It can occur when you are breathing in or out. What causes snoring and who is at risk? Snoring occurs when air does not flow smoothly through the air passages, or when the soft tissues or muscles in your air passages vibrate. As you fall into a deep sleep, the muscles in your tongue, throat and roof of your mouth (soft palate) relax. This muscle relaxation causes your throat tissues to sag. As you breathe, the sagging tissues narrow your airway and vibrate or flutter, creating the sound of snoring. The narrower your airway becomes, the greater the vibration...and the louder your snoring. Snoring affects 45 per cent of the population from time to time and an estimated 25 per cent of people are habitual snorers. There are a number of reasons why you may snore. Age can be a factor. The older you get, the weaker your throat muscles become. Weak throat muscles cause the surrounding tissues to sag and vibrate. If you are overweight, your throat tissues are less firm and more inclined to vibrate when you breathe.

The uvula and soft pallet collapses on the back wall of the upper airway. Then the tongue falls backward, collapsing on the back wall of the upper airway, the uvula and soft pallet forming a tight blockage, preventing any air from entering the lungs. The effort of the diaphragm, the chest and the abdomen only cause the blockage to seal tighter. In order to breathe the person must arouse or awaken, causing tension in the tongue thereby opening the airway, allowing air to pass into the lungs. When the SaO2 drops the heart will start pumping more blood with each beat. If the SaO2 continues to drop the heart will start beating faster and faster. As the CO2 increases the brain will try to drive the person to breathe. The effort and action of the abdomen and chest will increase. Eventually that action can become severe enough to cause an arousal, clearing the upper airway blockage, allowing the person to breathe. Then you go back to sleep and it happens all over again. The American Sleep Disorder Association rates the average number of OSA events per hour as your Respiratory Distress Index (RDI). An RDI of to 5 in normal; 5 to 20 is mild; 20 to 40 is moderate; over 40 is considered severe. An apnea event must last at least 10 seconds to be considered an event. It is not uncommon to see RDIs well above the 40. In some cases RDIs were well above 100, with events lasting as long as 90 to 120 seconds and SaO2s going below 70% when normal is 95% to 100%.

Yet, we tend to laugh at snoring, get angry at the snorer for keeping us from sleeping, or dismiss snoring as a sign of good sleep. Snoring may sound like a fog horn to warn sailors of danger. In fact, snoring is certainly not funny-it is a warning of danger. Unfortunately, the sleeper does not hear it, and those who do, usually don't recognize it as a danger signal. When a person with sleep apnea syndrome falls asleep, the relaxation of the body leads to an excessive relaxation of the throat, a narrowing of the airway, and snoring. If the airway is completely blocked, even though the muscles of the diaphragm and chest are struggling to draw in air, no air can move past the blockage-apnea means without breath. The extra effort due to snoring, or partial or complete obstructions, arouses the brain. As soon as the brain awakens, the airway opens (sometimes with an explosive gasp), breathing resumes, and sleep can begin again. Usually the sleeper is not aware of the hundreds of arousals each night which keep him or her from getting normal sleep. Like a phantom in the night, unseen and unsuspected, sleep apnea syndrome ruined my health, my happiness, disrupted my family, friendships, and work-leaving me nearly destitute. As a result of my constant fatigue, I was unable to function in my family, business, or social life. Yet, although everyone who knew me was aware that I was a world-class snorer, no one recognized the danger signals.
