What Is Reflex Sympathetic Dystrophy?
Reflex sympathetic dystrophy syndrome is a problem with the autonomic or sympathetic nervous system, the nerve system that controls the diameter of blood vessels.  How it happens is not known.  But as the nerves are damaged, they send the wrong signals to the brain.  These signals then send back pain messages to the affected area. The illness can cause:
  • sharp pain tenderness when touched changes in blood flow to the skin increased or decreased sweating muscle spasm
There are three stages to the disease. Stage one lasts about three months.  Its main symptom is pain, sometimes described as a burning feeling. Stage two happens after three or four months.  This is when swelling in the affected area can be seen.  It is also when many of the other symptoms listed below begin to appear.  The pain may get worse with changes in the temperature and it may spread to other areas.  X-rays may show that the bones have started to weaken. Stage three occurs after eight or nine months.  At this time, changes that cannot be reversed occur.  Muscle tissue is lost.  The joint begins to contract and motion is difficult.  The pain increases.

Hyperhydrosis and Reflex Sympathetic Dystrophy (RSD) Hyperhydrosis and reflex sympathetic dystrophy (RSD) are two disorders with a common cause and treatment. Hyperhydrosis is a commonly noted as sweaty palms and feet. Patients generally note increased sweating in the palms of their hands, feet and possibly also in the trunk. RSD is a painful condition involving one or more of the extremities. Both of these conditions result in an overactivity of the sympathetic nervous system. The sympathetic nervous system generally controls the "fight or flight" activities of the body. Treatment for both hyperhydrosis and RSD is injections or surgery. The surgery is a minimally invasive, transthoracic endoscopic sympathectomy. Our results have been extremely positive and most patients leave the hospital the following day.

incorporates 9867 REFLEX SYMPATHETIC DYSTROPHY Gabe Mirkin, M.D. Recent studies from France and Italy report a new treatment for reflex sympathetic dystrophy, severe pain and swelling that is associated with nerve pain in an arm or leg that may be caused by a weakening of bones (1, 9, 10). All people have two separate nervous systems in their bodies: a voluntary nervous system in which they decide to move an arm, leg or anything else and an involuntary nervous system over which they have little direct control, such as contraction of intestinal muscles, the opening and closing of the brown spot in the front of the eye to let in light, sweating and so forth. Damage to the involuntary nervous system on one arm or leg can be caused by a stroke, spinal cord injuries, shingles, heart attacks, surgery or trauma such as broken bones following an auto accident. First, you develop severe pain, swelling, sweating and excessive warmth of an extremity, followed by a later phase characterized by cool skin, loss of hair, severe pain forcing a person to avoid any movement of the extremity and then osteoporosis or loss of bone in that extremity. Until recently, the only treatment was to inject cortisone and an anaesthetic into the nerve roots of that extremity. Doctors never cured anyone with this treatment, but they still continued to do this. In 1989, researchers from Japan showed that the pain of reflex sympathetic dystrophy comes from weakened bone, not from nerves, so the cortisone injections really have no place in the management of this condition.

  • Mr. CUNNINGHAM. Mr. Speaker, I rise to recognize the promise of medical research. With the advancements in medical research being announced every day, the possibilities for improving the length and quality of life for all Americans appear impressive and unprecedented. We can maintain hope, buoyed by good science, that improved treatments and cures can be found for cancer, diabetes, AIDS, arthritis, Alzheimer's, Parkinson's and spinal cord injuries to name a few. However, to take full advantage of this possibility we must increase our funding for the National Institutes of Health and all federally funded medical research. I could marvel you with the achievements in medical research that I have seen in the last year through my role as a member of the Appropriations Subcommittee on Labor, Health and Human Services and Education. However, I would instead like to focus on those individuals that experience pain in their daily lives. The National Arthritis Foundation tells me that nearly one in six Americans will suffer from some form of arthritis and according to the American Chronic Pain Association, pain is part of the daily lives of one in three Americans. I am blessed to know a wonderful lady in San Marcos, California, Alfie Burns. Alfie is President of the Reflex Sympathetic Dystrophy Syndrome Association of California, serves as an Appeals Board Member on the California Department of Rehabilitation, is involved in her community and still has time to raise a family.

Reflex sympathetic dystrophy is the feeling of pain associated with evidence of minor nerve injury. Description Historically, reflex sympathetic dystrophy (RSD) was noticed during the civil war in patients who suffered pain following gunshot wounds that affected the median nerve (a major nerve in the arm). In 1867 the condition was called causalgia form the Greek term meaning "burning pain." Causalgia refers to pain associated with major nerve injury. The exact causes of RSD are still unclear. Patients usually develop a triad of phases. In the first phase, pain and sympathetic activity is increased. Patients will typically present with swelling ( edema ), stiffness, pain, increased vascularity (increasing warmth), hyperhydrosis, and x-ray changes demonstrating loss of minerals in bone (demineralization). The second phase develops three to nine months later, It is characterized by increased stiffness and changes in the extremity that include a decrease in warmth and atrophy of the skin and muscles. The late phase commencing several months to years later presents with a pale, cold, painful, and atrophic extremity. Patients at this stage will also have osteoporosis It has been thought that each phase relates to a specific nerve defect that involves nerve tracts from the periphery spinal cord to the brain. Both sexes are affected, but the number of new cases is higher in women, adolescents, and young adults. RDS has been associated with other terms such as Sudeck's atrophy, post-traumatic osteoporosis, causalgia, shoulder-hand syndrome, and reflex neuromuscular dystrophy.

Dear Readers: My last article on hyperthyroidism prompted some interesting responses. In that article, I mentioned I had an unusually difficult time finding any useful information on alternative treatment options for hyperthyroidism patients who wanted to avoid surgery or radioactive treatments to their thyroids (because of the increased risk of developing hypothyroidism). As usual, you came through in a big way. Here's an excerpt from one of the most hopeful letters I got: Hi Ruth, I would like to share my experience of hyperthyroidism. I've been able to control it naturally, and thought you may be interested. Once diagnosed with hyperthyroidism, I started to do some research. I was having daily headaches, sweating, night sweats, tremors, heart palpitations, confusion, etc. I would do one massage and be totally exhausted. The doctor suggested radioactive therapy. I gently explained to him that it didn't make sense to zap my thyroid and then try to bring the levels up again with medication. I guess I was trying to avoid being on medication the rest of my life.

Diagnosis: Reflex sympathetic dystrophy
Full history:
74-year old woman with chronic left ankle pain and no known history of trauma.
Radiopharmaceutical:
21.2 Tc-99m MDP i.v.
Findings:
Bone scintigraphy shows increased activity on radionuclide angiogram, blood pool, and delayed images throughout the left ankle and foot. The accompanying radiographs dated 6-15-95 reveal marked swelling of the soft tissues of the left foot and ankle as well as diffuse osteopenia. The combined scintigraphic and radiographic findings as well as the clinical history permit a diagnosis of reflex sympathetic dystrophy.
Discussion:
Reflex sympathetic dystrophy, also known as Sudeck¹s atrophy and causalgia, is a sympathetic mediated disorder of the extremities characterized by pain, stiffness, swelling, weakness, and skin changes, and vasomotor instability. Radiographically, soft tissue swelling and osteopenia are seen. Scintigraphically, there is increased activity in the affected limb during the radionuclide angiographic and immediate post injection images. Delayed scintigraphy reveals increased activity, diffusely in the hand foot. Treatment strategies include physical therapy and, in refractory cases, neurosurgically created dorsal root entry zone lesion.

Hypertension RSD (Reflex Sympathetic Dystrophy) "A fter many years of TMJ pain, I was diagnosed with RSD (Reflex Sympathetic Dystrophy) IN 2001. The many medications and steroid injections I was given while relieving pain seriously incapacitated me and did not cure my condition of constant pain, fatigue, depression and insomnia, and muscle spasms. During the summer of 2002, I moved to the Lehigh Valley area and sought the help of Chung-Hu Tao, an acupuncturist and herbalist.
    I found what he treated relieved many of my symptoms and enabled me to do activities I had not done in years. Although RSD and TMJ
    problems are something I will always have to live with, acupuncture has enabled me to resume a more normal life with many less medications (that have their own side effects) and much less pain. I t is clear to me that Dr. Tao's patient, caring, and informed approach is an excellent adjunct to the usual Western way of medicine. I believe it is a healthier way to deal with certain intractable medical problems and recommend to those with chronic problems to give a try."
