Extractions: *For Medical Professionals only, registration required Non-neurogenic neurogenic bladder, also called Hinman's syndrome, characterized by clinical and urodynamic evidence of involuntary bladder outlet obstruction at the level of the external sphincter, in the absence of a definable structural or neurological cause. The bladder becomes large and trabeculated, in response to repeated contraction against a closed sphincter. Children present with incontinence, infrequent difficult voiding, faecal soiling and constipation. Urinary tract infections may also occur. Some believe the condition is psychogenic in origin, because excessive punishment for bedwetting results in a vicious cycle of sphincter tightness, poor bladder emptying and further enuresis. Defaecation is similarly affected, with resulting constipation, faecal soiling and encopresis. Radiological findings include hydroureteronephrosis , renal scarring, vesicoureteric reflux , large volume trabeculated bladder, conical dilatation of the posterior urethra, and persistent or intermittent narrowing in the region of the external sphincter ( Fig.1

Extractions: Normal bladder function is dependent on the nerves that sense the fullness of the bladder (sensory nerves) and on those that trigger the muscle movements that either empty it or retain urine (motor nerves). The reflex to urinate is triggered when the bladder fills to 300-500 ml. The bladder is then emptied when the contraction of the bladder wall muscles forces urine out through the urethra. The bladder, internal sphincters, and external sphincters may all be affected by nerve disorders that create abnormalities in bladder function. There are two categories of neurogenic bladder dysfunction: overactive (spastic or hyper-reflexive) and underactive (flaccid or hypotonic). An overactive neurogenic bladder is characterized by uncontrolled, frequent expulsion of urine from the bladder. There is reduced bladder capacity and incomplete emptying of urine. An underactive neurogenic bladder has a capacity that is extremely large (up to 2000 ml). Due to a loss of the sensation of bladder filling, the bladder does not contract forcefully, and small amounts of urine dribble from the urethra as the bladder pressure reaches a breakthrough point.

ANAMNESIS: On August 20, 1991, Valentina C. and her father, presented themselves to my chiropractic office for a consultation for scoliosis. Valentina had recently had an orthopedic consultation for the spinal complaint and pain with weakness in dorsiflexion of the right foot when walking. When the orthopedist requested an EMG of the sciatic nerve and prescribed a L'NARD boot, Dr. M. raised an objection, intervened and requested they try a chiropractic approach to the problem (for complications which will become obvious later). REMOTE ANAMNESIS: Valentina, a twelve year-old student, was born with Spina Bifida Manifesta (L5-S3) and a myelomeningocele. At the age of four years, she underwent neurosurgery (by the same Dr. M.) to re-position the escaped contents of the spinal canal. The surgery was successful, but she developed a "clubfoot" and a spastic urethral sphincter, necessitating two surgical catheterizations following enteric bacterial infections of the urinary bladder with vesiculoureteral reflux.

Like I was awakened every hour or two. Talk about feeling miserable. Well, two days ago he prescribed a very mild muscle relaxant and an alpha blocker by the name of Halenol. I got up to urinate two times the night before last, and only one time last night. My doctor, wife and I regretfully believe that I will eventually need spinal surgery. Yeah, I hurt it leg pressing - believe it or not - 840 pounds. What an idiotic thing to do! But, that was way back in 1954 when I was 21 years old, and the superman syndrome had me firmly by the "you-know- whats". I've had trouble on and off since then finally culminating in the misery I'm experiencing now. Anyway, two days of relief is God sent. Maybe, it'll continue. I thought I'd tell you about it. Extractions: OVERACTIVE BLADDER According to a two-year study undertaken at the University of North Carolina School of Nursing, Chapel Hill, older rural women can improve their urinary incontinence problem through behavioral management intervention. Molly Dougherty, PhD, RN and a professor at the university led the study group. The National Institute of Nursing Research, part of the NIH, funded the research. There were a total of 178 subjects involved in the study group, all of whom resided in Florida and were 55 years of age or older. The behavioral management intervention involved the women setting goals for their continence. Three behavioral phases in sequence were also established. The first sequence involved emptying the bladder at regular timed intervals. These timed intervals were gradually increased in timing, until it reached 2 1/2 hours or more. The women kept a bladder diary that included such information as descriptions of their liquid intake and intervals between urination. Another intervention that was used sought to strengthen the muscles to prevent leaking urine. This involved pelvic muscle (kegel) exercises.
