If you do back surgery, this book is a"must-read". It is brief but provides far more information than the multi-volumed spine text books. In simple and concise language, each chapter not only explains "how to", but also "why". No other book explains quite as well, the concepts of how various spinal diseases impact the various regions/segments and therefore, the appropriate method of decompression. Infact, when done correctly asdescribed, the "decompression" in effect becomes a "reconstruction". The techniques described are simple, elegant and respectful of maintaining normal anatomy. Despite its relative brevity, the book does not short change on appropriate figures and diagrams to reinforce the text.
Written by unquestioned masters (author list includes the likes of Yasargil, McCulloch and Young), every page is a gem. For its conciseness, clarity, simplicity and substance, this book should become a part of a surgeons library as early as second year of training. The information provided is timeless and very useful to peruse throughout one's career. If anyone is aspiring to become a "surgeon's surgeon", here is a place to start. In a way the reader is standing on the shoulders of those giants, who already are.

Abstract: Managing acute lumbar disk herniations in active patients can be challenging for physicians, especially when controversies surrounding treatment options make choosing the right care more difficult. The prognosis for lumbar disk herniations is generally favorable, and most patients will improve with conservative measures, such as early activity and avoiding bed rest. Minimally invasive techniques, such as percutaneous diskectomy, laser diskectomy, and epidural corticosteroid injection, may help control sciatic pain. Athletes who sustain late-season disk injuries should receive conservative treatment before considering surgery. Those who have early-season injuries may be able to return to competition sooner by having a microdiskectomy before a full course of conservative therapy is completed, but data supporting this approach are currently lacking.

The incidence of low-back pain in college athletes ranges from 6.9% to 64.8%,1-6 and, similar to findings in the general population, most back injuries do not involve herniated intervertebral disks.3,7,8 One 10-year study3 of college athletes reported that only 7.2% of back pain complaints were related to a disk injury.3 Although disk herniation is not a frequent cause of sports-related back pain, its treatment is often difficult because of the controversies surrounding nonoperative management versus surgery, particularly if the injury occurs during the competitive season. For the first time in the (para)medical science-history of Low Back Pain, a book is written that is based on scientific evidence. The list of References includes no less then 47 articles, scientific papers and books.And more important even than that: McKenzie opens up a total new chapter onthe causes of the so-called (until now!) 'non-specific' LBP, which to thereader is not at all 'non-specific' anymore once you've read (and maybere-read) this book! "The Lumbar Spine" taught me things I neverlearned in college (PT and MT) and that are still not taught in general,unfortunately: specific disc behaviour: the mechanical principles thatdamage the disc and make it to heal (or to not heal!), and how theseprinciples cause LBP, even long before discs are herniated. One superbevidence-based book, especially since it describes a low-cost therapy! And:this book explains why manipulative therapy (MT) often does not give theresults we had hoped for. All physiotherapists, chiropracters, osteopaths,general practitioners and neuro- and orthopedic surgeons: this book willalmost certain bethe basis for the future treatment of 'non-specific'LBP. If there's one book you want to have read, this is it!
