This book by Barent Walsh is an excellent resource for understanding the many facets of Self Injury (SI), and for being able to move beyond SI as a means of coping.It contains case studies--real life situations with a variety/diversity of ages, genders and backgrounds.Walsh writes for the clinician, but this book is a gem for those who struggle with SI and for those who want to help/treat/support those who struggle with SI.It is a mature and compassionate book.While the price may present as prohibitive (a bit pricey), it's worth every penny.The book is up-to-date, practical and proves insightful into seemingly confusing behaviors.I highly recommend this book, along with Robin Collins, Dusty Miller, et al...

In the first part of this accessible and comprehensive work, Barent does a sterling job of distinguishing the difference between self-injury and body alterations (tattoos, piercing, brands, etc.) and self-injury and suicide.Echoing my own view, he also presents a strong case for steering clear of suicidal language when referring to self-injury, for example, terms such as "attempted suicide," "suicidal gesture," and "parasuicide."

The main thrust of the book is on assessment and treatment of self-injury, for which Barent proposes an inspiring bio-cognitive-behavioural approach.Topics addressed in Part Two include contingency management, replacement skills training, cognitive treatment, body image work, exposure treatment and resolution of trauma, family treatment, psychopharmacological treatment, and managing the reactions of therapists and other caregivers to self-injury.

Later chapters deal with specific issues such as self-injury contagion, managing self-injury in school settings, and treating major self-injury.

Overall, Barent has done a masterful job of blending facts, theories, case examples, and practical exercises to create an engaging and thorough guide to understanding, assessment, and treatment of self-injury.This excellent resource deserves a well-earned place on the bookshelves of mental health practitioners, researchers, and anyone else interested in this challenging and complex behaviour.

5-0 out of 5 stars An Excellent Text For Therapists and Those Seeking Help
There are many self-help paperbacks already published on self-injury that are readable, helpful and under $15. "Treating Self-Injury" is written like a textbook but it is the most organized and comprehensive of all that I have seen. I would recommend it especially for school personnel or therapists who have little experience with "cutters" or those who cope through other forms of self-injury. The text would aid senior clinicians in intergrating various models into a coherent approach for treatment. The author wrote one of the first books that brought self-injury out of the medical closet, "Self-Mutilation: Theory, Research and Treatment" (1988).

Too many times, self-injury is confused with suicidal ideation and this book helps to dispell a lot of old myths. Mr. Walsh has done considerable research in synthesizing what has already has been written and wrote a "bill of rights for people who self-harm" against those myths. This is not a book that will be read in one sitting but one that the reader can consult and review. A more readable approach, though more narrow and less comprehensive, is "Cutting" (1999) by Steve Levonkron (as a matter of full disclosure, I once did a conference with Mr. Levenkron years ago). "A Bright Red Scream" (1998) by Marlee Strong remains the classic in this field. ... Read more

This book would be a great starting point if you self-harm but don't really know much about the behaviour, how to get treatment, or what you can do to help yourself. If you already have a lot of information and are looking for a comprehensive self-help guide, it's probably not the best option.

The book is divided into three parts: information on self-harm (which is the longest section), treatments and how to access them, and self-help strategies. The authors' reasoning is that being well informed is an important part of the recovery process. I agree with this, but there are plenty of good information sources out there already, and from the synopsis I had been expecting more of a self-help workbook.

What I liked: The authors are very understanding and nonjudgemental, and the book is easy to understand. The treatment section was fascinating, and included details of some very new but promising-looking therapies. The book is an excellent overview of self-harm and the things that can help, making it ideal for someone who is "new" to self-harm.

What I didn't like: The self-help techniques are not described in as much detail as they could be. Given the title of the book, I felt more space could have been devoted to DBT and other skills. The very simple language feels patronising at times. And I'm not sure how helpful the treatment section will be to the average reader without any specialist self-harm programmes in their area.

Overall, I would recommend this if it's the first book on self-harm you're buying, and you need general information, or would be too overwhelmed by a full-on workbook. If you're looking for more comprehensive self-help, I'd recommend the Dialectical Behavior Therapy Workbook, although it isn't specifically about self-harm.

Finally, a note on the previous review. Freedom From Self-Harm is by no means an exclusively DBT book. Although the self-help techniques are derived largely from DBT, the treatment section covers many approaches including interpersonal therapy and mentalisation-based therapy, and explains each in an unbiased way. The authors stress the importance of finding an approach that works for you, and that this will not be the same for everyone. They discuss understanding the reasons why you self-harm, and coming to terms with issues from your past. So if you're not sure DBT is the best approach for you, don't let that put you off this book.

The book "Freedom from Selfharm: Overcoming Self-Injury with Skills from DBT and Other Treatments" becomes a sham when practicing DBT. I have wasted so much time in DBT programs. Dialectical Behavior Therapy relies on the here and now, trying to alleviate urges, ideations, in the moment. DBT is a band-aid for the incapacitating emotions and urges some people have. DBT is not designed to get at the root cause of a participants condition; it is designed to ignore the past, where a lot of dysfunction occurred. I say again, watch out for this "therapeutic" program in the future.

As the very first Christian resource on self-injury, this booklet accomplished a lot: it brought the issue to light for Christians.As the first resource that could've set the standard and as a potential tool for healing, this booklet falls drastically short.The author writes from a biblical-counseling perspective (the perspective that thinks all the answers to all life's problems can be directly found within the pages of the Bible) and ignores decades, if not centuries, of psychological research.

While the booklet accurately assesses the reasons WHY people give for wanting to self-injure, it reduces their reasons to one: it's all about God.On some level, that may be true, but many things point directly to things in our past.Once we recognize what causes us to act in certain ways, only then can we begin utilizing God's help to overcome our faulty behaviors.And this help can come in many forms: Bible reading, medication, prayer, counseling, talking with family and friends, etc.Prayer and Bible reading are not, as the author suggests, the ONLY answer; they are a PART of the answer.

For a better introductory booklet, read "Cutting: Self-Injury and Emotional Pain" (InterVarsity Press).For full-length Christian treatments of the issue, read Jerusha Clark's "Inside a Cutter's Mind," Jan Kern's "Scars That Wound, Scars That Heal," or Marv Penner's upcoming "Hope and Healing for Kids Who Cut."

This is a very brief book on Self Injury, the title a little misleading regarding pain feeling good.That part misses the point.The book does say there are many reasons why someone would choose SI as a coping mechanism.The book promotes compassion and alternative messages for Self-Injury (SI)such as :encouraging self-love, Christ-like love and utilyzing scripture for direct communication between self and God, rather than self-inflicted violence.This book is a good intro book for persons of faith who struggle with this issue; and this book can be a good book for other persons of faith who may be in a supportive role, helping friends or loved ones who struggle with SI.More needs to be written from a Christian, compassionate perspective that is more in depth.Robin Connors books and Walsh's new book on Treating Self Injury are much more thorough and better, tho without the religious perspective.

I found this book to be a very helpful overview of the topic.Obviously, any major problem that needs counseling is also going to need a thorough amount of research also, and this provides a very basic manual to the causes and some helps for self-injurers.

As an experienced clinician often working with high risk teens, I've long-found self-injuring -- cutting, burning and more -- both disturbing & frustrating.Disturbing because this self-violence against their own bodies IS deeply disturbing, not only for parents, but also for those of us who care for and care about these young people.Frustrating because these teens often present serious, serious challenges in building relationships, ESPECIALLY on their self-injuring.And clearly teen self-injuring, now, doesn't mean mostly what it did back in the 1980's, when it was more limited to proto-borderlines and other victims of fairly extreme abuse.

That abuse/borderline population is still there, but there's a whole lot more.This book well-shows this in its chapters on epidemiology and on teen self-injuring, how widespread self-injuring now is in the teen high school community -- 13-23%!Up to nearly 1 in 4 high school students!Recent surveys suggest self-injuring includes not only teens -- average age to start self-injuring being 12-14 years old -- but self-injuring carries on through young adulthood, including college students.Young adults, 18 and 25, have rates even higher.One college reported, in a survey, rates similar to hospitalized adolescents, around 40%.Nor is self-injuring only female -- some recent studies suggest rates are becoming more the same between boys & girls.Given that many if not most teens hide their self-injuring, these rates could easily be higher.Truly shocking.

Worse, repetitive self-injury can be a precursor to suicide attempts and, especially with males, even completed suicide.One study found that 44% of a sample of teens who completed suicide had previously repetitively self-harmed.(Though please note, most self-harmers are, indeed, non-suicidal.)As this book makes clear, though, while NSSI is without intent to commit suicide, it should be taken seriously.Not only is there evidence that, in some cases, NSSI can develop an addictive character and become longer-term repetitive as well as progressively more severe, there's true concern that, if NSSI is not taken seriously and the purpose behind it recognized and handled, it could lead to suicide.

This is the first academic comprehensive "Review of the Field".Its 15 chapters thoughtfully cover most key areas, including Defining the Field, Epidemiology, Models for Understanding Self-Injury, Biological/Neurological Understandings of Self-Injury, Self-Injury with Developmental Disabilities, Influences of Peers & the Media/Internet, Assessing, Cognitive Therapy, Behavioral Therapy, DBT, Residential Treatment, Psychopharmacology.And it has a special chapter on treating teens.I suspect that all the best academic researchers are here.Nock, also a suicidologist, is a tenured professor at Harvard, and his thinking in particular is careful and clear, making all his chapters well worth reading.If you really want to know what we know about the academic research field as of 2009, this is THE source.It's not always easily readable, but, hey, keep a dictionary handy.It never hurts to learn a few new words, or to be reminded of some from a perhaps-distant graduate student past.

This field, which has so very long lacked research, has clearly had, in the past few years, much more money poured into research.Nock with his colleagues are obviously pushing for including non-suicidal self-injury in DSM-V (or beyond).That would likely open up even more research money and attract more scholars.

If you treat teens who are self-injuring (and their families), definitely buy this book.You'll find information & understandings which will help you and your clients.

So why am I not giving this book five stars?

Sadly & predictably, academia focuses more on theory & research than on practice, and worse it focuses more on research with adults.This book, as do many books in the field, falls into the cognitive-behavioral treatment dogma.Not that they say this directly -- that would be much better if they did.But as part of the psychology wars of Cog-Behavioral Treatments Only vs everything & everyone else, the authors simply don't mention anything else.There are few family treatments given, mainly DBT, and even that is cautiously presented, stating in the chapter's conclusion, "researchers may find that in a comprehensive treatment DBT is not necessary to treat NSSI in adolescents who present with single disorders or for whom no BPD [Borderline Personality Disorder] features exist."

Sadly & predictably, we clinicians -- those of us who actually treat NSSI -- are given no options that don't meet a single style of research standards.And while books can stop at that, what are we clincians supposed to do? Because our clients, and their situationss & needs won't let us stop there.Thoughtful treatment programs do exist, in particular I mention Matthew Selekman's recent (also 2009) The Adolescent & Young Adult Self-Harming Treatment Manual: A Collaborative Strengths-Based Brief Therapy Approach.His manual is research-based, but a different type of research.He has worked and done research, what might be called clinical research, in this field for many years.He is well-published, and by reputable publishers.So why isn't he even mentioned in "Understanding NSSI"?Even if the authors didn't wanted to present needed options in the face of gaps, they might at least state, honestly & openly, their disagreements or concerns.I find their silence shabby.

Also not mentioned is the meta-analysis of psychotherapy research studies -- see especially the 2nd (2010) edition of The Heart and Soul of Change: Delivering What Works in Therapy -- which clearly show that far more effective than "specific treatments for specific disorders" in psychotherapy is building a treatment alliance -- a combination of relationship, mutual agreement on goals and objectives and mutual agreement on treatment approaches -- especially one that concentrates on client strengths & resources, something particularly strong in Selekman's work.

This is certainly my experience with teens who self-injure -- the hardest & most vital work is building a relationship and getting agreement on what we're trying to accomplish and how we're going to do it.And to pull this off, I find I often need to incorporate every strength & resource my client has.Yet these very issues -- relationship and treatment alliance, client strengths and resources -- what in my experience are the most important and central needs & skills for treatment -- all these are given very short shrift in this book.They're not even listed in the comprehensive index.

In some ways, a more balanced and clinically-friendly overview of this field, one written together by both clinicians and researchers, including a few researchers who also write in "Understanding NSSI", is Mary K. Nixon & Nancy Heath (ed., also 2009), Self-Injury in Youth: The Essential Guide to Assessment and Intervention.

But I really & truly would buy & read "Understanding NSSI".It's a very thoughtful and careful book, with much information that I suspect you'll be glad to have -- I know I certainly was.Anyone who works with teen self-injury knows that we need all the good help we can get.And I would also buy Selekman's book and Nixon & Heath's book.With those three together (and it wouldn't hurt add in the 2010 "Heart & Soul of Change"), I found I was so much better prepared to help these challenging and needful clients & their families heal.

This book was amazing. I have had a personal struggle with the topics covered in this book. I was really moved and touched in a healing way while reading this book. For anyone that has struggled with self-mutilation I would say this is a great read. I am recommending this book to my friends that have had similar struggles, and will continue to recommend it to anyone wanting a healing touch.

This book is great as it covers the issue of self-harm from all angles. From the perspective of the self harmer, from an external viewpoint and even gives pointers to those trying to help the self injurer. It also provides direction for holistic healing as it demonstrates God's role in the healing. Struggling with self harm myself I really appreciated that this book had raw honesty. It wasn't this fairy tale book of recovery. It was the hard slog of years of work and effort with a hope-filled ending.

This work provides a well-rounded culmination of explanation, conceptualization, supportive research, and applicable tips for the frontline responder.

D'Onofrio helps us to understand the inner world of the traumatized child, who can later become the self-injuring adolescent.He articulately puts the reader in touch with the pain and complexity of sufferers and the people who surround them. Readers are provided with an opportunity to endure a mere taste of the numbness or "deadness" that often drives self-injurers to inflict pain on themselves.

This book is my first book on SI and I found it a really informative and helpful. While there are some discussions that may be graffic to some but maybe not all. Over all I found the book insightful and while the stories in the book were different then mine the book was still worthwhile.

In this book, Jane Wegscheider Hyman interviewed fifteen women who self-injure themselves to learn more about their battle with self-injury and explain how and why they continually and intentionally injure themselves. This is a fantastic and informing piece of work. Hyman has obviously done a great deal of work over a period of time. The women talk about their pasts, reasons for their self-injury, and troubles associated with it and their hopes of recovery. Hyman is not only very educated in this field but she also is very compassionate about the women's stories and situations. This is an excellent source of information, and along with, A Bright Red Scream: Self-Mutilation and the Language of Pain by Marilee Strong, they are the best books on the subject matter. I greatly suggest this book for those who self-injure, those who know someone who does and professionals that care for them.

As someone who has self injured for over 25 years (since age 5), this was THE book that helped me to start coming out of my denial and being able to seek help.While it can be triggering, it is outweighed by the number of "ahhhh haaa's" one get's from reading it.It was very enlightening and helped me to reduce the shame I'd experienced, enough that I could finally seek help.One thing ESPECIALLY unique about this book is that it does not limit itself to cutting and burning - it deals with all forms of self injury - something that FEW books on the topic of self injury do.If you are looking to buy just one book on the topic, let this be it... ... CAPTIV is a very accessible book about S. Marie's healing journey through child abuse, its painful and challenging consequences, and the light she creates at the end of the tunnel. The author's extraordinary gift is to say so much about her experience, with tremendous insight, in so few words.

This book should be required reading, not only for every psychologist, therapist and university psychology student, but for every parent. For after reading this book, few parents could possibly perpetrate on their innocent child, the devastating damage that results from physical, emotional and sexual abuse.

Buy this book. You will see the world in a new way, and be better for it.
