Extractions: Ocular Motility Two principle methods of evaluating ocular motility are: Observation of ocular ductions , which are the actual monocular movements of the eye, and Observation of binocular ocular alignment, using cover/uncover and alternate cover testing. There are six extraocular muscles controlling eye movement: the four rectus muscles (medial, lateral, superior, inferior), and two oblique muscles (superior and inferior). While the medial and lateral rectus muscles are fairly straightforward in their respective adduction (inturning) and abduction (outturning) of the eye, the remaining four muscles are more complex. The superior and inferior rectus, and the superior and inferior oblique muscles each have primary and secondary actions, depending of the position of the eye. This is illustrated by the drawing below: The right eye in the orbit is shown, from a superior view. Note that while the eye is directed straight ahead that the superior rectus attaches to the eye at an angle. Similarly, the superior oblique has an angled insertion (from the trochlea near the superior orbital rim). Because of this angling, the superior rectus will act primarily as an elevator of the eye especially when the eye is abducted. The superior oblique more strongly will depress the eye when it is adducted, and will rotate the eye torsionally when abducted (twists the eye inward.)

Each eye has six muscles, four rectus and two oblique, which, when functioning properly, allow the eyes to work together in a wide range of gaze. Muscles of the Right Eye Patients are usually tested for eye position and movement during routine eye examination. To observe the action of all twelve muscles, the patient is asked to look in the 9 diagnostic positions of gaze. The patient is instructed to follow a target to various points of gaze while the physician closely monitors their eye movements. Any noted limitation or misalignment of the eyes could indicate muscle weakness or paralysis and warrant further investigation. upward gaze Ocular motility photographs may be requested in order to document any improper muscle action. Patients with strabismus and those diagnosed with cranial nerve palsies are good candidates for photographic documentation. These photographs can be used in reinforcing the physicians clinical impression, for medical-legal reasons or as a starting point prior to muscle surgery. Strabismus is a generic term used to describe any misalignment of the eyes often due to an imbalance of the extraocular muscles. This misalignment may develop at any age and occur in any direction. However, strabismus can be especially harmful to the development of vision in young children. If not diagnosed and treated early, visual acuity could be irreversibly damaged.

Extractions: Many individuals who work at a computer video display terminal (VDT) report a high level of job-related complaints and symptoms, including ocular discomfort, muscular strain, and stress. Surveys of computer workers show that eye and vision problems are the most frequently reported health-related problems, generally occurring in 70 to 75 percent of computer workers. The complex of eye and vision problems related to near work experienced during computer use has been termed "computer vision syndrome" The elements of working at a computer which make it visually demanding include frequent saccadic eye movements (ocular motility) and continuous eye focusing (accommodation) and alignment (vergence) demands. Problems occur when the visual demands of the task exceed the visual abilities of the individual to comfortably perform the task. Eye movements, eye focusing, and eye alignment processes involve repetitious muscular activity. The movement of each of the eyes is controlled by six extraocular muscles that are responsible for proper alignment of the eyes when viewing objects such as the computer screen. The ability of the eyes to change focus is controlled by the ciliary muscle, which exerts a force on the internal lens of the eye. In many respects, many computer related vision problems can be considered a form of musculoskeletal disorder, commonly referred to as cumulative trauma disorder (CTD) or repetitive stress injuries (RSI).
