Extractions: December, 2002 Download this factsheet An enlarged prostate known as benign prostatic hyperplasia or BPH is caused by an overgrowth of prostate cells. This enlargement constricts the urethra so the flow of urine is reduced, making it increasingly difficult to empty the bladder. The prostate is a gland about the size of a walnut that is only present in men. It is located just below the bladder and surrounds the urethra, the tube through which urine flows from the bladder and out through the penis. Please see the diagram, overleaf. One of the main functions of the prostate is to produce an important liquefying component of semen, which allows the sperm to move freely. The gland is divided into three zones, peripheral, transitional and central. With BPH, it is the central part where overgrowth of cells takes place. BPH is very common, affecting about one third of men over 50. Although it is not prostate cancer, the symptoms of BPH are similar to those of prostate cancer so you should see your doctor if you start to experience problems passing urine. Prostate cancer is the most common cancer and the second most common cause of cancer death in American men, causing about 38,000 deaths in 1994. However, the vast majority of men with microscopic evidence of prostate cancer will never develop clinically significant disease. In recent years, the use of radical prostate surgery has increased dramatically for treatment of newly diagnosed, clinically localized prostate cancer.

Reviewed by Michael Zacks, M.D. Clinical question How do terazosin, finasteride and combination therapy compare in the treatment of benign prostatic hypertrophy (BPH)? Background A variety of therapeutic options are available for the treatment of BPH. These include prostatectomy, medical treatment with alpha-1-adrenergic antagonists such as terazosin or 5-alpha-reductase inhibitors such as finasteride, and no treatment. However, the comparative efficacy of medical therapies has not been assessed. Population studied Men aged 45 to 80 with symptomatic benign prostatic hyperplasia who were seen in Veteran's Affairs outpatient clinics were enrolled. No specified level of prostatic enlargement was required. Exclusion criteria included concurrent treatment with alpha adrenergic agonists, cholenergic agonists or antagonists, topical beta adrenergic antagonists; anti-hypertensive medications other than diuretics and ACE inhibitors; hormone therapy; cardiac, cerebrovascular, or other prostatic disease; diabetes mellitus; orthostatic hypotension; renal or hepatic impairment; and elevation of prostate specific antigen. Study design and validity This was a four-armed, randomized, placebo-controlled trial lasting 1 year which compared placebo, terazosin, finasteride, and a combination of the two. Patients in the study were similar in age, prostatic volume, American Urological Association symptom score, peak urinary flow, and prostate-specific antigen level. Withdrawal rates were significantly higher in the treatment arms compared to the placebo group; however, statistical analysis was appropriately done on an intention-to-treat basis.

Extractions: An extract of the fruit from the American saw palmetto plant has been used by native American Indians for hundreds of years to treat a variety of men's problems, and prostate gland swelling since the 1800s. Does it work? Systematic reviews [1,2] say that it does. Search The search for randomised controlled studies of Saw palmetto (using all those peculiar botanical names that plants have) against placebo or other treatments was thorough. It found 24 studies, 18 of which met the inclusion criteria. The mean age of men involved was 65 years and the mean duration of the studies was nine weeks. Results Compared with placebo, Saw palmetto improved peak urinary flow rate by an average of 1.9 mL/second and mean urinary flow rate by 2.2 mL/second (Figure 1). Saw palmetto did not decrease prostate size. Patient rating of prostate-related symptoms showed significant benefits for Saw palmetto over placebo. In six studies (Figure 2) 242/329 (74%) men had symptom improvement with Saw palmetto compared with 168/330 (51%) with placebo. This means that for every 4.4 men treated with Saw palmetto (95% CI 3.4 to 6.5) one will have symptoms improved who would not have done with placebo. The problems One problem was the short duration of studies, with a mean of nine weeks, and that half the men had symptom improvement with placebo. The authors present the results of a double-blind study vs. placebo in 40 patients with non-advanced benign prostatic hypertrophy [BPH]. The patients were treated with a standardized Pygeum africanum extract (at a dosage of 100 mg per day, administered orally, for 60 days). The clinical pre-treatment and post-treatment evaluation was based on the symptomatology, on the results of a rectal examination, and on an analysis of the results of urine flow measurements. Preliminary results reveal a statistically significant improvement in polyuria, urgency, dysuria, and urine flow in the patients who were treated with the active substance.
