Peer Review Status: Externally Reviewed by the Department of Internal Medicine Virtual Hospital Editorial Board Pharmacological COPD treatment should be individualized to maximize benefit and minimize side effects of the medications. The American Thoracic Society recently suggested defining the severity of COPD based on spirometry and designing treatment based on disease severity (1). Regardless of severity, the therapeutic goal is bronchodilation, reducing inflammation, and facilitating expectoration. In patients with mild disease, this can be attained using intermittent inhaled beta agonist drugs. A trial of steroids may also be considered with monitoring for objective response as outlined below. Additional treatment decisions should be based on symptoms and response to treatment. A description of the drug classes available follows. Short and long-acting beta-agonist inhalers:
These metered dose inhaled beta-agonist bronchodilators are the mainstay of treatment for COPD. However, the most appropriate dosing and dosing intervals have not been extensively studied. Most patients use the short acting inhalers at two puffs four daily, either scheduled or as needed. New, long-acting formulations might be useful when administered at bedtime to patients who report nocturnal symptoms. It is important to remember that the response to bronchodilators on spirometry does not identify all patients who will benefit from these agents. Clinical response should guide the use of the medication. Patients must use the metered dose inhalers appropriately to benefit, and instructions on the proper use of aerosol inhalers is mandatory for maximal effect. Here we are in the middle of a major economic downturn, deflation. Now all we have to do to go from bad to worse, a depression, is to waste money on war to create a huge black hole in the budget.
Then when Japan lowered interest rates to 0%, even then still nothing worked. It can happen here.
Lowering interest feels good, but won’t work as well as building houses for every person here. Pres. Clinton was able to rebuild our economy from the terrible wreck it was in, to an actual budget surplus in 8 years. Bush was able to destroy the surplus, our Mental Economy and our budget in a mere 3 years.
Who we need to elect now is another “share the wealth,” Huey P. LONG! The rich will never listen to just an elected President. Why are we marching against this war? To build the next government! We need to have a mass movement with a variety of program issues that can save money from the wildly spiraling medical costs, build housing, create a 30 hour work week at 40 hours pay

S yndrome X is a cluster of metabolic risk factors for heart disease, separate from high total cholesterol. The disorder is very common in economically developed countries. One in every three to four Americans are at risk for developing Syndrome X a whopping 60 to 75 million in the USA.
Allison, like most people, thought that keeping LDL (bad) cholesterol low was the key to preventing heart attacks. Keeping total cholesterol low is very important, but half or more of all heart attacks occur because of Syndrome X, even with normal cholesterol levels. Allison is worried that she or her children have Syndrome X. It's an understandable concern since the disorder is genetically linked. I am Roosje Smid - I have Pulminary Hypertension
The girl you see on the picture is the 5 year old Roosje Smid, she has a unique congenital heart lung defect which results in a life threatening high blood pressure especially in the lung arteries. This defect is known as (Primary) Pulmonary Hypertension or PH / PPH . Today we know that Roos has Secondairy Pulmonary Hypertension. In Roos' case the defect is at least caused by the fact that the upper half of the arteries in the lungs that assimilate the oxygen from the air into the blood, have not been formed. Instead of these arteries extra (so called aberrant) arteries have been formed between the short and long blood circulation system, which is the cause for the high blood pressure in Roosjes lungs (around a factor of 4 above normal levels). Against this high pressure her heart has to work. It is beyond saying that eventually this little heart will lose this struggle. When this congenital defect was discovered at the age of 4 months, the lungs had already lost all their elasticity (compare this with an airballoon which had been filled with air for a long time) and the only hope was that her situation would remain stable - which it is up till today. The only thing which can save her today is a heart lung transplant, which is - of course - a very dangerous operation. However, the longer she will be able to fight this battle the further the medical sciences will develop and the better her future will be.
