If it compresses the pituitary stalk or involves the area of the pituitary gland itself, the tumor can cause partial or complete pituitary hormone deficiency. This frequently results in one or more of the following: growth failure, delayed puberty, loss of normal menstrual function or sexual desire, increased sensitivity to cold, fatigue, constipation, dry skin, nausea, low blood pressure, and depression. Pituitary stalk compression can also cause diabetes insipidus and may increase prolactin levels causing a milky discharge from the breast (galactohhrea). If the craniopharyngioma involves the optic tracts, chiasm, or nerves, then visual disturbances can result. Involvement of the hypothalamus, an area at the base of the brain, may result in obesity, increased drowsiness, temperature regulation abnormalities, and diabetes insipidus (DI). Other common symptoms include personality changes, headache, confusion, and vomiting. Diagnosis The best means of visualizing a craniopharyngioma is with an MRI or a CT scan of the pituitary region performed without and with contrast.

Extractions: PITUITARY TUMOR PROGRAM PITUITARY DIAGNOSES INDEX Cushing's syndrome is a debilitating endocrinopathy characterized by excessive cortisol levels in the blood which may be produced from tumors of the pituitary gland, adrenal glands (located above the kidneys) or from tumors or cancer arising elsewhere in the body (ectopic ACTH producing tumors). Cushing's disease refers specifically to excessive ACTH secretion by a pituitary adenoma. The cause of Cushing's Syndrome is a pituitary adenoma in over 70% of adults and in approximately 60-70% of children and adolescents. Most pituitary ACTH-secreting adenomas are small in size (microadenomas). Overall, Cushing's Disease is relatively rare, affecting 10 to 15 of every million people each year, and most commonly affects adults aged 20 to 50 years. Women account for over 70% of cases. Symptoms and signs of Cushing's syndrome and disease may include: Change in body habitus: weight gain in face (moon face), above the collar bone (supraclavicular) and on back of neck (buffalo hump) Skin changes with easy bruising, purplish stretch marks (stria) and red cheeks (plethora)

They exist "silently" in up to 5-10% of the adult population. Prolactinomas generally have very different presentations in women and in men. In women, relatively small elevations in prolactin cause irregular menstrual periods or complete loss of menses (amenorrhea) and may cause milky discharge from the breasts (galactorrhea). In addition, women may have a reduction in their sex drive. The normal prolactin level is hypopituitarism, especially hypogonadism), visual loss (from compression of the optic nerves or optic chiasm) and/or headache. A minority patients with large tumors may have acute hemorrhage into the tumor

Extractions: PITUITARY TUMOR PROGRAM PITUITARY DIAGNOSES INDEX These neoplasms account for approximately 15% of primary brain tumors and are the third or fourth most frequently encountered intracranial tumor after gliomas, meningiomas and schwannomas. From autopsy studies and magnetic resonance imaging (MRI) of normal individuals, it is known that 10-20% of the general population harbors a pituitary adenoma. Most of these tumors appear to remain small and do not cause the patient significant harm or symptoms. However some progress to cause major hormonal and neurological problems. Adenomas that are less than 1 cm in diameter are called microadenomas; those that are over 1 cm in size are called macroadenomas. The best way to determine if an adenoma is present in the pituitary gland is to perform an MRI of the pituitary without and with gadolinium (a contrast agent). Although an MRI is preferable, most pituitary macroadenomas and some microadenomas can be seen on a CT scan of the pituitary. (Above) MRI of Macroadenoma The great majority of pituitary adenomas are benign (not malignant) and are relatively slow growing. Most pituitary adenomas arise de novo and are not genetically inherited. Cases of familial pituitary tumors or inherited tendencies to develop pituitary adenomas are rare. Multiple Endocrine Neoplasia (MEN) type I accounts for less than 5% of cases of pituitary adenomas. This autosomal dominant condition is characterized by simultaneous tumors of the pituitary, pancreas and parathyroid glands. Pituitary adenomas develop in 25% of patients with MEN I.

The gland produces eight hormones. It hangs from the base of the brain. When the gland does not work right, it may produce either too much or too little of one or more of the hormones. Antidiuretic hormone (ADH), which is also called vasopressin, helps the kidneys (and body) conserve the right amount of water. For example, when a person becomes dehydrated, more ADH is produced to help the body conserve the water it contains. Lack of ADH leads to too much excretion of water (diabetes insipidus), and too much ADH leads to excessive retention of water by the body (syndrome of inappropriate antidiuretic hormone secretion).

Extractions: References The pituitary gland weighs about 0.5 to 1 gram and is divided into an anterior and a posterior lobe. It sits in the sella turcica immediately behind the sphenoid sinus. Cavernous sinuses are located laterally on each side of the sella, and include the internal carotid artery and cranial nerves III, IV, V , V , and VI. Magnetic resonance imaging (MRI) is the best method for visualizing the hypothalamic-pituitary anatomy, since the optic chiasm, vascular structures, and any tumor extension to cavernous sinuses can be well visualized compared with other imaging techniques (Figure 1) Anterior pituitary hormones are regulated by hypothalamic releasing and inhibitory hormones and the negative feedback action of the target glandular hormones at both pituitary and hypothalamic levels (Table 1) . Among pituitary hormones, only the secretion of prolactin is increased in the absence of hypothalamic influence, since it is mainly under tonic suppression through the prolactin inhibitory factor.
