Causes, incidence, and risk factors Return to top In somatization disorders, there tends to be a pattern of multiple and recurrent symptoms and complaints that can involve any body system and often persist for years. Most frequently, the complaints involve the gastrointestinal system, the nervous system, the cardiopulmonary (heart and lungs) system, or the reproductive system. The symptoms begin before the age of 30 and occur more often in females. The symptoms are severe enough to lead to the person to visit the doctor or take medication, and may interfere with the person's life. The cause is not specific but symptoms begin or worsen after losing a job, close relative, or friend, or other loss. A lifelong history of sickliness is often present, and often people with this disorder relate to others only through their symptoms.    Extractions: Somatization disorder is a syndrome described by diagnostic criteria found in DSM-IV, the current diagnostic and statistical manual published by the American Psychiatric Association. I practiced psychiatry for 20 years and had the opportunity to evaluate and follow over many years patients with this disorder. Many patients with CFS argue that this is an illness that is invented by psychiatrists. They are partly right. The illness has quite narrow criteria. The problem is that many doctors broaden the definition to include many CFS patients thereby harming them. The patients are justifiably angry. To make matters worse, some of the doctors who do this write scholarly books to support their position. Generally, the books are very intellectual but not very mindful I could take the position that the illnesses doesn't exist, but that would be negating my experience. I saw a lady once with hysterical paralysis of her limbs. A skillful neurologist can easily tell the difference between paralysis caused by nerve damage and hysterical paralysis. I could write a book about my many experiences and distinguishing diagnostic features. The doctor must ask very specific questions in order to make the diagnosis. In addition to applying the DSM IV criteria, I used the

A significant percentage of non-responders turned out to have somatization disorder. Since this condition is easy to diagnose if the interview is structured properly, many patients can be spared the agonies of unsuccessful medication trials. Somatization disorder is a disease presenting with multiple emotional and somatic symptoms seen predominately in young females. The condition tends to have a long chronic course, but generally improves somewhat after the age of 40. The patients NEVER fully recover. Somatization disorder was known by other names as long as 4,000 years ago by the Egyptians. They conceived of the disease as caused by a wandering uterus which wandered up to the shoulder and caused shoulder pain, and down to the knee and caused knee pain, and to the back, causing back pain, etc. They thought the uterus was not properly attached in place and put various sweet smelling poultices between the legs to entice the uterus back to the correct position.

The bodily symptoms cannot be explained by any known medical disorder or pathophysiological mechanism. Symptoms of pain or sexual dysfunction are specifically excluded. Such patients are not conscious of the psychological basis for their symptoms and therefore cannot control their disturbances. Conversion reaction in children had been described also as a form of "body language", a plea for help in a child who has no alternative method of communicating stress and therefore, conversion reaction doesn't usually indicate a major psychiatric disorder. The diagnosis depends on a determination of etiology: the psychological factors must be judged to be causative, as evidenced by the presence of primary or secondary gain. In primary gain, the conversion symptom alleviates psychic distress. Whereas, in secondary gain, the formation of the symptoms enables the patients to avoid difficult situations or to obtain support that might not otherwise be provided. Several theories have been proposed to explain the etiology of conversion reaction. According to the psychoanalytic theory, conversion is caused by the anxiety of the unconscious intrapsychic conflict. Another etiology suggests that conversion disorder is a nonverbal interpersonal communication, employed when direct verbal statement is not possible. It is an acceptable way of telling important others that their relationship is in danger or that he or she is in pain, is suffering, and is in need of special consideration and special treatment. It also may function as a nonverbal means of controlling or manipulating others. Some believe that conversion reaction may have a neuropsychological basis.

It is generally well accepted that stress and worry can take a physical toll on bodies. This can be the unseen source of headaches, backaches, chest pain, and stomachaches. Somatic complaints can be the presenting and/or comorbid symptoms of childhood depression and anxiety disorders (Livingston, 1988; Wilens, 1990; Sadeh, 1994; Hodgman, 1995). Conscious and unconscious worries can lead to somatic symptoms with a spectrum of degrees of severity in almost every organ system.

Body Dysmorphic Disorder Somatization Disorder A chronic, severe psychiatric disorder characterized by many recurring clinically significant physical complaints (including pain and GI, sexual, and neurologic symptoms) that cannot be explained fully by a physical disorder. A narcissistic personality (ie, marked dependency and intolerance of frustration) contributes to the physical complaints, which appear to represent an unconscious somatized plea for attention and care.
