TMJ and muscle dysfunction syndrome with paresthesia : Report of three cases.

TMJ and muscle dysfunction syndrome refers to a collection of symptoms associated with functional disturbances of the temporomandibular joint and muscles. While the number of patients, suffered from this syndrome are increasing, its a etiologic factors are still doubtful. A number of adumbrated hypotheses about the primary causes of TMJ and muscle dysfunction have been suggested, but generally all such concepts reflect heterogeneous, multifactorial aetiological factors.
In this article, three patients suffering from TMF and muscle dysfunction syndrome with superficial numbness on the face were reported. The occlusal splint was used as an initial treatment to provide reversible therapy as well as a diagnostic appliance. It helped mandible to position itself in response to reduction of muscle activity and healing of joint, provided the splint must be properly adjusted. The adjustment should be done as often as necessary to accommodate for vertical as well as horizontal changes in condylar position.
The patients recovered from pain and numbness in a few weeks. It indicated that three patients were really suffered from TMJ and muscle dysfunction syndrome with superficial numbness.

Shoulder Range of Motion Changes with the Use of Integrative Manual Therapy Author: Suzanne Simons, MPT, IMP,C Abstract: The woman in this case study was 48 years old and complained of left shoulder pain and paresthesia with a limitation of motion. The patient reported that she had difficulty performing activities of daily living involving her left arm due to this pain. The woman received treatment with Integrative Manual Therapy for one session of two hours. Prior to treatment, the patient presented with limitations in range of motion of her left shoulder as well as postural deviations. After treatment was completed with a focus on upper quadrant, the patient presented with a significant improvements in posture and increased range of motion of that arm. The paresthesia was eliminated and the woman reported increased functioning. The woman regained full use of her left arm. Key Words: Shoulder, Paresthesia, Bone Bruise Background The patient is a 48 year old female with complaints of left shoulder pain with limited range of motion, making it difficult to perform activities of daily living such as reaching overhead and dressing. Mild paresthesia was reported in the 4th and 5th digits of the left hand. Symptoms became evident 24 hours after the patient played golf for the first time in twelve years. Pain intensity in the affected shoulder increased from 3 out of 10 to 5 out of 10 (on a Pain Analog Scale) within the ensuing 48 hours. The patient was seen for treatment 4 days post-injury.

This is a case which the same thing happened to me. The sensation returned fully in a couple weeks. I theorized that my sealer puff impinged on the IA nerve-if you look close on the film, you can see the canal in pretty close proximity to the apex of the tooth I treated. Dentist and nurse anesthetist-wife try frozen green peas to reduce massive subcutaneous emphysema following wisdom tooth surgery under general anesthesia; patient has permanent lingual paresthesia ; Florida settlement for $96,250. Plaintiff was a 24 year-old single woman when she first visited the general dentist with complaints referable to her lower third molar teeth. She was told that she should have the teeth extracted and she told the dentist that she wanted a general anesthetic. He offered to refer her to an oral surgeon or, if she was willing to wait for the dentist to receive something from the state, she could return to him and he would be able to do the surgery under general anesthesia. He was, at the time, awaiting receipt of an anesthesia/parenteral sedation permit from the Florida Board of Dentistry. More than six months later, he called the patient back into his office to schedule her surgery. The dentist's wife was a CRNA (nurse anesthetist) and would do the anesthesia. The dentist used a high-speed air turbine handpiece to drill away bone and caused a massive subcutaneous emphysema to develop in the submandibular space which dissected its way down to the clavicles. The patient was sent home and the dentist and his wife made a house call the next day, but did not send the patient to a hospital emergency room. The following day, after seeing just how massive the emphysema was on the previous day, the dynamic duo returned to the patient's house with five boxes of frozen green peas which they bought at a nearby convenience store; they placed the green peas on the patient's upper chest and neck in a futile effort to reduce the swelling, mistakenly believing the emphysema to be an inflammatory process receptive to cold compresses.

    Facial paresthesia is a general term that can be applied to a variety of abnormal sensations occurring spontaneously. Paresthesias may be associated with many of the anesthetic and painful facial syndromes discussed in this chapter. The responsible lesion usually involves the nerve endings or the nerve trunk. Occasionally, the lesion is in the central nervous system. Itching represents an unusual form of paresthesia. Dysesthesias represent an abnormal perception of a common exteroceptive stimulus.
    Itching The common sensation of itching deserves mention in our discussion of the trigeminal sensory system. According to modern concepts based upon experimental observation, the sensation of itching is mediated by the same nerves as the sensation of pain. The difference between these two sensations is a function of the degree of stimulation. Itching results from a mild stimulus and pain from a more severe stimulus. According to Shelley and Arthur (1957), the sensation of facial itching is transmitted to the brain by impulses that travel centrally via afferent unmyelinated C fibers. These investigators believe that the stimulus that generates the sensation of itching is chemical in nature and commonly arises in the skin as the result of the action of proteinases on the fine nerve endings of afferent

    Position Statement Concerning Paraformaldehyde-Containing Endodontic Filling Materials and Sealers The American Association of Endodontists (AAE) is dedicated to maintaining the highest quality of care in the practice of endodontics. As part of that dedication, the AAE actively supports the use of safe and effective materials in connection with root canal therapy. The AAE recognizes that legitimate differences of professional opinion may exist as to the "safest" or "most effective" material for a specific patient or specific circumstances. In recognition of those legitimate differences of professional opinion, the AAE does not endorse the use of specific materials. However, the AAE does recommend against the use of materials containing paraformaldehyde as they have proven to be both unsafe and ineffective. Public health concerns and litigation have made the AAE aware of a significant number of patients who have suffered injuries as a result of treatment with filling materials and sealers containing paraformaldehyde. Undoubtedly, there are many other patients who have also suffered injuries because of these materials, but whose injuries have not been publicly disclosed.
