Marked changes of thehypothalamo-pituitary axis have been documented inthe acute phase of TBI with as many as 80% ofpatients showing evidence of gonadotropindeficiency, 18% of growth hormone deficiency, 16% ofcorticotrophin deficiency and 40% of patientsdemonstrating vasopressin abnormalities.Longitudinal prospective studies have shown thatsome of the early abnormalities are transient, whilenew endocrine dysfunction becomes apparent in thepost acute phase. There remains a high frequency ofhypothalamic-pituitary hormone deficiencies amonglong term survivors of TBI with approximately 28%patients showing one or more pituitary hormonedeficiencies. This is a higher frequency thanpreviously thought and suggests that most cases ofpost traumatic hypopituitarism (PTHP) remainundiagnosed and untreated.
