Pyrros, M.D. ...the CT scan of the heart has very severe limitations as a screening test. As a Board Certified Cardiovascular and Thoracic Surgeon, it is always exciting to begin a new year with the latest information on life enhancing technology that will benefit my patients and potentially save their lives. 2002 is especially remarkable with the emergence of the Cardiotron or 3D-MP, a mathematical and computer enhanced electrocardiogram (EKG) that can detect the early stages of heart disease. With the sensitivity and accuracy of over 90%, the Cardiotron can predict and thus give physicians the opportunity to intervene before a heart attack or irreversible damage to the heart occurs.
Physicians have always sought more advanced methods to tell us what is going on in the body. Computerized Tomography or the CT scan, is one method that is often utilized to tell us about the heart. Although it is useful in showing the deposits of calcium in the arteries and on the arterial wall, it has some drawbacks. The CT scan doesn't always tell us enough to accurately diagnose early stage heart disease. In addition, it can be expensive and necessitates exposure to radiation. In my opinion, it is not an adequate screening test for heart disease, although it continues to be advocated as such.

Lou Gehrig's Disease
Ian Maclean Smith, M.D.
Emeritus Professor
Department of Internal Medicine
University of Iowa Hospitals and Clinics

Creation Date: 1996
Last Revision Date: 1996
Peer Review Status: Internally Peer Reviewed Not only Lou Gehrig had this disease but also Vice President Henry Wallace, Dimitri Shostakovich and David Niven. Its name is amyotrophic (wasting of nerve cells controlling muscles) lateral (at the side of the spinal cord ) sclerosis (scarring) and is due to death of nerve cells that supply muscles usually for the limbs but sometimes in the mouth and throat. Its a progressive fatal nerve disease of unknown cause usually lasting less than 3 years. Twenty percent live over 5 years. An African American patient of mine, aged 66, first noted small local spontaneous muscle contractions easily visible through the skin of his forearm, a sign that the muscle was dying because its nerve supply had withered. Another man in his 50's (it's more common in men than women) had tiny tremors of his tongue. He was one of the 10% that have the disease run in their family. Often its weakness of one hand that is noted, then the other is affected. Later the toe catches on carpets. All four limbs are soon affected but there is no loss of sensation, mental ability, or bladder or bowel control. Weakness, incoordination, cramps and spasm develop with difficulty in using the hands for skilled movements, also with walking, speaking and swallowing. The final stages are often distressing for the patient and the family. With awareness and intellect unaffected, the victim is unable to speak, to swallow, or move purposefully. Then the patient has trouble breathing. Realistic choices have to be made about the use of "heroics" in an emergency and whether a respirator should be used.

Dimitri's Alive!
All My Children's Count Isn't Dead AFter All. But Don't Throw the Confetti Just Yet...
by Elaine G. Flores, SOD 9/14/99 "Investigative journalist Edmund Grey didn't win that Pultizer Prize for nothing. When his brother, Dimitri's sudden death came on the heels of a suprise marriage to a mysterious stranger, Edmund started sleuthing. This week on All My Children, Edmund learns the astonishing truth: the count is alive- but not well. Edmund's mounting suspicions about his new sister-in-law, Alexandra, drove him to enlist David to perform a secret autopsy. As the week opens, Edmund and David are shocked to find Dimitri's coffin empty. Edmund's first thought: 'What has that woman done?' cracks his portrayer, John Callahan. David, who has an ax to grind against Alex, speculates that she is a black widow, leading a rabid Edmund to confront her. The showdown comes at the Wildwind hunting lodge. Edmund accuses Alexandra of murdering his brother. 'She says, "I didn't murder your brother, he's not dead!"' previews Finola Hughes (Alexandra). 'Stunned as he is, Edmund cannot believe that all these lies have led to something wonderful, and the wonder is that his brother is alive,' muses Executive Producer Jean Dadario Burke.

Edmund : "In England?" Alexandra : "At a restaurant." Edmund : "The blind date with a mutual friend?" Alexandra : "Yes."
She has a flashback to their first date in London.
Alexandra (offering her plate to Dimitri): "Are you sure?" Dimitri : "I'm actually having more fun watching you eat it." Alexandra : "So, Sean tells me you're here on business." Dimitri : "Yeah, yeah. Sean's lying. He said that you might not meet me if you knew why I was here. He also said that he didn't know how you'd react to the prospect of a blind date, so I guess I'm taking my chances either way." Alexandra (stops, then continues with an angry tone): "So, which is it? Am I blind or just stupid?" Dimitri (quickly): "Well, I've known you through the rare lamb chop, peas and potatoes... oh, yes, some salmon to start. But I would say you're neither blind nor stupid. In fact, I think your eyes see just about everything."

Dimitri Yakov Marick, aka Count Andrassy, arrived in Pine Valley in the autumn of 1991. He was briefly involved with Natalie Hunter, but the relationship failed when his first wife, Angelique, reappeared. He also gained a brother, Edmund, previously thought to have been merely a boyhood acquaintance but in fact the son of Hugo Marick. A drunken youthful dalliance with a maid, Corvina Lang, had produced a son, Anton. Dimitri had at one time wanted to take over Erica Kane ’s company, Enchantment, but instead fell in love with Erica. They were married, but the marriage failed when, in 1992 Kendall Hart showed up and was revealed to be the product of Erica's rape at age 14 by Richard Fields, a friend of her father's. Erica and Dimitri divorced after she stabbed him, thinking he had raped Kendall. Later, after an affair with the evil Dr. Jonathan Kinder, Erica learned that she was pregnant with Dimitri's child. Still in love with him, she asked Dimitri to remarry her, unaware that he had slept with their sister-in-law, Maria Grey, after catching Erica in what appeared to be a compromising position with Jonathan. Erica miscarried Dimitri’s baby on what would have been their wedding day (round two). Maria later produced a son, which Skye Chandler arranged to appear to be Dimitri’s. Dimitri managed to maneuver Stuart Chandler into giving him WRCW, but

    HISTORY OF HUMAN DISEASE
    Determining Ancient Diseases
In studying the incidence of disease in ancient peoples, medical historians are confined to studying the available artifacts. Skeletal remains are often the only pieces of evidence available. Limited mummified remains have been found in peat bogs and ice packs and from later civilizations such as the Egyptians. Some information may also be gleaned from studying recent or current civilizations with lifestyles similar to those of ancient humans and from studying living primates in their natural habitat. Disease in Primitive Peoples
Primitive humans were hunters and gatherers. They lived in small groups and moved from one location to another following the availability of resources. Their population size was necessarily small and they made infrequent contact with other groups.
This particular lifestyle is consistent with a low incidence of non-communicable disease which is often associated with high fat, low fiber and sedentary ways as in many of our contemporary cultures. Therefore, it is likely that few individuals suffered from high blood pressure, heart disease, atherosclerosis and organ cancers, although there is evidence that such diseases did exist. The small population size and infrequent intergroup contact is consistent with a relatively low incidence of acute infectious disease. Recent studies have indicated that local population sizes must range from about 100,000 to 500,000 to begin to support such acute infectious diseases as measles. Therefore, it is most likely that the kinds of infectious disease suffered more often by primitive cultures was of the chronic and recurrent type such as herpesvirus diseases like cold sores and chicken pox, tuberculosis, syphilis, leprosy. Archeological evidence supports this notion.

WESTERN HISTORY OF THE CONCEPT, STUDY AND TREATMENT OF DISEASE
    Prehistoric Concept of Disease
The prehistoric view of causation of disease was supernatural. Disease was a punishment of the gods, sorcery performed by one's enemies, the spiritual presence of or possession by evil, the loss of the soul. As such, the treatment was equally superstitious. The duty of treatment eventually fell to the healer, medicine man, or shaman that is, the local clergy. This person was not only responsible for individual welfare, but also the welfare of the group as he invoked his powers for favorable weather, harvest and outcomes of battle.
Even without a sense of the true cause of disease, some healers did use a crude empiricism in attempting to derive treatments. 'Herbal' medicine led to the discovery of treatments still used or well understood today. For example, opium was discovered as a pain killer, honey with its antimicrobial properties was used to prevent wound infections, and a plant from India that was used as a sedative is now the source of a common drug used to treat high blood pressure.
However, one common treatment which was found in many primitive societies across the globe and very recently in the South Pacific was trephination. This procedure involved boring a hole in the patient's skull, supposedly to release the affecting demons. That the patient sometimes survived the treatment is evident in regrowth of the skull bone seen in some human remains.
