From the author: Thirteen persons diagnosed with amyotrophic lateral sclerosis were interviewed to determine the factors involved in making a decision whether to use mechanical ventilatory support. All study participants believed that they alone should make the decision regarding use of mechanical ventilation. The factors they considered to be of most importance were quality of life, severity of disability, availability of ventilation by means of a nasal mask, possible admission to a long-term care facility, ability to discontinue use of mechanical ventilation, desire to live, concern for their families' emotional health, and concern for care demands placed on their families. Suggestions are offered to facilitate the patient's decision-making process through provision of more information and improved dialogue between patient and professional.

An attempt has been made to study the ability of patients with amyotrophic lateral sclerosis (ALS) to 'plan' a task. Electroencephalogram (EEG) data corresponding to 'planning of a movement task' is analyzed in comparison with a normal relaxed state. The study was conducted on 12 patients with ALS (6 males, 3 females, mean age 46.75 years) and on same number of controls (10 males, 2 females, mean age 48.75 years) to evaluate a difference in the ability to 'plan' a movement task between them. Patients with ALS were divided in two groups defined by unclear/clear speech. It has been observed that patients with ALS having unclear speech (Group I) showed considerable reduction (p<0.0001) in 'planning' a movement task, whereas patients with ALS having clear speech (Group II) showed no deficit in 'planning' a movement task (p=0.0577), both in comparison with age-matched controls. Apart from supporting the earlier reports of a possible extended neuronal degeneration across wide area of the frontal lobes, the findings reveal a possible reduction in planning, an executive function of the prefrontal cortex of brain, and also reveal that speech impairment may be associated with cognitive deficits in patients with ALS. ... There is one FDA approved drug, riluzole, that modestly slows the progression of ALS.

Quite a number of approaches to treat ALS are being investigated in clinical trials:of the 20 different approaches, the majority (16) are small molecule based treatments, followed by biologics such as proteins, peptides and antibodies and more recently cell-based therapies have emerged for clinical testing. Among the targets being clinically tested are glutamate based therapies, antioxidants, cellular energy regulators, immunomodulators and neuroprotective/neurotrophic therapies. Biologic therapies study the neuroprotective effects of VEGF or are directed at the misfolded superoxide dismutase (SOD-1) among other approaches. Cellular therapies intent to induce regeneration of neuronal cells and tissues.

The report includes a compilation of current active projects in research and development of small molecules, antibodies, proteins, peptides, vaccines and other biologics including cells for treatment of amyotrophic lateral sclerosis (ALS).
