelige udspørgen og undersøgelse, evt. hjulpet af skanninger og blodprøver. Der findes ikke nogen effektiv behandling, som kan kurere sygdommen, men understøttende behandling undertiden i form af respirator er nødvendig.

Extractions: Transverse myelitis, or TM, is a neurological syndrome which is caused by inflammation of the spinal cord. Myelitis simply means inflammation of the spinal cord; transverse refers to involvement across one level of the spinal cord. Initial symptoms may include back pain, rash, headache, weakness, numbness and/or tingling of the legs and generalized discomfort. Eventual paralysis, sensory loss, and bowel and bladder dysfunction may occur, or patients can completely recover. TM can be acute or slow developing. There are several variations of the TM diagnosis, as well. TM may occur by itself, or in connection with another illness. When it occurs without apparent underlying cause, it is assumed to be a result of abnormal activation of the immune system against the spinal cord. TM often develops in the setting of viral and bacterial infections, although this is more common in association with viral infections, such as polio, measles and herpes. Poliomyelitus virus is acquired by fecal-oral transmission; anyone infected by this virus will shed new virus particles in the stools, and so this could be found in raw sewage. Measles is spread by aerosol transmission and therefore highly unlikely to be present in sewage; herpes viruses are transmitted either by direct contact or by droplets. The last two virus have human beings as their main resevoir of infection; virus particles are not likely to be found in the stools and therefore not likely to be found in raw sewage.

After retrospective study of 14 cases, we found that MRI is valuable for both diagnosis and prognosis in this illness. The criteria for the diagnosis of acute transverse myelitis consisted of acute onset (over less than 3 weeks) of symmetrical motor and sensory dysfunction referable to a distinct spinal cord level, with sphincter dysfunction. If you found out that the things that, until now, gave comfort and meaning to your life, lacked integrity? If what you felt to be safe now proved to be dangerous? If the people you counted on for help had no answers? All preliminary tests were negative except for the nerve conduction studies, which showed only a slightly delayed response in my right tibial nerve. These studies were the only clinical evidence of my deteriorating condition. After a battery of additional tests, by process of elimination, I was at long last, after more than two months, given the diagnosis of Transverse Myelitis with unsatisfactory, even harmful, treatment options and an uncertain prognosis. As my condition worsened, I learned that the doctors, my faithful doctors for 27 years, could not help me. This was particularly poignant because, I, myself, had naïve ambitions of one day being the type of doctor who could and would help anyone who knocked on my door. The laborious process of diagnosis and the endless arrays of diagnostic tests further discouraged me and deepened my despair.
