Spierpijn Om een diagnose te kunnen stellen moet de arts de verschillende types spierpijn onderscheiden. Spierkramp bijvoorbeeld is een vrij hevige spierpijn. Deze kan spontaan optreden en dat gebeurt nog al eens bij oudere mensen en bij zwangere vrouwen. Ze kan voortkomen van een verhoogde prikkelbaarheid van de spieren, zoals door een gebrek aan magnesium. Een spierkramp kan ook optreden door een metabole spierziekte, een ziekte die te wijten is aan een stofwisselingsstoornis. Hierdoor kan de spier soms onvoldoende bouwstenen in energie omzetten om een bepaalde inspanning te doen.

    This side effect is thought to be caused by damage to the mitochondria in muscle cells. Symptoms: Symptoms are weakness and loss of muscle tissue in the arms, legs and buttocks. This type of toxicity can also affect the heart muscle, a problem called cardiomyopathy. Drugs responsible: This side effect is most often associated with zidovudine (AZT, Retrovir) , but has also been reported with stavudine (d4T, Zerit) Risk of experiencing this side effect: Diagnosis: A blood test for an enzyme called creatinine kinase (CK or CPK) is used to help diagnose myopathy. CK levels are often higher than normal in people with myopathy. Normal CK levels are generally 10-79 u./L for women and 17-148 u./L for men. Taking a small piece of muscle tissue (a muscle biopsy) and studying it under the microscope can show if the mitochondria in muscle cells are damaged. In the case of cardiomyopathy, a heart test called an echocardiogram can help look for problems with heart function. Studies are needed to work out if regular echocardiograms should be done in people taking anti-HIV drugs, particularly Retrovir or Zerit. Treatment: Because there is no known treatment for NRTI-related myopathy, the best approach may be to identify the drug that is causing it. If possible, an alternative anti-HIV drug can then be substituted. A study published in 1998 looked at muscle biopsies from people with Retrovir-related myopathy. The researchers found that mitochondrial damage in the muscle tissue improved rapidly after Retrovir was stopped.

    Inclusion Body Myositis
    Inclusion body myositis (IBM) is an inflammatory muscle disease characterized by slow and relentlessly progressive muscle weakness and atrophy of the muscles. There is no standard course of treatment for IBM. The disease is unresponsive to corticosteroids and other immunosuppressive drugs. Some evidence suggests that intravenous immunoglobulin may have a slight beneficial effect in a number of cases. Physical therapy may be helpful in maintaining mobility. Other therapy is symptomatic and supportive. IBM is generally resistant to all therapies, and its rate of progression also appears to be unaffected by the present treatments.     Muscling in on myositis
    This rare but unpleasant disease is explained by Ernest Choy, Barbara Mason and Nicolo Pipitone. Myositis is a relatively uncommon condition, and only about five new cases occur a year for every million people. Women are two to three times more likely to be affected than men. It can affect children as well as adults.
    Symptoms
    Muscles: The commonest symptom in patients with myositis is weakness in the upper arms, hips, and thighs. Because of the weakness in the upper arm, many patients find it to difficulty to comb and shampoo their hair, get dressed and reach for objects above their heads, while weakness in the thigh muscles makes climbing stairs and rising from a chair difficult. Often muscle weakness starts slowly, although in children with dermatomyositis, symptoms may develop more rapidly. Occasionally, muscles of the gullet (the canal through which food and drink pass from the mouth to the stomach) are also affected, causing difficulty in swallowing. When the muscles of the throat are involved, the voice can become hoarse. In severe cases, muscles of the chest and heart could also be affected and patients can become short of breath. Although less common than weakness, more than half of the patients also experience muscle pain.

    Definition
    Myositis ossificans is a reparative process in which bone forms in skeletal muscle . When bone forms in soft tissues other than skeletal muscle, the process is termed ectopic ossification. The two, for all intents and purposes, are the same.
    Clinical Presentation
    Most commonly, myositis ossificans (MO) occurs in the major muscle groups following a significant episode of trauma. Initially, there is an area of induration and ecchymosis which, in two to three weeks, is followed by the development of a firm mass . As the inflammatory signs diminish, the mass may be asymptomatic Myositis ossificans often follows major surgical procedures and is particularly noted in and about the hip after total hip replacement . Less frequently, there is no associated trauma, and the process is first noted as an asymptomatic mass. A rare syndrome in young children in which multiple lesions occur is called myositis ossificans progressiva . A second term is fibrous ossificans progressiva (FOP). These children present with extensive involvement of the major muscle groups that begin as large painless fibrous masses resembling juvenile fibromatosis. With time ossification takes place within the masses producing significant stiffness and disability. In addition to the ossifying masses there is abnormal shortening of the first metatarsal and metacarpal bones producing a
