Antidiuretic hormone (ADH), which is also called vasopressin, helps the kidneys (and body) conserve the right amount of water. For example, when a person becomes dehydrated, more ADH is produced to help the body conserve the water it contains. Lack of ADH leads to too much excretion of water (diabetes insipidus), and too much ADH leads to excessive retention of water by the body (syndrome of inappropriate antidiuretic hormone secretion). To better understand the frequency of pituitary tumors, Dr. Ezzat and a number of colleagues performed a meta-analysis of 13 different studies (eight autopsy studies and five medical imaging studies) that were designed specifically to examine and describe the nature of abnormalities within the pituitary gland. Across eight autopsy studies, the overall prevalence of pituitary abnormalities was 14.4 percent, and across the five imaging studies, the prevalence of pituitary abnormalities was 22.5 percent (one of the more recent autopsy studies used in the meta-analysis revealed that, consistent with the MRI findings, pituitary abnormalities were found in roughly one out of four subjects). The researchers found an overall prevalence of 16.7 percent when the numbers of patients from all studies were compared. Pituitary malfunctions result from infections, inflammation (especially around the time of pregnancy), injuries, cancerous spread and most commonly, non-cancerous pituitary tumors, called adenomas, according to Dr. Ezzat. Some pituitary tumors may be hormonally inactive and may be discovered incidentally during the course of MRI performed for unrelated reasons, such as head injury. While some pituitary tumors may be very small (microademomas), they have the potential to cause health complications through excessive hormone production.

For individuals with pituitary hormonal problems or a pituitary tumor, Dr. Kelly recommends seeking initial evaluation from an endocrinologist with specific expertise in pituitary disease or at a specialized pituitary clinic where endocrinologists and neurosurgeons see patients together during the same office visit: "Patients are best managed by a team approach that ensures all the bases are covered in terms of the hormonal and neurological issues, to determine the best treatment for a given patient, be it pharmacological therapy, surgery or radiation therapy and whether hormone replacement therapy is needed. Long-term follow-up is also needed given that pituitary tumors can recur over time and pituitary failure may develop after treatment."

Extractions: Pituitary Disorders The pituitary gland has been called the bodys "master" gland. It manages long-term growth, day-to-day functions, and reproductive processes. The gland produces eight hormones. It hangs from the base of the brain. When the gland does not work right, it may produce either too much or too little of one or more of the hormones. This causes changes elsewhere in the body. DISORDERS OF THE PITUITARY GLAND RESULT FROM: CONGENITAL AND GENETIC PROBLEMS: These usually affect the supply of the hormones. For example, a low supply of growth hormone slows the growth of a child. Dwarfism or other problems can result. INJURY: A head injury may stop the gland from working. IMPAIRED BLOOD SUPPLY: When the blood supply is cut off by a growing tumor or aneurysm, the gland may not be able to function. RADIATION: Decreased hormone production from the pituitary gland may result from radiation therapy used to treat a tumor. TUMORS: Pituitary tumors are usually benign or noncancerous. One type of tumor reduces the production of specific sex hormones. This can lead to a failure of sexual function. Monthly periods may stop in women and the amount of sperm may be reduced in men. Another type of tumor causes an excess of hormones. Rapid growth before adulthood may occur. The hands, feet, jaw, and skull may overgrow in size. Infertility and impotence may result. Diabetes may develop as well. If the gland enlarges, it can press on surrounding tissue and cause other symptoms, such as vision problems.

Extractions: Incomplete development of the pituitary gland during embryogenesis results in congenital pituitary hypoplasia. We are studying a severe X-linked form of this disorder, X-linked pituitary hypoplasia (XPH), which only affects males. We have shown using cytogenetic and linkage analysis of XPH families that this disorder results from increased dosage of a gene located at Xq26-q27. Using a mouse model system, we are investigating genes at Xq26-q27 for expression within the developing pituitary, and are testing the function of XH candidate genes in transgenic mice.
