Employees Priapism Priapism Priapism may occur in one of the following ways: There may be repeated, reversible painful erections occurring over several hours (penis becomes erect then erection goes away, then becomes erect again, then erection goes away, etc.) This is also called "stuttering" priapism. There is not a problem with sexual functioning once the episode has gotten better. Repeated episodes of this type of priapism are common and occur in up to 40% of patients. This type of priapism can usually be managed at home. Patients are encouraged to take warm baths, increase fluid intake and empty their bladder (urinate) often. If the episode is not better after 3 hours, notify the doctor. Take Tylenol for pain. There may be a prolonged, painful erection that does not go away for more than several hours. This can last for up to several days or weeks. This type of priapism needs attention by a doctor. Without medical treatment, severe priapism can lead to partial or complete impotence in over 80% of cases. Sometimes a blood transfusion is given during the attack.

It is relatively common in young males with sickle cell anemia. The onset may be in early childhood or adolescence. Recurrent or severe episodes can result in future impotence. Treatment is often not satisfactory. - Priapism. The diagnosis of priapism is usually obvious from the physical examination. Patients usually present with the spontaneous onset of erection which frequently occurs in the middle of the night. Can be precipitated by prolonged intercourse or masturbation. Penis remains erect and is very painful. If there is a history of nocturnal erection, pain and persistence for more than thirty minutes must be present to differentiate priapism from normal nocturnal erections. There are three basic patterns seen with priapism. - Stuttering Priapism. Many patients, especially children, have a pattern of multiple short episodes over a period of days or several weeks. The prognosis is generally good and therapy is conservative.

Priapism is drug induced, injury related, or caused by disease, not sexual desire. As in a normal erection, the penis fills with blood and becomes erect. However, unlike a normal erection that dissipates after sexual activity ends, the persistent erection caused by priapism is maintained because the blood in the penile shaft does not drain. The shaft remains hard, while the tip of the penis is soft. If it is not relieved promptly, priapism can lead to permanent scarring of the penis and inability to have a normal erection. Priapism is caused by leukemia, sickle cell disease, or spinal cord injury . It has also been associated as a rare side effect to trazodone (Desyrel), a drug prescribed to treat depression. An overdose of self-injected chemicals to counteract impotence has also been responsible for priapism. The chemicals are directly injected into the penis, and at least a quarter of all men who have used this method of treatment for over three months develop priapism.

Although many men may think this might be a good and desirable thing to have there are a number of reasons why it isn't. Firstly, it may be a real nuisance since priapism can occur when a man doesn't have any sexual desire. Secondly, priapism can damage the erectile tissues of the penis permanently. And thirdly, it can be extremely painful. Priapism occurs because the blood in the penis that forms the erection can't escape. This might be because of prolonged sexual intercourse, medical conditions such as polycythaemia, or drug side effects. Very occasionally drugs used to treat impotence can cause priapism too. The problem of priapism can be relieved immediately by releasing the blood from the penis. A doctor will do this at the local casualty (accident and emergency) department with a needle and syringe.

    Extractions: A 53-year old male was admitted with a painful priapism of 72 hours duration. No medication was involved: the patient had previously suffered with prolonged erections, lasting for a few hours. No predisposing factors for priapism have been identified. Corporal aspiration and aramine was unsuccessful in achieving decompression, therefore an Ebbehoj or modified Winter shunt was performed. This was temporarily successful, but the erection returned. A right cavernoso-spongiosus shunt was performed, which again was partially successful, the patient was catheterised post-operatively. The procedure was complicated by a brief post-operative infection and the patient developed a discharge from his urethra. After his operation the patient was discharged with a catheter in place with arrangements to come back into hospital after one week for a trial without catheter, and then again one week after that for flows as an outpatient. The patient returned to hospital and had his catheter removed. The penis was indurated and still partially tumescent. A trial without catheter (TWOC) was successful but there was some terminal haematuria.
