*For Medical Professionals only, registration required Acute disseminated encephalomyelitis, (ADEM), a monophasic multifocal immunomediated white matter disease usually related to a previous viral illness, most frequently measles, followed by rubella, chickenpox, Epstein–Barr, mumps and pertussis, or vaccination. Neurological symptoms vary in their mode of presentation, either with rapid onset within a few days or preceded by weeks of fever, headache and drowsiness and severity; with seizures, multiple focal neurological deficits followed by coma and death in some cases. The disease is self-limiting and resolves within one month; about 10–20% of patients may, however, suffer from permanent sequelae. Pathologically, the lesions appear similar to those of MS (see multiple sclerosis ) with confluent areas of demyelination, perivascular and perivenular inflammatory infiltrates. A rare variant of ADEM is

Definition: Chronic Fatigue Syndrome (CFS) is one of fifty names for an illness that has caused suffering since the 1850s. Some other names for this illness are Chronic Epstein-Barr Virus Disease, Chronic Fatigue, and Immune Dysfunction Syndrome (CFIDS), and Post-Viral Fatigue Syndrome (PVFS). The emphasis is put on 'fatigue'. Chronic means ongoing and therefore refers to a fatigue that is long-lasting. The term Chronic Fatigue Syndrome is used primarily in North America. Myalgic Encephalomyelitis (ME) is used in most other countries. It can be best understood when broken down into its individual parts. As you look at the term it becomes clear that the emphasis is on the central nervous system. My - muscle
algic - pain
encephalo - brain
myel - spinal cord
itis - inflammation Neither term is satisfactory. People with CFS have many other symptoms contributing to their disability besides fatigue. Seldom is an infection in the brain found in those who have ME as was once suspected. CFS/ME affects thousands of men, women and children. The majority of sufferers seem to be women between the ages of 30 and 50. Generally these people are high functioning, productive individuals. There are some differences in how children of different age groups are affected by the illness. This topic is dealt with later.

Case Reports: Postvaccinal Encephalomyelitis Following Hepatitis Vaccination
Burton A. Waisbren, Sr., M.D., F.A.C.P.
Only that shall happen
which has happened. Only that occurs
which has occurred. There is nothing new beneath the sun! INTRODUCTION The general acceptance of the concept of universal vaccination against hepatitis B, regardless of risk factors, makes it incumbent on physicians to be aware of the complications that have been reported to occur following the hepatitis B vaccination. These complications have included a wide variety of reactions, most of which fit into an autoimmune category (3-19, Table 1). It is in this frame of reference that the two cases of postvaccinal encephalomyelitis seen by the author are being reported. A theoretic framework that might explain the pathogenesis of this complication and the studies suggested by this framework are discussed. CASE REPORTS Case 1
The patient is a 43-year-old female nurse who was in good health until August 1988. She had received two injections of hepatitis B vaccine in June 1988. Four weeks after the first vaccination, her husband noted that she began to have difficulty in concentrating and to have frequent severe headaches. She was taking a postgraduate course in nursing and contrary to her scholastic efforts in the past, she had difficulty in understanding and placing in context the relatively simple concepts that were being presented. In spite of headaches and cognition difficulties, she continued to work as a nurse. In October 1988 she developed a rash in her axilla which was nondescript in nature and which subsided in a month. She remained chronically ill with headaches, arthralgia and cognition defects, but she continued to try to work.

European journal of neurology, 1999, 6, 63 - 69. One of the main differences between ME and most cases of CFS is the phenomenon of prolonged muscle fatigue after minimal exertion. However, the research on muscle function during and after exercise as been far from illuminating. Some studies (on patients with CFS) have found an abnormality during exertion but most have not. Moreover, none have detected an obvious problem during the recovery phase. On the basis of this research, psychiatrists claimed that the vast majority of fatigued patients don't have anything wrong with their muscles and that exercise when tired was therefore safe. However, it is now generally accepted that CFS covers a number of disorders, so what is safe for one fatigue syndrome may not be safe for another.

Journal of Neurological Sciences (Turkish) Table of Contents NOROL BIL D 18: 1 , 2001 http://www.med.ege.edu.tr/norolbil/2000/NBD12600.html Case Report FULMINANT ACUTE DISSEMINATED ENCEPHALOMYELITIS Nur YÜCEYAR, Ayse SAGDUYU, Aysin KISABAY, Hadiye SIRIN Ege University Medical Faculty Department of Neurology, Izmir, Turkey ABSTRACT Acute disseminated encephalomyelitis (ADEM) is a widespread CNS inflammation and demyelination with an immune mediated process and usually follows an infection and immunisation. ADEM is more frequently encountered in children than in adults. We had the opportunity to examine and follow-up a rare case of elderly patient with fulminant ADEM treated successfully with a regimen of intravenous immunglobulin (IVIG) followed by intravenous methylprednisolone (IVMP). Follow-up magnetic resonance imagines (MRI) showed striking resolution of abnormalities over 4 months. He had free of symptoms for 3 years. Distinctive clinical and radiological features of ADEM and multiple sclerosis (MS) were also discussed on the basis of our case and recent literature. Key-words: acute disseminated encephalomyelitis, intravenous immunglobulin, intravenous methylprednisolone, multiple sclerosis
