If there were an award for the Best Chapter Endings, D.P. Lyle would be the hands down winner. A master in the art of tethering readers to each and every crisp, deftly written word, his bold imagery snakes through the pages of his latest action packed thriller "Stress Fracture," making it physically painful to put down.

Dub Walker--an expert consultant in crime scene investigation--is shaken to the core by the butchery as he lingers in the home and now crime scene of his friend and former Huntsville Sheriff Mike Savage. Rage and unrelenting anger are the only clues that seep from the scene, linking the brutality to two previous murders. This senseless act upon a fellow officer and much loved member of society makes the hunt for this particular brand of killer personal for all involved.

But it just doesn't add up as Dub receives the first of many phone calls from the killer. He is perfectly calm, intelligent and not easily goaded, which is at odds with the viciousness found onsite. As Dub continues to push, he never gives thought to the danger he may be helping to set in motion.

If you are already a fan or are at all aware of Lyle's work, then you've probably figured out this man doesn't seem to know how to fail. "Stress Fracture" gives you a ringside seat for the action, making you feel like an active participant in the drama.

In construction, a stress fracture is a compromised structural integrity that could cause collapse. Medically, a hairline fracture is caused by repetitive stress or strenuous loads. For psychiatrists, it's sort of a break in transmission of neurotransmitters that makes patients lose touch with reality. In this psychological thriller, D.P. Lyle knows how to put readers into the minds and motivations of predators and their prey --- and law enforcement officers who untangle webs they use to snare: "A criminal leaves behind not only physical clues...but also fragments of his personality. Not always easy to read, and probably not overly accurate, these clues often reflect motivation."

Years ago, another Octomom must have given birth to eight D.P. Lyles for the multi-talented California cardiologist to be so prolific and diverse. Author of such thrillers as 2002's DOUBLE BLIND, his nonfiction FORENSICS FOR DUMMIES was nominated for the 2005 Edgar Award and earned Mystery Readers International's Macavity Award. Dr. Lyle has contributed material for many TV shows, including "Law & Order," "Monk," "CSI: Miami" and "Women's Murder Club." STRESS FRACTURE introduces the Dub Walker series with psychological intrigue, told in a too-realistic-to-be-fiction thriller.

Dub Walker cut his forensics teeth as a Marines MP and consultant in Quantico and with the FBI's Behavioral Assessment Unit --- sort of an "NCIS" agent wannabe. Along with detective friend Tommy Tortelli (T-Tommy to friends and readers), Dub investigates horrific murders perpetrated by a psychotic serial killer in Huntsville, Alabama. Dub revisits the building where he consulted with police on particularly gruesome serial murders: "The room hadn't changed. Same faded yellow walls. Same metal desks and chairs. Same tired coffee maker sat on the same wobbly wooden table. The glass coffee pot was new, but there was always a new pot. Glass had a short life span around here."

Catching a doodlebug --- "Ugly caterpillar-like creatures with pincer mouths" --- with a blade of grass (those from the South don't need to ask) is used as an analogy to catch psychopath Brian Kurtz. Way beyond half-mad, Kurtz suffers from post-traumatic stress disorder after experiencing unimaginable situations in Iraq. Identified early on as the perp, this is not so much a whodunit as a whydunit. The "why" involves tormented Kurtz receiving psychiatric treatment from clueless physicians Melvin Wexlar and Bob Hublein at the posh North Alabama Neuropsychiatric Research Institute. From some mysterious source, money flows like the Tennessee River into the country-club-like Institute, adding intrigue about an experimental drug called RU-1193 being injected into Kurtz and other PTSD soldiers. These uncharacteristically greedy doctors care more for money than they do for the well-being of their patients.

Though a scene that tracks the money trail ties loose strings into a knot a little too neatly, Lyle writes the perfect prescription for a psychological thriller. Laws are written to keep the majority honest --- and challenge Dub to circumvent them. He breaks into the Institute to get Kurtz's medical records before the money-grubbing doctors can destroy them.

Because something doesn't sit right with him, Dub is as uneasy "as a long-tail cat in a room full of rockers." After all, his name is Dub, not Dupe. An easy solution to crimes raises red flags. Eye-cringing descriptions of heinous murders that likely can't be committed by the calm person who phones Dub, to give details about the homicides, wake him before the roosters. Dub uses ex-wife and on-again girlfriend/reporter Claire to bait the mysterious murderer with provocative innuendo during TV interviews as a figurative blade of grass, to catch the diabolical doodlebug. He hears a recording of a murder-in-progress: "The killer's voice was high pitched, angry. His demon had taken over. Mr. Hyde was out." The calm voice claiming to be the murderer seemingly has a Jekyll/Hyde thing going on, caused by a controlled substance not controlled nearly enough.

With rock-solid writing, Lyle turns a phrase: "The morning sun silvered the [power] lines that swagged above the tree line. They looked like new guitar strings, waiting to be tightened to the proper key." Multiple homicides --- and solved crimes --- are compressed into one week, with brief flashbacks, to bring readers up to speed in this bullet-train-paced thriller.

After retiring from homicide investigations, Dub Walker became a forensic consultant to the FBI's Behavioral Assessment Unit and a noted author on the topic.However, he returns to field work when his close friend Sheriff Mike Savage is brutally murdered in Alabama.Apparently this psychopath has viciously killed two other people.

The annihilator kills with rage and Dub knows he will not stop.He even knows who the predator is.Iraqi veteran Brian Kurtz came home with a severe case of PTSD; the Neuropsychiatric Research Institute gave him an experimental drug that for no apparent reason failed to calm him down, but instead turned him into a sadistic vicious beast.Dub must hunt down the animal, who has residents of Huntsville in a panic.

The key to this strong police procedural medical thriller is the killer whom ironically the audience meets at the beginning of the tale as Kurtz the monster receives empathy from readers who know he must die to end the blood rage.Dub is terrific as the hunter who feels the prey is stalking him.Expecting a High Noon showdown, fans will enjoy this entertaining cautionary tale as underneath the non-stop action, D.P. Lyle warns readers of the excessive use of drugs.

Fantastic review with great illustrations & step by step management.I'm a family practice resident interested in urgent care as well and this book is fairly conclusive for both of those topics in terms of "walk-through-the-door" complaints.
Fantastic do's & don'ts for each syndrome/injury with Pt. instructions and background info on each in case you've forgotten anything from med school.
The only thing missing, I would say, is a template procedural consent form and some web links related to each topic that may be useful.However, I'm really stretching for something wrong.
Great book for any physician dealing with minor procedures!

This book is a true gem! Buttaravoli has clearly presented his distillation of experience and research into the diagnosis and treatment of the most common minor emergencies encountered by primary care providers. I work as a PA in an Urgent Care/Occupational Medicine setting and find that I frequently refer to this book. I read it cover to cover, and it was a joy to read, being clearly written and very wellillustrated in short, problem-oriented chapters. It makes a great quick reference, as the organization is simple and to the point. For each problem there is a brief introduction describing the typical patient presentation, followed by a "What To Do" section, as well as a "What Not To Do" section. Further elaboration on pathophysiology and the basic science is offered at the end of each chapter for those who wish to explore further. It is very well indexed and includes helpful appendices on such topics as performing digital and oral nerve blocks, procedural sedation and analgesia, and rabies prophylaxis. The Fracture Zone describes both of those trips, concentrating on the history and character of the region more than the recent war and its aftermath. Winchester has spent most of his career as a foreign correspondent, but his more recent occupations as historian and a writer for Condé Nast Traveler are in evidence here. Winchester's angle on the Balkans is unique and well written: those who have been bewildered at best and bored at worst by the Balkan conflict may find that The Fracture Zone captures their interest better than hundreds of news accounts of war atrocities. The book itself is wonderful and author does a wonderful job of explaining the years of conflict in the in this particular region of the world.

There are times particularly in the introduction when you want the author to shut up and get on with the story.Later on you see how much this one example(in the beginning) kind of weaves all of the points to come together.

Overall-Good book if you like history (and good journalism)

For a thousand years the Balkan peninsula has been the situs of vast cultural upheavals -- the sensibilities and hatreds of the Slavic peoples pushed and pulled and molded by the East and West, by Catholicism, Orthodoxy, Islam, Communism and Nazism. The result is a tragic legacy of manipulation, of hatred, of violence and of death.

Simon Winchester proves himself the perfect guide to this shattered and tragic realm.He is part History Professor, part Philosopher but always a masterful Storyteller. With this and his previous books (especially "The Professor and the Madman") he proves himself to be one of the most eloquent and gripping narrative writers working today.

Some other thoughts:

1. This book ought to be required reading for every American and a warning about the horrible consequences of hate. One of the greatest tragedies of the Balkan conflict is that, as Winchester puts it, "almost all the people who have been so horribly at odds with one another are all, in essential ethnic terms, the self-same people."And lest Europeans and Americans feel smug in their own situations, let them remember the calamities of Antietam, of The Somme, and of Auschwitz.

2. The smart reader will prepare himself or herself with two things before reading.First, a good map.The history of the Balkans is so utterly complex on many different levels, not the least of which is geographically.Second, a good dictionary.Not since Henry Kissinger have I read an author with his quiver so full of (to me) new and interesting words: e.g. from only the first fifty pages I noted, emollient, osmotically, ordure, gyre, excrescences, orotund, refulgent, meretricious, harquebuses, embrasures, barbicans and ravelins.

3. The editing of this book is horrendously sloppy -- it is an embarrassment to Harper Collins.I have never seen so many typographical errors, missing phrases and other stupid mistakes.

4.This is not Winchester's best work.The early chapters are among his best work, but the narrative loses significant punch beginning in chapter six, which deals with the authors time in Montenegro.

It's a pleasure to read reminiscences of masterful writers such as William F. Buckley (his sailing books have won him a well-deserved place of honor in sports writing) and Simon Winchester... and as he has seen Yugoslavia in her salad days, prosperous and peaceful, then returned to catalogue the horrors that followed her dissolution, Winchester is the perfect guide through that terribly unhappy place.

Winchester takes us with him on journeys along the Dalmatian coast (through land that is now divided unequally between Croatia, the Bosnian Serb Republic, and the patchwork government of Bosnia-Hercegovina), through the vainglorious, charming and not yet war-torn land of Montenegro, across the ghastly fields of Kosovo just as the Serbs withdrew before NATO forces, across Macedonia, then through parts of Eastern Europe that have been spared the advantages of Serbian leadership and thus remain peaceful and happy.

All through his travels, Winchester shows us the people who live in these lands, shares with us his bemusement at the human capacity to live around disaster and his shock and sorrow at our capacity to abandon our humanity when we go a-warring, to save our worst outrages for our close neighbors, and forget that Europe has been Christian for nearly 1800 years or, for that matter, what the word "Christian" means.

Winchester's command of the history of the area is intimidating and overpowering... he shows us entire worlds that we never suspected even existed... capital cities nestled in remote mountain summits and ruled by dynasties of hereditary bishops, places where there are two Orthodox churches vying for a shrinking pool of believers, customs sublime and gross.

I usually hesitate to award a perfect score to a book, no matter how well-written - but Simon Winchester has defeated me. His is a complete book, and for me to criticize it would be sheer effrontery. Buy it, read it, and be delighted.

A very probing look into the Balkans, Winchester's book raises simple yet profound questions about the hatred between so many peoples in the Balkans.The author did a very good job of putting the smoldering anger and resentment in historical perspective taking us not just back to the Austria-Hungary and Ottoman empires, but even further back to the Byzantine empire!It is this historical perspective that makes the questions so well thought out.In the end, Winchester has no answers, and I found myself disappointed that I could not have simple answers to such difficult questions.I walked away from the book with a better understanding, but every bit as puzzled, as when I entered the book, so it has provoked me to look deeper into the subject.It has put me on a path to learn more about the region. ... Though not a giant one, Lawn's piercing thoughts into the essence of brittle fracture can be clearly read and transferred (especiaaly for those who have studied and indulged into fracture like me). From the beginning, Griffith's spirits are extracted from his classic papers and put as a corner stone through out the book. Unlike many other authors, the author apparently appreciates and must have digged deeply into Griffith's paper. The remaining chapters illustrate it in an enlightening way, i.e., the subjects are treated as concepts-faced, not a pile-ups of entangling "dislocations" (facts). At least for me, some concepts had firmly taken their roots in my mind after reading. Not the least, the references given guides one fracturing through the literature and aims at providing some classic papers in the field for further study. They really take effect! I am at least not missed in library.

I recommend this book for residents,general orthopedist and even pediatric orthopedists, it is clear, easy and with a very practical POV about this vast field. It would not be stretching the truth too far to say I woke up one morning with a stress fracture of the femoral neck; a particularly nasty one.I am recovering nicely, thanks, but have yet to find anyone who can tell me how the heck I did it.So much for the nice 'warning' one is supposed to notice over a period of weeks.That didn't happen in my experience.Consequently, I am left trying to answer the question, How in the world did I get so far over the line and not know it?And, How do I keep from repeating this performance?This book is providing answers.It's technical.It will give the average non-scientist more information than they want to know; the section on research methodology made my eyes glaze.But, it is helping me construct an explanation of how I got here, and more importantly, how I can avoid a next time.If you already know a whole lot about bone remodeling, this book isn't likely to help you much.However, if like me, you didn't understand that bone undergoes microtears in response to loading, and has a pattern of repair that parallels muscle tissue, the contents can be revealing.Five stars minus one because I have no plans to publish research in this realm, but four stars for the questions I didn't know enough to ask, and the answers it offers.I recommend it. ... The 369 cases in this atlas are arranged anatomically. In Chapter 1, we illustrate fractures and dislocations involving the hand, beginning with injuries to the distal phalanges of the fingers, continuing with injuries of the distal interphalangeal, proximal interphalangeal, and metacarpophalangeal joints, and ending with injuries of the carpometacarpal joints. In Chapter 2, we consider fractures and dislocations of the wrist, including fractures and dislocations of the individual carpal bones and combinations of carpal bones, perilunate injuries, radiocarpal injuries, distal radius injuries, and isolated distal radioulnar joint injuries. Chapter 3 begins with forearm injuries, including Galeazzi, Monteggia, and Essex-Lopresti fracture-dislocations, continues with elbow fractures and dislocations, and ends with humeral shaft fractures. In Chapter 4, we illustrate fractures and dislocations about the shoulder and shoulder girdle, including the proximal humerus, scapula, clavicle, and thoracic cage. Most of images are drawn from cases seen at Harborview Medical Center (Seattle, WA) the Level 1 Trauma Center that serves the states of Washington, Wyoming, Alaska, Montana, and Idaho. Additional cases were drawn from the teaching collections of the University of Washington and from the teaching collections of the authors. A few images have been previously published and are used with permission.
