Extractions: Signs, symptoms and indicators Ochronosis is the muscoskeletal manifestation of alkaptonuria - a rather rare (one in 200,000 births) inherited disorder of protein metabolism characterized by an inability of the body to metabolize the amino acids tyrosine and phenylalanine . It affects especially the large joints (hip, knees and vertebral column) by a progressive degenerative arthrosis The outward signs are the ocular (eye) and the skin pigmentations, the genito-urinary calculi (stones) and cardiovascular ochronosis (especially the aortic valve). The symptoms mostly begin within the third or fourth decade. It was Scribonius who described the first known case of ochronosis in 1584. He mentioned a schoolboy who passed urine black as ink. In 1902, Albrecht and Zdareck discovered the link with alkaptonuria. The incidence of alkaptonuria is 1 per million with the highest prevalence in Slovakia by inbreeding. [ Laoussadi S., Menkes C.-J. It is a disorder of tyrosine (an amino acid) metabolism resulting from a defect in the enzyme homogentisic acid oxidase. Homogentisic acid is excreted in the urine and turns a brown color upon exposure to air. This is the result of a dark pigment with an ochre color (earthy red or yellow) which led to the name ochronosis. The bones and cartilage of the body can be brown colored. Gallicano IRCCS Roma Summary Persons with dark skin often use depigmenting agents to lighten the color of their skin for cosmetic purposes. Very often, however, topical depigmenting agents can produce the opposite effect, leading to brownish coloring of the site of application. Two cases of hyperpigmentation of the face are described, which, on histologic examination revealed the cause as exogenous ochronosis. At history taking it was discovered that for indefinitely long periods of time both women had regularly applied hydroquinone-based skin-bleaching creams. The mechanism producing hyperpigmentation is not yet fully understood. The clinical features of cosmetics-induced lesions in two clinical cases of exogenous ochronosis are described. Key words: Exogenous ochronosis The chronic use of skin-bleaching creams, particularly hydroquinone- and corticosteroid-based products, can produce complex dermatologic pictures, especially if the products are applied regularly and for long periods of time. Various products from questionable sources are available on the market, often from African countries (Nigerian, the Ivory Coast, South Africa) or Europe, and are produced and sold outside the norms of health safety regulations.

Extractions: Case #2 Patient: F.G., 53-year-old woman Duration: Four years Distribution: FaceFace History: The patient presented to the Bellevue Hospital Center Dermatology Clinic in June, 2001, with a history of dark patches that developed on the cheeks and progressed over a four-year period. She admitted to using many creams on her face prior to their development but was unsure of their names. She denied arthralgias and had no family history of a similar disorder. Past medical history included hypertension controlled by amlodipine and atenolol. A biopsy was performed which confirmed the clinical diagnosis. Subsequently, the patient was advised to discontinue all previous topical preparations and use a cover make up. She also had rosacea for which she was given a topical metronidazole preparation. Physical Examination: Speckled, blue-black patches along with erythematous patches and telangiectases were present in a malar distribution bilaterally.
