Causes, Incidence, and Risk Factors
The causes of neuralgias are varied. Chemicals can cause nerve irritation. Inflammation, trauma (including surgery), compression by adjacent structures (tumors or inflamed tissues), and infections can all lead to neuralgias. In many cases, however, the cause is unknown or unidentifiable. Neuralgias are most common in elderly persons, but they can occur at any age.
Trigeminal neuralgia
is the most common form of neuralgia. It affects the main sensory nerve of the face, the trigeminal nerve ("trigeminal" literally means "three origins", referring to the division of the nerve into three branches). This condition involves sudden and short attacks of severe pain on one side of the face, along one of the areas supplied by the trigeminal nerve. The pain attacks may be severe enough to cause a facial grimace, which is classically referred to as a painful tic (tic douloureux).

Cali, Colombia. El diagnóstico y tratamiento de los dolores faciales representan un reto supremo para el clínico. Además de la neuralgia del trigémino, que es el más bien definido de los síndromes neurálgicos formales, existen varios otros dolores, algunos sujetos a controversia aún hoy día. El listado incluye: la neuralgia del glosofaríngeo, las neuritis similares a tic del V par asociadas a tumores u otros procesos patológicos, la neuralgia del geniculado, la cefalea asociada con la neuropatía diabética, las neuralgia herpética y post-herpética del V para y de los ganglios de las raíces dorsales cervicales, la neuralgia occipital, el sindrome de Raeder y el dolor facial atípico. Adicionalmente, algunos sindromes de sobreposición como el tic-cluster y combinados del V y el IX. La neuralgia del trigémino casi siempre se presenta después de los 40 años, a menos que el paciente tenga un esclerosis múltiple concomitante. El dolor es de una alta intensidad, paroxístico, y ocurre particularmente en asocio con las zonas gatillo, las cuales inician el ataque cuando son estimuladas, frecuentemente con un estímulo trivial. El paciente evita entonces masticar, hablar, cepillarse los dientes, afeitarse o exponerse a una corriente de aire frío. Esta técnica de evitación es una clave valiosa para el diagnóstico. Curiosamente, el paciente rara vez es despertado por los paroxismos de dolor, al contrario del que tiene neuralgia del glosofaríngeo. El dolor se acompaña de una mueca (tic doloroso), frecuentemente en la distribución del trigémino, segunda y tercera división, solas o en combinación. Casi siempre es unilateral, más frecuente del lado izquierdo.

Under this heading will be discussed atypical facial pain and other neuralgias not listed in the categories below. In the IHS classification, pain in and around the eye caused by conditions such as optic neuritis, herpes zoster, post-herpetic neuralgia in the Tolosa-Hunt syndrome are described. I have chosen to discuss these elsewhere as noted. First, the condition we discuss is atypical facial neuralgia.
Atypical facial neuralgia
None of the foregoing criteria is essential to the diagnosis. Pain arising from deep seated infections or from tumors of the face, head, or cranial cavity may have similar characteristics, so that thorough studies are required to eliminate these potential etiologies (O'Connell, 1978; Destee et al., 1980). Differential diagnosis of atypical neuralgia should also include temporal arteritis, temporomandibular joint neuralgia, and pain from dental sources (Symonds, 1956).
Raskin describes atypical facial pain as a continuous, unilateral, deep, aching pain, sometimes with a burning component. However, deep, dull continuous pain is the type commonly encountered in a variety of disease states. Some of these patients eventually turn out to have disorders such as nasopharyngeal carcinoma or squamous cell carcinoma of the facial skin that has migrated perineurally intracranially (Morris and Joffe, 1983). In these two latter instances, facial pain may antedate cranial nerve dysfunction by several months. Therefore, a better designation for this syndrome is facial pain of unknown cause; as knowledge is advanced, other causes of pain will undoubtedly be uncovered. A significant number of patients with atypical facial pain have had infections at previous tooth extraction sites (Roberts and Pearson, 1979; Ratner et al, 1979).

    Treatment: Treatment of neuralgias is aimed at reversing or controlling the cause of the nerve problem (if it is identified) as well as providing pain relief. Therefore, the treatment varies depending on the cause, location of the pain, severity of the pain and other factors. Even if the cause of the neuralgia is never identified, the condition may improve spontaneously or disappear with time.
    The cause (if known) should be treated. This may include surgical removal of tumors, or surgical separation of the nerve from blood vessels or other structures that compress it.
    Mild over-the-counter analgesics such as aspirin, acetaminophen, or ibuprofen may be helpful for mild pain. Narcotic analgesics such as codeine may be needed for a short time to control severe pain. These traditional pain killers, however, often have disappointing results. Other types of medications work in different parts of the nervous system and often provide better symptom control. For instance, antiseizure medications such as carbamazepine, gabapentin, lamotrigine or phenytoin may be helpful for pain associated with trigeminal neuralgia . The most common adverse effects of antiseizure drugs are drowsiness, tremor, and incordination.

    Medical literature describes the attacks of TN pain as transient paroxysms which means brief stabs or jolts of intense pain. The pain characteristically is either on or off with no residual pain. An individual attack can consist of many rapid jabs of pain. The entire attack often lasts a second or less. The intensity of an attack can stop people in their tracks. Uncontrollable TN can drive people to suicide. People may experience tens or hundreds of attacks per day. The pain is most commonly felt in the the mid face (second division) or the lower jaw (third division) of the Trigeminal Nerve. The pain is almost always limited to one side of the face (unilateral). For some unexplained reason the pain occurs slightly more frequently on the right side (60%). The first attacks of TN most often occur in middle or old age. The average age of onset is 50. The number of new cases of TN increases with age. The course of the disorder is variable. The pain may go away (go into remission) for unpredictable lengths of time weeks, months or even years. Thus in the beginning, TN pain is highly unpredictable. Over time, however, the periods of pain attacks (exacerbation) become more frequent and last longer. TN is a rare disease. TN may occur in 4 or 5 people out of a population of 100,000. TN seems to occur about equally in men and women. Some surveys contradict this notion and indicate that TN may occur up to twice as frequently in women.
