Extractions: November 2002 Syphilis WHAT IS SYPHILIS? Syphilis is a sexually transmitted disease (STD), once responsible for devastating epidemics. It is caused by a bacterium called Treponema pallidum . The rate of primary and secondary syphilis in the United States declined by 89.2 percent from 1990 to 2000. The number of cases rose, however, from 5,979 in 2000 to 6,103 in 2001. The U.S. Centers for Disease Control and Prevention reported in November 2002 that this was the first increase since 1990. Of increasing concern is the fact that syphilis increases by 3- to 5-fold the risk of transmitting and acquiring HIV (human immunodeficiency virus), the virus that causes AIDS (acquired immunodeficiency syndrome). HOW IS SYPHILIS TRANSMITTED? The syphilis bacterium is very fragile, and the infection is almost always transmitted by sexual contact with an infected person. The bacterium spreads from the initial ulcer (sore) of an infected person to the skin or mucous membranes (linings) of the genital area, mouth, or anus of an uninfected sexual partner. Syphilis is a complex sexually transmitted disease (STD) caused by the bacterium Treponema pallidum . It has often been called "the great imitator" because so many of the signs and symptoms are indistinguishable from those of other diseases. How is syphilis spread? Syphilis is passed from person to person through direct contact with a syphilis sore. Sores occur mainly on the external genitals, vagina, anus, or in the rectum. Sores also can occur on the lips and in the mouth. Transmission of the organism occurs during vaginal, anal, or oral sex. Pregnant women with the disease can pass it to the babies they are carrying. Extractions: by Richard B. Roberts, M.D. Epidemiology: Between 1986-1990, the incidence of primary and secondary syphilis was reported to be at its highest level in forty years. Fortunately, in 1994 cases reported dropped to 450. Syphilis primarily occurs in the lower socioeconomic heterosexual population. Noteworthy is the dramatic increase in the number of reported syphilis cases among black women. From 1984-1989 the cases among black men rose 129% and among black women 192%. This is quite different from the cases reported in the late 1970's and early 1980's which primarily occurred in homosexual white men. Cases among this group has diminished dramatically, presmably because of the efforts of organizations such as Gay Men's Health Crisis which have educated people in the practice and importance of safe sex. Treponema pallidum is a spirochete 5-15 micrometers in length. The organism is most easily seen under the microscope using dark-field examination or silver stain. :Silver stain demonstrating the coiled spirochete Incubation period: 10-90 days with an average of 21 days.

Yet in addition to the medical and racist parallels, the PHS offered the same morally bankrupt defense offered at the Nuremberg trials: they claimed they were just carrying out orders, mere cogs in the wheel of the PHS bureaucracy, exempt from personal responsibility. In 1990, a survey found that 10 percent of African Americans believed that the U.S. government created AIDS as a plot to exterminate blacks, and another 20 percent could not rule out the possibility that this might be true. As preposterous and paranoid as this may sound, at one time the Tuskegee experiment must have seemed equally farfetched. Who could imagine the government, all the way up to the Surgeon General of the United States, deliberately allowing a group of its citizens to die from a terrible disease for the sake of an ill-conceived experiment? In light of this and many other shameful episodes in our history, African Americans' widespread mistrust of the government and white society in general should not be a surprise to anyone.

The disease begins with a chancre which normally forms on the genital area. If untreated, this chancre will heal but the disease will progress possibly causing many complications- and in some instances death. Syphilis can now be effectively treated with penicillin or other antibiotics (for those allergic to penicillin). The widespread use of antibiotics has reduced the prevalence of syphilis significantly. What are the symptoms of syphilis? Symptoms of syphilis undergo several stages of development. Throughout these stages, the symptoms felt by the patient vary. The Primary Stage This first stage of syphilis is characterized by a lesion (or chancre) which normally forms around the genital region. In about half of the cases reported the lesion has a raised, firm red border. The chancre is generally a dark pink color and does not bleed easily. It must be stressed that the appearance of this chancre can vary greatly. Furthermore, it is not necessarily a single lesion; multiple lesions may exist but are less common. If the lesion is untreated, it may heal on its own. However, without the attention of a physician the disease will progress.

"About 22 percent of African-Americans who we surveyed in the Birmingham area had some mistrust with regards to participating in research studies because of the Tuskegee Syphilis Study," said Lee Green of the University of Alabama. The Tuskegee study is a symbol of racial and scientific exploitation. "We found that it's both government and doctors. A lot of the people we spoke with mentioned how they're treated in health settings," Green said. James Jones' book, "Bad Blood: The Tuskegee Syphilis Experiment," chronicles the study that ran from the 1930s into the 1970s. "They did not understand that treatment was being withheld. They did not understand they had syphilis," Jones said. "They were not given enough information to make anything like an informed decision."

The final report of the Committee from May 20, 1996 is abstracted and presented in its entirety below: Abstract of the Syphilis Study Legacy Committee Final Report of May 20, 1996 From 1932 to 1972, 399 poor black sharecroppers in Macon County, Alabama were denied treatment for syphilis and deceived by physicians of the Unites States Public Health Service. As part of the Tuskegee Syphilis Study, designed to document the natural history of the disease, these men were told that they were being treated for "bad blood." In fact, government officials went to extreme lengths to insure that they received no therapy from any source. As reported by the New York Times on 26 July 1972, the Tuskegee Syphilis Study was revealed as "the longest nontherapeutic experiment on human beings in medical history." The Study continues to cast a long shadow over the relationship between African Americans and the bio-medical professions; it is argued that the Study is a significant factor in the low participation of African Americans in clinical trials, organ donation efforts, and routine preventive care. In view of this unacknowleged wrong and the the damage it has caused, the Tuskegee Syphilis Study Legacy Committee pursues two inseparable goals:
