In Canada, rates are highest in Newfoundland (with diets traditionally high in salted and cured foods) and low in British Columbia. Although the overall incidence of stomach cancer is decreasing, there has been a rise in the incidence of proximal and gastroesophageal junction cancers. Dietary risk factors are important in this disease. Risk is reduced with the consumption of fresh fruits and vegetables, and increased with higher nitrate consumption from pickled, salted, and cured foods. Refrigeration and improved food preservation (with less conversion of nitrates to carcinogenic nitrites) have contributed to the decline in incidence of stomach cancer. Other risk factors are associated with either upper or lower stomach cancer. Family history and infection with Helicobacter pylori bacteria are associated with lower stomach cancer; whereas alcohol, smoking and hiatal hernia are associated with upper stomach cancer. Familial syndromes such as hereditary nonpolyposis colorectal cancer, Li-Frameni , familial adnomatous polyposis and Peutz-Jeghers syndrome can all be associated with gastric cancer. Hereditary diffuse gastric cancer (HDGC) is a rare disorder, probably only accounting for 1-3% of gastric cancers. HDGC is transmitted in an autosomal dominant pattern and the penetrance is 60-80%. The BCCA is involved in research on HDGC.

But despite a wealth of scientific data, most people remain unaware of how they can reduce their risk of developing cancer. While no one is certain exactly how fiber protects against digestive tract disorders, there are several possibilities. By definition, fiber cannot be digested by humans early in the digestive process. It moves food more quickly through the intestines, helping to eliminate carcinogens. It also draws water into the digestive tract. The water and fiber make fecal matter bulkier, so carcinogens are diluted.

This similarity is one of the reasons Lawrence Livermore scientists are studying mice. At Livermore's Human Genome Center, biomedical scientist Lisa Stubbs is leading a team that is studying the mouse genome to better understand the functions of human genes. Comparative genomics-analyzing and comparing the genetic material of different species-is viewed by bioscientists as an important method for studying evolution, the functions of genes (what they do and why), and inherited diseases. Hunting Down Damaged Chromosomes To find out which gene is responsible, a researcher takes a small snip of skin from a mouse tail and grows the skin cells in a petri dish. Chromosomes from those cells are then spread on a microscope slide. In the laboratory, the researchers look for one particular kind of mutation called a translocation, which involves obvious changes in chromosome structure. Because the chromosomes are visibly disrupted, researchers can easily map the position of the mutated gene using only a simple light microscope. For example, the figure at right shows two mouse chromosomes-2 and 14-where such a translocation has occurred. "We knew immediately that the gene responsible for the trait would be found on one of those two chromosomes," Stubbs explains. The researchers then use a procedure called fluorescent in situ hybridization (FISH), a technique for painting chromosomes with a fluorescent dye, to pinpoint the gene's location. They label a gene from a normal chromosome 2 with the fluorescent dye and add it to a slide containing the mutant mouse chromosomes. The labeled gene probe recognizes DNA sequences spread over the slide that are identical to its own and binds to the chromosome at that site. "In this way," Stubbs says, "we can map any gene relative to the translocation and `trap' the mutated gene in a small, well-defined region."

According to David Forman, Professor of Cancer Epidemiology at the University of Leeds, UK, there is "persuasive evidence" from various studies linking Helicobacter pylori infection to stomach cancer. Professor Forman stated, "A recent meta-analysis shows that these are now consistent in showing an approximate three to sixfold increased risk of non-cardia stomach cancer in H. pylori positive individuals (Eur J Cancer 2001; 37 Supp. 6: 248). This indicates that at least half of all non-cardia cancer can be attributed to the infection. Apart from smoking, very few established causes of cancer have been associated with an attributable risk of this magnitude.
