Achondroplasia is an inherited disorder that causes short stature.
What is going on in the body? When a person has achondroplasia, abnormal cartilage at the growth plates of the bones cause short stature. Men with achondroplasia are usually less than 52-inches tall. Women are somewhat shorter. What are the signs and symptoms of the disease? The features of achondroplasia are:
  • dwarfism a large head with a prominent forehead a low bridge of the nose disproportionately short arms and legs curvature of the spine, known as kyphosis bowlegs
What are the causes and risks of the disease? Achondroplasia is caused by a defective gene. It is one of the most common forms of dwarfism. This disease affects both sexes and people from all racial and national groups. What can be done to prevent the disease?

Our lengthening for stature program for dwarfism is designed to not only increase height but also correct deformities of the ankle, knee, hip, and elbow and to decrease lumbar hyperlordosis (swayback). The latter reduces the risk of spinal stenosis in adulthood. There are two strategies of treatment, depending on the age of the patient at the time of presentation. Juvenile (young children) strategy First lengthening between the ages of 6 and 10 years
  • lengthen both femora (upper legs) and both tibiae (lower legs)
    a total of 10 cm per leg (6 cm in the femur and 4 cm in the tibia) correct genu varum (bowleg) or genu valgum (knock-knee) deformity average external fixation treatment time = 5 months

Normal bone growth depends on the production of cartilage (a fibrous connective tissue). Over time, calcium is deposited within the cartilage, causing it to harden and become bone. In achondroplasia, abnormalities of this process prevent the bones (especially those in the limbs) from growing as long as they normally should, at the same time allowing the bones to become abnormally thickened. The bones in the trunk of the body and the skull are mostly not affected, although the opening from the skull through which the spinal cord passes (foramen magnum) is often narrower than normal, and the opening (spinal canal) through which the spinal cord runs in the back bones (vertebrae) becomes increasingly and abnormally small down the length of the spine. Causes and symptoms Achondroplasia is caused by a genetic defect. It is a dominant trait, meaning that anybody with the genetic defect will display all the symptoms of the disorder. A parent with the disorder has a 50% chance of passing it on to the offspring. Although achondroplasia can be passed on to subsequent offspring, the majority of cases occur due to a new mutation (change) in a gene. Interestingly enough, the defect seen in achondroplasia is one of only a few defects known to increase in frequency with increasing age of the father (many genetic defects are linked to increased age of the mother).

Q: What is achondroplasia ?
A: Achondroplasia is derived from Greek and it means "without cartilage formation". Though patients with achondroplasia have cartilage, only a small amount of cartilage in the growth plates of the long bones turns into bone and short stature. It is the most common type of disproportionate short stature. Q: What are the clinical features of achondroplasia?
A:
The patients have a disproportionate short stature- with a normal trunk, short arms and short legs. There is proximal shortening of the limbs (rhizomelic dwarfism). The average adult height is 4 feet. The head appears larger than the body, with a prominent forehead and flattened bridge of the nose. Teeth may be crowded with poor alignment. Spine is usually straight in the upper back with a marked lumbar lordosis. Abnormalities of the thoracolumbar vertebra may lead to kyphosis in infancy, which disappears once the child starts walking, but may cause spinal cord compression is adolescents. There may be bowing of the legs and feet are generally short and flat. Hands are short with stubby fingers and there may be a trident hand (separation between the middle and ring fingers).

Polymerase Chain Reaction (PCR)/Allele Specific Digestion
Samples are amplified for the FGFR3 gene region (4p16.3) by polymerase chain reaction, then cut by restriction enzyme digestion and separated by size on a gel. The reliability of the test results exceeds 99%. Specificity may be compromised by PCR primer site mutations. PCR is performed pursuant to a license agreement with Roche Molecular Systems, Inc.
Clinical Significance:
Direct DNA testing for the common mutation is recommended to confirm a clinical diagnosis or for prenatal diagnosis, regardless of family history. Over 98% of cases of achondroplasia are caused by a ACH1138G A or ACH1138G C mutation in the fibroblast growth factor receptor 3 (FGFR3) gene. Over 98% of cases of achondroplasia are caused by one of these mutations. Please note that a few patients with achondroplasia will not have one of these mutations.
Due to the unique nature of genetic testing, patients should receive pre-test and post-test counseling. Informed consent is recommended. Consent forms are available from ARUP upon request.
