Less commonly, volvulus, strangulated hernias, Crohn's disease, neoplasm and trauma necessitate massive resection. Two major types of diarrhea can develop after massive ileal resection. One is induced primarily by malabsorbed bile acids, and the other by malabsorbed fat. When the ileal resection is small (less than 100 cm), hepatic synthesis of bile acids is sufficient to compensate for increased fecal losses. The luminal concentrations of bile acids are maintained within the micellar range, and significant steatorrhea does not occur. However, with inadequate absorption in the terminal ileum, bile acids enter the colon, impairing electrolyte and water absorption. Thus the term "bile acid diarrhea" is applied to this circumstance.

When the ileal resection is extensive (greater than 100 cm), hepatic compensation for wastage of bile acids is incomplete and the concentration of bile acids in the lumen is too low for adequate micellar solubilization of fat. Steatorrhea results. Here the malabsorbed fat is primarily responsible for the diarrhea. With excessive amounts of fatty acids now in the colon, electrolyte and water absorption are further impaired.

Short Bowel Syndrome (SBS) short bowel syndrome. When a person loses or is born without a large portionof their small intestines, it is referred to as short bowel syndrome (SBS). http://www.fortunecity.com/millenium/lassie/717/short.htm
Extractions: web hosting domain names email addresses related sites Short Bowel Syndrome When a person loses or is born without a large portion of their small intestines, it is referred to as Short Bowel Syndrome (SBS). It can happen from a variety of circumstances, Hirschprungs Disease, bowel malrotation, Chrohns Disease, Gastrochisis as well as other diseases or malformations. The problem with SBS is that with only a small portion of intestine, the GI tract is unable to absorb the nutrients as easily from food ingested or tube fed (enteral feeds). Each person with SBS is different with regards to how well they can cope with oral or enteral feeds. Some are able to adapt well, and go on to have as normal a life as possible, while others end up TPN dependent . In situations where the ileo-cecal valve is missing (like Sean), it means that food moves along quite quickly and often leads to dumping syndrome (inability to process and absorb food in a normal fashion). If you are interested in learning more about SBS, or are looking for support if you have this, please go to the Short Bowel Syndrome web page.

Extractions: ReMedica Publishing, London U.K. with permission. This acute gastrointestinal disorder was first described in the German literature in 1825 [1], but is still something of an enigma. Despite years of investigation, the etiology remains unclear, and accepted prevention and treatment strategies are lacking. Necrotizing enterocolitis (NEC) predominantly affects premature infants; less commonly (10%), term newborns, and, rarely, severely debilitated children. There have been instances of nursery epidemics. Some centers see this problem affecting as many as 8% of newborns born weighing less than 1.5 kg, while other units have a very low incidence of only 0.3% among their premature babies. Several countries, for example Switzerland and Scandinavia, have a very low incidence. In a study involving 52 centers in Japan [2], the incidence was only 0.3%. In the USA, the incidence is 2.4 in 1000 live births [3,4]. NEC is the commonest neonatal gastrointestinal emergency in both the UK and America. Susceptibility to NEC is increased by the following: Extreme prematurity [5] Birth weight under 2.0 kg

Extractions: Welcome To The No Guts Country Club (Short Bowel Syndrome) Am I The Only One Left? Hi and welcome to The No Guts Country Club. My name is Jim Combs, just so you will know I am a real person. Every thing I say is from my experience in the last ten years with Short Bowel Syndrome, in fact I have what is called Extreme Short Bowel Syndrome. I have about 12 inches of active small intestine and about 10 inches of small intestine that was really not useable, but was viable enough to use as a conduit and reconnect. This give the Doctors enough that I could be re-connected and not have an ostomy. I have about 22" total out of a possible 22 FT or so that is normal My small Intestine is made up of short pieces, mainly some of the "loops", sewn together, and according to the Doctors the longest single piece of intestine is "almost" 6" long. I have found that if I stretch while reaching to a top shelf, or have a "jar" by hitting a "chug hole" in the road, or even the shaking of a ridding lawnmower, it stretches the intestine enough that I have terrific pain. I don't dare go on a ride in an amusement park.

Suzanne K. Thomas - Short Bowel Syndrome We did it. Thank God we did it. We want all of you who have a child with short bowelsyndrome to know that this is not your fault and you must stay positive. http://www.thomasholdingco.com/short_bowel_syndrome.htm
Extractions: Suzanne K. Thomas was born on 3/31/00. Stephanie had a perfect pregnancy and was going to deliver one month early. When the baby was born cesarean section the baby looked perfect and beautiful but the doctors were sucking out brownish/green fluid from her mouth. We asked if this was normal and the doctor said"yes, but not this much." The baby's tummy looked very bloated. The doctors continued to suck this bile or mconium from her mouth and stomach for over 24 hours. The doctors told us that she most likely had cystic fibrosis. She has a 90% chance of having cystic fibrosis they said. Turns out that 36 hours after she was born the staff decided to do a surgery to see what was wrong and they found that her small intestine was pinched off in utero and most of her small intestine had evaporated. They called this an atresia. What should have been 150 to 200 centimeters of small intestine was now 30 and was sown together in 4 places. The staff explained that nothing was certain. All you can do is wait and drip feed Suzanne through a g-tube and hope that her small intestine grows and adapts to her body. She had a central line that fed nutrition into her body through her veins. This nutrition is called t.p.n

Other symptoms include cramping, bloating, and heartburn . Many people with short bowel syndrome are malnourished because their remaining small intestine is unable to absorb enough water, vitamins, and other nutrients from food. They may also become dehydrated, which can be life threatening. Problems associated with dehydration and malnutrition include weakness, fatigue, depression, weight loss, bacterial infections, and food sensitivities. Short bowel syndrome is treated through changes in diet, intravenous feeding, vitamin and mineral supplements, and medicine to relieve symptoms.

Causes and Risks The lack of adequate surface to absorb nutrients from food occurs when specialized areas of the bowel are removed. When one-half of the bowel or more is removed during surgery, the risk of small bowel syndrome is great. Risk factors include diseases of the small intestine that may require surgical intervention such as regional enteritis or Crohn's disease Necrotizing enterocolitis is a common cause of this syndrome in infants. Nursing Spectrum- Career Fitness Online short bowel syndrome and the Hospitalized Pediatric Patient Janet Ashmore, RN,BS. short bowel syndrome new therapeutic approaches. Curr Probl Surg. http://nsweb.nursingspectrum.com/ce/ce97.htm
Extractions: Janet Ashmore, RN, BS Interestingly, the exact mechanisms leading to intestinal adaptation are still not known. Clinical Snapshot In any event, pediatric patients with SBS are at risk for numerous complications. These babies have frequent episodes of diarrhea and fluid and electrolyte imbalances. Inadequate vitamin intake can cause cracked skin at the corners of their mouths and sore tongues from riboflavin deficiency, or bleeding because of a vitamin K deficiency. Anemia secondary to the problems of malnutrition may occur. And patients who require a central venous line (CVL) are at risk for developing life-threatening sepsis. Other chronic conditions include changes in bacterial intestinal flora, including bacterial overgrowth and parenteral nutrition-induced liver disease, which can be deadly. The most common cause of death in these children is, in fact, liver failure as a consequence of the parenteral nutrition required during the first years of treatment.
