The neurological examination is normal. Most frequently the skin is normal over the vertebral defect however, associated dimples, sinuses, increased hair, fatty lump and hemangiomata (swelling consisting of blood vessels) may indicate the presence of spina bifida. Meningocele: Meningocele becomes evident at birth with observation of a cystic skin covered midline mass over the back. Half of one or more vertebrae are absent and the sac is composed of CSF (brain fluid), meninges (tissues covering the brain and the spinal cord) and skin. The reminder of the CNS is not involved and the prognosis for development is excellent. About 10% of the patients with spina bifida will have meningocele.

Hierdoor ligt het ruggenmerg aan de oppervlakte. Het ruggenmerg zelf ligt ter plaatse ook "opengeklapt". Er zijn ook vormen waarbij de het ruggenmergsvlies de buitenste begrenzing vormt. Het ruggenmerg kan overlangs in twee helften gespleten zijn. Bij intacte huid maar open wervelbogen spreekt men van spina bifida occulta ("verborgen"). Als gevolg van de afwijking bestaat er een uitval van de neurologische functies onder het niveau van de open rug. Dit betekent in meerdere of mindere mate een verlamming van de benen en meestal ook uitval van de controle over de sluitspieren. Hoe lager het defect, hoe gunstiger het uitvalt. Enkele centimeters kunnen het verschil tussen lopen en een rolstoel betekenen.

Embryonalwoche und wird zusammen mit der Anenzephalie Spina bifida heißt wörtlich übersetzt "gespaltenes Rückgrat". Es handelt sich um eine Fehlbildung im Bereich der Wirbelsäule und des Rückenmarkes. Da sich die Wirbelsäule und das Rückenmark aus dem Neuralrohr entwickeln, spricht man auch von einem Neuralrohrdefekt. Normalerweise verschmelzen die beiden Bogenanteile der Wirbel zu einem Ring, der das Wirbelloch nach hinten begrenzt. Im Wirbelloch selbst liegt das Rückenmark, umgeben von den Rückenmarkshäuten. Bei der Spina bifida bleibt der Bogenschluss eines oder mehrerer Wirbel aus. Durch den Spalt können sich die Rückenmarksanteile und Nerven sackförmig vorwölben (Myelomeningozele). Befindet sich nur Hirnhaut im Bruchsack, handelt es sich um eine Meningozele. Symptome Harnweginfekte sowie Entleerungsstörungen der Blase, sowohl im Sinne einer "Durchlaufblase" als auch einer "Überlaufblase" beobachtet.
