Occasionally, both strep throat and scarlet fever are transmitted indirectly by contact with objects used by infected persons or contaminated hands. Group A streptococci have also been associated with foodborne outbreaks. Individuals with strep throat frequently have fever and an inflamed, painful throat with swelling of the tonsils. Patients with scarlet fever may have all of the symptoms associated with strep throat, plus a fine, reddish rash. The rash commonly appears on the neck, chest, armpits, elbows, groin, and inner surfaces of the thighs. In severe cases of scarlet fever, "strawberry tongue," vomiting, and high fever may also be present. During recovery from scarlet fever, the skin may slough off the fingers and toes.

The diagnosis requires examination by a health care professional who can distinguish this disease from other upper respiratory infections. Diagnosis of strep throat can only be confirmed by laboratory testing. Symptoms The symptoms vary from a mildly sore throat with virtually no other complaints, to a severe illness with difficulty swallowing, high fever, painful swollen throat and swelling of the lymph glands in the neck. It is generally of a more sudden severe onset than the more gradual low grade discomfort seen with viral sore throat. Treatment Treatment requires antibiotics, usually penicillin or erythromycin, for a period of ten days. With appropriate medication, patients are no longer contagious after 2 or 3 days. Additional treatment consists of rest and salt water gargles to relieve discomfort (1/2 tsp. of salt in one glass of warm water), and aspirin or tylenol to reduce fever and pain. The treatment of sore throats due to viral infections with antibiotics is a practice to be avoided. Viral sore throats do not heal faster or more completely with antibiotic treatment, and the use of unnecessary antibiotics only assists your body in colonizing antibiotic resistent strains of common bacteria which can cause other infections.

Extractions: Researchers are studying a rare condition that occurs in children called Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal infections (PANDAS). In PANDAS, children suddenly show the symptoms of obsessive-compulsive disorder (OCD), Tourette's syndrome, or nervous tics right after they have strep throat. OCD causes people to dwell on thoughts or repeat certain actions over and over again. Tourette's syndrome and nervous tics cause patients to make sudden, abnormal, or uncontrollable movements with their bodies. Sometimes people with Tourette's can't control their speech or have short attention spans. Researchers think PANDAS involves a problem with the immune system. To fend off infection and heal the body, the immune system creates antibodies that attack germs. Sometimes, researchers suspect, instead of targeting the infection, the immune system mistakenly attacks the part of the brain that controls movement. Symptoms of PANDAS occur suddenly. After the strep infection clears up, the symptoms slowly fade. PANDAS may last a few weeks to several months. It may reappear if strep hits again. And symptoms will come on as quickly and as strongly as they did during the first attack. Although PANDAS is largely a mystery, there are treatments to help control some of its symptoms. New treatments to suppress PANDAS before it strikes also show great promise. Many clinical trials of drugs are in progress through the National Institute of Mental Health.

Sometimes the tonsils are also infected ( tonsillitis ). The disease is caused by group A Streptococcus bacteria. Untreated strep throat may develop into rheumatic fever or other serious conditions. Strep throat accounts for between 5-10% of all sore throats. Although anyone can get strep throat, it is most common in school-age children. People who smoke, who are fatigued, run down, or who live in damp, crowded conditions are also more likely to become infected. Children under age two and adults who are not around children are less likely to get the disease. Strep throat occurs most frequently from November to April. The disease passes directly from person to person by coughing, sneezing, and close contact. The "rap sheet" on this organism, also known as group A streptococci or GAS, stretches long: GAS infection can lead to strep throat, scarlet fever, the skin infection impetigo, pneumonia, acute kidney inflammation, toxic shock syndrome, blood "poisoning," acute rheumatic fever, rheumatic heart disease, and the flesh-eating disease known as necrotizing fasciitis. "This exceptionally virulent organism is difficult to study because it infects only humans and very few animal models of group A strep diseases exist," says Anthony S. Fauci, M.D., director of the National Institute of Allergy and Infectious Diseases (NIAID). "We need to know more about how group A strep interact with humans to cause so many different illnesses. The genetic sequence should shed light on these questions and pave the way for better treatment and prevention." "Infection with this bacterium occurs worldwide, and acute rheumatic fever is the major cause of heart disease in children of developing countries," says Fran Rubin, Ph.D., a respiratory diseases program officer at NIAID. "This is one reason why sequencing this organism is so critical." Several million cases of strep throat and impetigo occur each year in the United States. In addition, in 1999 GAS infection led to 9,400 more serious illnesses such as toxic shock syndrome or necrotizing fasciitis in the United States.
