Chronic burning paresthesia in scrotum

For five years I have suffered from chronic burning paresthesia on the posterior side of my scrotum. Sometimes I also experience stinging and hyperesthesia on the skin of my penis. The paresthesia is predominantly, if not
exclusively, on my left side. Sitting for long periods of time seems to aggravate it, lying down seems to eventually lessen the pain, and when I wake up in the middle of the night to urinate, I don't notice the sensation at all. Neither do I notice it when I first wake up in the morning. During the first three years of this condition, the paresthesia would disappear for occasional two-three week periods. Then about a year and a half ago my condition improved and the sensation was easily tolerated until about a month ago, when it returned with a vengeance. When it is really bad, it get relief by sitting on an ice pack! Dermatologists have ruled out a skin pathology. Two to three years ago, a physician advised taking amitryptolin (spelling?), but

Dear CCF:
Nine months ago while on the medication Levaquin I experienced Paresthesias that then intensified with acute burning pain to my entire body and bone pain. I wrote to the Federal Drug Administration and on their listing of reported adverse drug reactions the Paresthesia Cases all said ongoing therapy next to them. Can you explain two things. First, is a Paresthesias that is ongoing for 9 months with acute burning pain really
small fiber neuropathy? That is, is a Paresthesia and a Small Fiber Neuropathy the same thing. Second, would a Paresthesia last 9 months and include not only intense burning pain but bone and joint pain along with stiffness to the body. Do Paresthesias cause stiffness to the body.

Dear CCF Doctors,
had hundreds of tests all negative except the SFN. Any ideas? We are all merely trying to get some help, but first we need to understand if a paresthesia implies a minor disturbing one in a while sensory thing, then how can people be receiving ONGOING THERAPY FOR 6 PLUS MONTHS OR MORE.
Do you have any ideas? We are just wanting to know is this forever! Any ideas PLEASE!!! Thanks for the question. I'm not sure how much light I can shed on your concern. Paresthesia is a fairly generic term that is used to refer to an abnormal tingling sensation, sometimes described as a pins and needles sensation. It does not define an etiology but is descriptive. The symptoms are usually thought to be due to injury to a peripheral nerve. Some patients with SFN will describe a burning pain more than a pins and needles sensation. As you know the treatment of SFN is inadequate at this point. If an offending agent can be found then it is removed but the symptoms may linger. Research continues in this area and I trust that your symptoms are not too diabeling. Good luck This information is provided for general medical education purpsoes only. Epineurium: careolar connective tissue surrounding the fasciculi.
Perineurium : sheath of connective tissue enclosing all the bundles of nerve fibres.
Endoneurium : thin connectival membrane ensheathing each nerve fibre. It is also called Henle's sheath or Key and Retzius's sheath. The endoneurium is profoundly joined to the Schwann cells which are its major constituent. Lastly, the endoneurium is closely connected to the perineurium giving a certain elasticity to the nerve bundle, which is extremely important for implantology.
When stretched, the elasticity of the epineurium and the two nerve fibres inside the fasciculi allow a degree of stretching that is tolerated by the nerve trunk, but the extent to which it can be extended depends on the elasticity of the perineurium.
Recent research has identified 20% as the maximum limit of extension, whereas 30% leads to structural laceration.

Thalamus Programming Worksheet: Side Effects: Paresthesia Patients may experience paresthesia that may be limited to one extremity or face and occur when the stimulation current spreads to the ventro-caudal thalamus (ventro-posterior lateral thalamus by other conventions)(See limited parethesias). Alternatively, the patient may experience paresthesias on the whole half of the body contralateral to the side of the DBS lead placement. (See extensive paresthesias). Limited Paresthesias This may occur if the DBS lead is placed too posteriorly. Often the orientation of the DBS lead is such that as the lead goes deeper, the tip moves more posteriorly. The following response options may be considered:
  • Limit the current spread by using bipolar rather than monopolar stimulation Choose another electrical contact for stimulation, which may move the current superiorly and, therefore, more anteriorly, away from the ventro-caudal thalamus. If the symptoms persist despite changing the stimulation parameters, it might be necessary to reposition the DBS lead. The physician may want to consider performing an MRI

I have had synptoms for a month exam results are as follows: MRI C2-3 normal; C3-4 small oncovertebral joint changes in addition to endplate changes, no sig neural foramen or spinal stenosis; C4-5 posterior endplate resulting in a diminished CSF signal anterior and posterior to the cord. Mild left and moderate right neural foramen narrowing due to uncovertebral joint changes; C5-6 large rt. posteriolateral endplate spur which extends to the lateral recess and and diminishes the the AP diameter of the right aspect of the spinal canal. There is also a mild uncovertebral joint change and as a result there is a bilateral moderate to marked neural foramen narrowing ; C7-T1 - Only mild posterior element degenerative change. No evidence of disc herniationbulge or spinal stenosis. Impressioin: mid level spondylosis, worse at D5-6 and C6-7 levels on the right with diminished AP diameterof the adjacent cord and significant neural foramen narrowing at these levels. No evidence of herniarion of the disc
