Although syphilis remains a significant health problem in the United States, the number of cases reported nationally and in Illinois during 1998 was the lowest in any year since the mid 1960s. Preventing syphilis is especially important because persons infected with the disease are at increased risk for acquiring and transmitting HIV, the virus that causes AIDS. The U.S. Centers for Disease Control and Prevention, in collaboration with state and local health departments and with affected communities, is launching a national campaign to eliminate syphilis in the United States by 2005. This goal is within reach for two important reasons: the low number of reported cases in the past few years and the fact that half of all new cases in 1998 were reported in only 28 U.S. counties. Cook County, which includes Chicago and surrounding communities, was one of those counties and ranked second in the nation in 1998 in reported cases. What is syphilis?

It usually penetrates the moist mucous membranes of the mouth, the vagina, and the penis urethra, through which urine passes from the bladder to the outside. Symptoms The first stage can occur any time between two to four weeks after infection has occurred. A painless ulcer develops at this site most of the time. The ulcer starts as a red spot, turns into a pimple, which ulcerates, forming a sore. The ulcer is usually found on the genitals or anus, but can appear on any part of the skin, including the mouth. It usually heals in 4-6 weeks, leaving a thin scar. During this period the bacterium is circulating in the blood throughout the body. The second stage appears one to six months after the ulcer heals, you feel generally ill, you have a fever and a headache and lose your appetite. The glands in your neck, armpit and groin may swell. Most people develop a skin rash that does not itch. The spots appear on the palms of the hands and the bottom of the feet.

Syphilis is passed when someone touches a sore on a person who has syphilis. These sores are can be on the person's mouth, penis, vagina, anus, or skin. Most syphilis is passed during oral, vaginal, or anal sexual contact, though it can be passed through breaks in the skin and infected females can pass it to their baby if they become pregnant. If a baby is born with syphilis, it often has serious mental and physical problems.

The most widely accepted theory is that the venereal form of the disease arrived on the shores of Europe along with Christopher Columbus's crew, when they returned in 1493 from a journey to the New World. Indeed, although no cases of the disease seem to have existed in Europe before Columbus sailed to the New World, it had reached epidemic levels on the continent by around 1500. But in recent years, pre-Columbian skeletons such as those unearthed at the Hull friary in England have been found with distinctive signs of syphilis. Those skeletons have turned the nice, tidy picture of New World origins into a muddy mess. The sexually-transmitted form of syphilis is caused by a corkscrew-shaped bacterium called Treponema pallidum, which is one of a closely-related group of bacteria called the treponomes. Other treponomes are responsible for the three non-venereal forms of syphilis, which primarily affect the skin and are most common in early childhood. Bejel, also caused by Treponema pallidum, is prevalent among Bedouin tribes and elsewhere in the Middle East; pinta, caused by the Treponema carateum bacterium, is common in Central and South America; and yaws, the result of infection with the Treponema pertenue bacterium, is found in moist, tropical regions throughout the world. Venereal syphilis probably mutated out of one of those other forms most likely, researchers say, from the bacterium that causes yaws. When that happened, however, is the big mystery.

However, the approach is sufficiently different (and there is material here which does not appear in SSS), that I am leaving this paper available on-line. Why did syphilis become such a reverberating topic around the turn of the nineteenth and twentieth centuries? Was this simply because it clearly encoded several resonant anxieties around sex and gender at a time of ever-accelerating social change and increasing threats to the self-confidence of the 'advanced' Western nations which throughout the nineteenth century had regarded themselves as at the pinnacle of evolution? Syphilis certainly provided a potent metaphor for a plethora of moral concerns, but at the end of the nineteenth century also stood as a ghastly reminder of the inadequacies of medical and sanitary science. The nineteenth century had seen many successes in the war against disease: improved sanitation and water supplies had largely eradicated the epidemics of typhoid and cholera which had seemed so devastating in the earlier decades of the century. Isolation hospitals further prevented the wildfire spread of infectious diseases, while vaccination (albeit a contested practice) had made significant inroads towards eradicating this dangerous and disfiguring ailment.
