The way we view chronic heart failure management has changed dramatically over the last 10 years. We can no longer plan our chronic therapy based on acute hemodynamic benefits because of pharmacological manipulation. While agents such as direct vasodilators and inotropes acutely improve hemodynamics, they may often worsen the biology of the myocyte. More recent therapeutic agents, neurohormonal antagonists such as ACE inhibitors and B-blockers do just the opposite. These agents may acutely cause hypotension or worsening heart failure because of their pharmacologic effects, but long-term they improve the biology of the myocyte. This leads to improved ventricular function over time, reverse remodeling, and improved clinical outcomes. Discussed are the pathophysiology of heart failure and its surgical and medical treatment. Particular attention is paid to the molecular genetics of heart failure, the pathologic remodeling process, the importance of neurohormonal activation and free radical formation in this pathologic process, and the role of apoptosis. Also examined are the newest therapeutic options including medical therapy, surgical therapy, and genetic therapy of heart failure (taken from the Preface by Eric J. Eichhorn, MD).
