Asbestosis is a scarring of lung tissue caused by the inhalation of asbestos fibers. A portion of the fibers reach the alveoli (air sacs) where oxygen is transferred into the blood. Asbestos activates the lung's immune system and starts a reaction best described as an inflammatory process. Scavenger white blood cells (macrophages) try to break down the asbestos (phagocytosis) but are not successful, causing other cells (fibroblasts) to grow and form connective-tissue-based scars.

    1. What is Asbestosis?
Asbestosis is a scarring of lung tissue caused by the inhalation of asbestos fibers. When humans inhale asbestos fibers, a portion of those fibers reach the alveoli (air sacs) where oxygen is transferred into the blood. Asbestos is an inert mineral material which excites the lung's immune system. The presence of asbestos in the lung starts a reaction best described as an inflammatory process. It is this inflammatory process which causes the formation of scar tissue or fibrosis (collagen) in the lungs. The scar tissue slowly builds up and in some individuals will progress to where it interferes with lung function, including the reduction of lung volumes and impairment of the lung's ability to deliver oxygen to the blood and receive back carbon dioxide. The inflammatory process starts within hours or days after inhalation of asbestos and injury at the cellular level begins shortly thereafter.

SEATTLE POST-INTELLIGENCER SENIOR NATIONAL CORRESPONDENT Dr. George Wineburgh stood up for sick and dying New York miners whose suffering was being ignored by most of the medical community. For his efforts he was fired from his job and ostracized by doctors in two counties. Wineburgh practiced at New York City's largest hospital for 16 years before moving, with new wife and four Labrador retrievers, up to the peaceful upstate New York in March 1982. He was eager to start his new job as the radiologist at Ogdensburg's Barton Hepburn Hospital. "I thought this was going to be a piece of cake, you know, no stress, a handful of cases, a great change from the big city," he recalls. "Boy was I wrong." It didn't take long before the skills Wineburgh gained becoming a board-certified radiologist started generating questions within the pile of X-rays waiting on his desk each morning. The films were from the patients examined overnight, usually for assorted trauma, aches and pains and heart problems.

Inhaled asbestos particles can cause a scarring of the lung tissue called pulmonary asbestosis. Over a long period of time (the latency period is 10 to 30 or more years), microscopic asbestos fibers that are retained in the lung tissue can cause scarring to occur in the alveoli; the tiny balloon-like structures in the lungs where oxygen is distributed to the blood vessels. It takes many years for this scarring to become visible on an x-ray or CT-scan.
Pulmonary asbestosis is not treatable or curable. It is a progressive disease that gets worse over time and never goes away. Some people, however, can have pulmonary asbestosis that is visible on an x-ray and have no symptoms. Similarly, many people who develop pulmonary asbestosis never see or feel a progression of their disease.

A: Since your question has two distinct parts, I'll answer it in two sections. 1) Coal tars and the residues of tobacco products can cause health problems, primarily cancer, if they are inhaled or chronically exposed to mucous membranes in the mouth or respiratory tract. To my knowledge, topical products containing these substances are safe when occasionally used on intact skin. 2) Years ago, studies showed that Alzheimer's patients had increased amounts of aluminum in their brains. This finding predictably led to a national concern about aluminum products. Consumers stopped buying aluminum cookware and avoided the metal whenever possible. Recent studies, however, have shed doubt on this across-the-board indictment of aluminum products. Casual contact with the mineral is now judged to be safe and harmless. Nonetheless, some medical personnel, this doctor included, continue to advise prudence. For example, the long-term effects of aluminum salts in antiperspirants are unknown. But since the products are not necessary for good health, I recommend not using them. I admit that my position may be arbitrarily unfair, but every doc is entitled to an occasional bizarre thought.
